La sarcoidosis es una enfermedad granulomatosa sistémica de etiología desconocida que puede afectar pacientes de ambos sexos, de todas las edades y razas. Tiene un pico de incidencia en la tercera y cuarta década de vida. La fisiopatología de la enfermedad se basa en la exposición a antígenos ambientales en un paciente con predisposición genética generando en él una respuesta inflamatoria granulomatosa. Su presentación clínica cubre una amplia gama de manifestaciones, desde una afectación leve y limitada hasta daño a multiorgánico y muerte. Clásicamente el órgano más afectado es el pulmón, sin embargo, también llega a afectar órganos como la piel, el ojo, el hígado, el bazo, los ganglios linfáticos periféricos y el sistema nervioso central. El diagnóstico es basado en la clínica del paciente, que debe ser asociada a imágenes radiológicas y biopsia de tejido u órgano afectado, donde histológicamente se observan de forma típica, granulomas no caseificantes asociados a células gigantes. El tratamiento es individualizado para la necesidad del paciente, se puede dar un manejo expectante esperando resolución espontánea o manejo farmacológico con corticoesteroides.
La enfermedad de Kawasaki es una enfermedad que se manifiesta con una vasculitis aguda de vasos de pequeño y mediano calibre, esta afecta principalmente a niños menores de cinco años de edad. Tiene un pico de incidencia entre los 18 y 24 meses, con alta variabilidad geográfica, estacional y por etnia. Su etiología es desconocida, pero se postula que la exposición a un agente infeccioso desconocido produce una activación desmedida del sistema inmune en pacientes con susceptibilidad genética. El diagnóstico está basado en criterios clínicos que incluyen fiebre, inyección conjuntival bilateral, cambios en mucosa oral y/o labios, rash, cambios en las extremidades superiores e inferiores y linfadenopatía cervical no supurativa. El tratamiento se basa en la administración de inmunoglobulina intravenosa durante la fase aguda para disminuir la inflamación y prevenir eventos trombóticos. Las complicaciones más temidas son las relacionadas con el sistema cardiovascular como aneurisma de las arterias coronarias e infarto agudo de miocardio.
