Existen aproximadamente 2.1 millones de niños viviendo con el virus de inmunodeficiencia humana (VIH) a nivel mundial. La terapia antiviral altamente efectiva (TARAE) ha logrado disminuir de manera dramática la mortalidad relacionada a VIH tanto en niños como en adultos. Lo que en algún tiempo fue una infección progresiva y fatal es ahora una enfermedad crónica con mayor expectativa de vida. El daño directo por el VIH, la inflamación crónica, el envejecimiento prematuro del sistema inmune, las co-infecciones, la TARAE y el estilo de vida pueden provocar distintas alteraciones sistémicas. En los niños con VIH el tener un mayor tiempo de exposición al virus, un tratamiento más agresivo y mayor supervivencia ha hecho evidente la presentación de un sinnúmero de alteraciones endocrinológicas. Este artículo menciona las principales alteraciones metabólicas asociadas al VIH y la importancia de su detección en la población pediátrica.
