Uvod. Terapija bolesnika sa akutnim infarktom miokarda sa ST elevacijom (STEMI) predstavlja izazov, tako da se stalno traže nove mogućnosti za poboljšanje ishoda lečenja. Služba hitne medicinske pomoći (SHMP), glavni akter u prehospitalnom zbrinjavanju STEMI, primenjuje dvojnu antiagregacionu terapiju ((Morfin, kiseonik (O 2), Nitroglicerin, Aspirin) MONA + klopidogrel ili tikagrelor) u cilju povećanja reperfuzionog efekta. Cilj rada je bio da ispita učestalost primene dvojne antiagregacione terapije u prehospitalnom tretmanu STEMI kao i vreme od prijema poziva do predaje bolesnika u bolnicu. Metode. Retrospektivnim istraživanjem obuhvaćen je period od 01.01.2014. do 31.12.2017. godine. Dijagnoza je postavljena na osnovu anamneze, fizikalnog pregleda i elektrokardiografskog nalaza. Kod bolesnika, kod kojih je postavljena dijagnoza STEMI, praćena je primena dvojne antiagregacione terapije. Mereno je i vreme od trenutka pozivanja 194 do predaje bolesnika u bolnicu. Rezultati. Od 8.843 bolesnika sa akutnim koronarnim sindromom (AKS) kod kojih je intervenisala SHMP kod 2.982 postavljena je dijagnoza STEMI. Procenat primenjene dvojne antiagregacione terapije u toku 2014. godine iznosio je 40,29%, dok je u 2017. godini porastao na 70,01% (60,38% pacijenata je dobilo MONA i tikagrelor, a 9,63% MONA terapiju i klopidogrel). Žene u kasnijim godinama (70,17±13,05) imaju STEMI u odnosu na muškarce (61,89±13,43). Od pozivanja broja 194 do bolnice, 75% pacijenata stigne za 65,77 min. Zaključak. U posmatranom periodu primećeno je značajno povećenje procenta primenjene dvojne antiagregacione terapije kod STEMI bolesnika. Vreme transporta bolesnika sa STEMI je važna karika prehospitalnog tretmana. Kontinuirana edukacija timova HMP ima za cilj unapređenje dijagnostike i lečenja u prehospitalnim uslovima. Kratak sadržaj Akutna respiratorna insuficijencija (ARI) predstavlja po život opasno, hitno stanje u kojem usled neadekvatne disajne funkcije dolazi do hipoksemije i hiperkapnije. Kako je disanje, kao jedna od osnovnih vitalnih funkcija, uslovljeno međusobnom interakcijom više organa i organskih sistema i respiratorna insuficijencija može biti posledica različitih bolesti i patoloških stanja. Lekari hitne medicinske pomoći moraju biti osposobljeni za rano prepoznavanje, ranu primenu dijagnostičkih i terapijskih mera do i tokom transporta ovako teškog bolesnika u jedinicu intenzivne nege. U ovom radu su sistematično prikazani osnovni etiološki aspekti, klinička slika, ABCD pregled, postavljanje dijagnoze i načini prehospitalnog zbrinjavanja ARI.
Syncope is a reversible loss of consciousness occuring as a result of cerebral hypoperfusion and is characterized by a quick onset, short duration and a spontaneous and complete recovery. It is often the reason for calling 194 and makes up 3% of all interventions in the field. Regarding what caused it, it can be: reflex syncope (neuronal cause), caused by orthostatic hypotension and cardiovascular syncope. It can be caused by anything from benign factors to life threatening conditions. Patients with vasovagal syncope are not expected to have a resulting rise in morbidity and mortality, but patients with cardiovascular syncope have a 30% greater risk of sudden death during the following year. Initial prehospital evaluation of a patient with transitory loss of consciousness includes taking medical history, performing a physical examination, measuring blood pressure and recording an ECG. The initial evaluation defines the cause of the syncope in 23-50% od the patients. In most cases, minimal prehospital treatment is needed. Therapy depends on the cause of the syncope and risk assesment. It is necessary to evaluate the airway, breathing and circulation and treat life-threatening conditions. The patient should be prepared for transport and transported to the hospital. The patient's vital functions should be continuously monitored.
Kratak sadržaj Uvod Hipoglikemija predstavlja česti neželjeni događaj kod osoba sa dijabetes melitusom tipa 1 (DM1) ili dijabetes melitusom tipa 2 (DM2) koji su lečeni insulinom i/ili i insulinskim sekretagogom (sulfonilureja ili glinidi). Cilj Istraživanje je sprovedeno sa ciljem da utvrdi učestalost hipoglikemije kod pacijenata sa dijabetesom koja zahteva angažovanje ekipe hitne medicinske pomoći (HMP), način zbrinjavanja i broj pacijenata koji su prevezeni u bolnicu. Metod rada Sprovedno je prospektivno istraživanje za period od mesec dana (april 2015.god.) u Gradskom zavodu za hitnu medicinsku pomoć (GZZHMP) u Beogradu u koje su uključeni pacijenti sa dijagnozom Diabetes mellitus, Hyperglycaemia ili Hypoglycaemia kod kojih je intervenisala HMP. Podaci su dobijeni uvidom u lekarski izveštaj koji se popunjava za svaku obavljenu intervenciju i analizirani deskriptivnim statističkim metodama. Rezultati Od ukupno 8043 vraćena lekarska izveštaja, na 116 (1.44%) registrovane su navedene dijagnoze, a od tog broja je kod 63 (54,31%) pacijenta hipoglikemija bila razlog za intervenciju ekipe hitne pomoći. Vrednost glikemije manja ili jednaka 2.2 mmol/l izmerena je kod 41 pacijenta. Kod 61 (96.83%) pacijenta, u cilju korekcije hipoglikemije, primenjena je 50% glukoza. Prosečno vreme čekanja poziva kod dispečera iznosilo je 4.52±5.55 (Med=2.68 IKO (1.06; 7.36)) minuta. U 90.48% slučajeva pacijenti sa hipoglikemijom, u našem istraživanju, zbrinuti su na mestu intervencije. Zaključak Hipoglikemija je često urgentno stanje kod pacijenata koji boluju od dijabetesa. Ozbiljnu hipoglikemiju u našem istraživanju imalo je 65.08% pacijenata. U cilju korekcije hipoglikemije korišćena je 50% glukoza, a samo 9.52% pacijenata prevezeno je u bolnicu.
Gradski zavod za hitnu medicinsku pomoć, Beograd, Srbija SAŽETAK Infarkt miokarda bez opstrukcije koronarnih arterija (MINOCA) je heterogeni klinički entitet koji se karakteriše kliničkim znacima akutnog infarkta miokarda (AMI), odsustvom opstruktivne bolesti koronarnih arterija (≥50% stenoza) i očiglednog uzroka za akutnu prezentaciju u vreme angiografije. Prevalenca MINOCA-e se kreće od 1% do 14% kod pacijenata sa dijagnozom AMI. Češći je kod žena kao i kod pacijenata koji se prezentuju kao infarkt miokarda bez elevacije ST-segmenta (NSTEMI). Iako zavisi od uzroka, prognoza MINOCA je ozbiljna, sa jednogodišnjim mortalitetom od oko 3,5%. Različiti patofiziološki mehanizmi, odgovorni za MINOCA uključuju disrupciju plaka, spazam koronarnih arterija, spontanu koronarnu disekciju, koronarnu trombozu/emboliju i koronarnu mikrovaskularnu disfunkciju. Utvrđivanje osnovnih uzroka MINOCA treba da usmeri lekara ka izboru optimalnog lečenja. The prevalence of MINOCA in patients with the diagnosis of AMI is between 1% and 14%. It is more common in women and patients presenting as a non-ST elevation myocardial infarction (NSTEMI). Although dependant on the cause, the prognosis of MINOCA is severe with one-year mortality at about 3.5%. Various patophysiological mechanisms that cause MINOCA include: plaque disruption, coronary artery spasm, spontanious disection of coronary arteries, coronary thrombosis/embolism and coronary microvascular dysfunction. Defining the cause of MINOCA enables the doctor to choose the optimal form of treatment.
