Method The Clinical Practice Research Datalink records all medical consultations, referrals and diagnoses in primary care for 6% of the British population. Using this register we identified 1348 working-aged patients who had consulted medical services over a 20-year period for workplace injury (cases) and 6652 matched controls. Risks were assessed in relation to visual impairment, common eye diseases, hearing loss, perforated ear drum, non-acute otitis media, and disorders of balance, using conditional logistic regression. Results In all, 173 subjects had had an eye problem before the date of injury consultation (index date), 793 an ear problem (including 336 with impaired hearing and 482 with non-acute otitis media), and 266 a disorder of balance. No associations were found with specific eye diseases or perforation of the ear drum, but odds ratios (ORs) were moderately elevated for eye and ear problems more generally, and higher still in relation to blindness or partial sight (OR 1.90) and non-acute otitis media (OR 2.03). The OR for consulting with disorder of balance within the 12 months immediately preceding injury consultation was 1.81 (95% CI 1.03-3.17). Conclusions Problems of vision, impairments of hearing, and disorders of balance may all carry a moderately increased risk of occupational injury. BackgroundBreast cancer is the leading cause of cancer morbidity among Shanghai and Hong Kong women, which contributes to 20–25% of new female cancer incidents. This study aimed to describe the temporal trend of breast cancer and interpret the potential effects on the observed secular trends.MethodsCancer incident data were obtained from the cancer registries. Age-standardized incidence rate was computed by the direct method using the World population of 2000. Average annual percentage change (AAPC) in incidence rate was estimated by the Joinpoint regression. Age, period and cohort effects were assessed by using a log-linear model with Poisson regression.ResultsDuring 1976–2009, an increasing trend of breast cancer incidence was observed, with an AAPC of 1.73 [95% confidence interval (CI): 1.54–1.92)] for women in Hong Kong and 2.83 (95% CI, 2.26–3.40) in Shanghai. Greater upward trends were revealed in Shanghai women aged 50 years old or above (AAPC = 3.09; 95% CI, 1.48–4.73). Using age at 50 years old as cut-point, strong birth cohort effects were shown in both pre- and post-menopausal women, though a more remarkable effect was suggested in Shanghai post-menopausal women. No evidence for a period effect was indicated.ConclusionsIncidence rate of breast cancer has been more speedy in Shanghai post-menopausal women than that of the Hong Kong women over the past 30 years. Abdominal wall necrotising fasciitis secondary to fish bone ingestion is extremely rare. We present a case of ingested fish bone complicated with self-sealing small bowel perforation and abdominal wall necrotising fasciitis. Following principles of necrotising fasciitis, a high index of suspicion led to early diagnosis and early treatment. The patient enjoyed a good recovery.
