Objective: Understanding the practice of reporting adverse events by health professionals. Method: A qualitative case study carried out in a teaching hospital with participants of the Patient Safety Center and the nursing team. The collection took place from May to December 2015, and was conducted through interviews, observation and documentary research to treat the data using Content Analysis. Results: 31 professionals participated in the study. Three categories were elaborated: The practice of reporting adverse events; Barriers in the effective practice of notifications; The importance of reporting adverse events. Conclusion: Notification was permeated by gaps in knowledge, fear of punishment and informal communication, generating underreporting. It is necessary to improve the interaction between leaders and professionals, with an emphasis on communication and educational practice.
Buscou-se identificar experiências de Sistematização da Assistência de Enfermagem (SAE) na rotina diária de trabalho da enfermagem em instituições brasileiras. Realizou-se busca eletrônica na Biblioteca Virtual de Saúde nas bases “Scielo, Lilacs e Bdenf” por meio das palavras “Sistematização Enfermagem” e “Processo Enfermagem”. Selecionaram-se trabalhos de campo realizados em instituições nacionais no período entre 1979 e 2009, totalizando 71 estudos. As principais temáticas foram: estudo de caso 28%, elaboração de instrumento de coleta 21% e perfil de diagnósticos de enfermagem 20%. Apenas 32% demonstraram a aplicação das cinco etapas do Processo de Enfermagem. Predominam experiências na atenção hospitalar 92%. As principais teorias de enfermagem foram: Necessidades Humanas Básicas 36% e Déficit do Auto Cuidado 17%. Conclui-se que apesar da resolução do COFEN determinando a obrigatoriedade da SAE em todas as instituições de saúde brasileiras, a implantação e manutenção dessa metodologia ainda se desenvolvem de forma lenta e gradativa.
Identificar as ações realizadas pela equipe de SCIH e Gestão da Qualidade para alcançar a meta redução do risco de infecções relacionadas à assistência à saúde. Pesquisa qualitativa, exploratória e descritiva, realizada em duas instituições hospitalares. Houve triangulação de dados com entrevistas, observação e análise documental. Para a análise dos dados utilizou-se a análise de conteúdo. As ações mais realizadas foram educação permanente, identificação e correções de problemas em procedimentos, busca ativa, vigilância e higienização das mãos. Foi possível identificar a ocorrência de eventos adversos. A comunicação e os problemas estruturais aparecem como dificultadores no alcance da meta. As ações identificadas foram práticas tradicionais e muitas vezes ineficientes. A prevenção e o controle de infecções ainda são desafios para as instituições de saúde. Há necessidade de adoção de estratégias inovadoras para alcançar a meta de redução do risco de infecção relacionada à assistência à saúde.
<p>Objetivo: compreender a implementação do Núcleo de Segurança do Paciente em uma Unidade de Pronto Atendimento na perspectiva dos enfermeiros. Método: estudo de caso qualitativo, realizado em uma Unidade de Pronto Atendimento em um município da Região Centro-Oeste de Minas Gerais, Brasil, com a participação de 12 enfermeiros. Os dados foram coletados de setembro a novembro de 2019, por meio de entrevista guiada por roteiro semiestruturado, Técnica do Gibi e observação. Para a análise adotou-se a análise de conteúdo. Resultados: emergiram duas categorias temáticas: Desafios da implementação do Núcleo de Segurança do Paciente e Facilidades da implementação do Núcleo de Segurança do Paciente. Conclusão: foram identificados fragilidades e desafios que impactam no processo de implementação do núcleo e consequentemente no cuidado ao paciente, o que pode servir para subsidiar a implantação de novos Núcleos de Segurança do Paciente em serviços de saúde.</p><p>Descritores: Serviços Médicos de Emergência.
