Os autores relatam uma microepidemia de histoplasmose pulmonar, com cinco crianças que desenvolveram a doença em um período de 7 a 14 dias após a limpeza de um forno desativado para produção de carvão vegetal. Todas apresentaram quadro de febre alta persistente, tosse seca, astenia e anorexia, com 28 dias de evolução. Quando buscaram atendimento médico, uma delas encontrava-se taquipnéica, febril, com hepatomegalia e palidez cutânea, estando as restantes em regular estado geral e já sem febre. As radiografias de tórax demonstravam, em todos os casos, infiltrados reticulonodulares grosseiros, difusos e bilaterais, além de linfonodomegalias hilares. As tomografias computadorizadas evidenciaram pequenos nódulos difusos, com distribuição aleatória, além das linfonodomegalias. Os diagnósticos foram confirmados por meio da imunodifusão em gel para Histoplasma capsulatum, que foi positiva em todas as amostras pareadas coletadas com 15 dias de intervalo. Apenas uma criança necessitou de internação, por causa de importante queda no estado geral, sendo realizado tratamento de suporte e observação. Todas as crianças evoluíram com melhora clínica, sem o uso de antifúngicos, e foram submetidas a tomografias de controle após cerca de 50 dias, que demonstraram importante regressão das lesões.
A ruptura traumática do diafragma é uma condição incomum, porém cada vez mais freqüentemente diagnosticada pela tomografia computadorizada, especialmente pela técnica helicoidal associada às reconstruções multiplanares, possibilitando a adoção de conduta terapêutica cirúrgica rápida e eficiente. Os autores estudaram seis pacientes com ruptura traumática do diafragma submetidos a tomografia computadorizada, que demonstrou herniação de estruturas abdominais para o interior do tórax através de área de ruptura na hemicúpula frênica esquerda em quatro casos; os outros dois pacientes foram submetidos a procedimento cirúrgico por trauma abdominal associado, que demonstrou lesões diafragmáticas, sem evidência de herniação na tomografia computadorizada.
Objective: To describe high-resolution computed tomography findings in several diseases that run their course with the "crazy-paving" pattern. Materials and Methods: The present study has evaluated seven patients with diagnoses of alveolar proteinosis, bronchioloalveolar carcinoma, lipoid pneumonia and pneumocystosis, correlating tomographic and pathological findings. Results: In the cases of pneumocystosis, the alveolar spaces were filled with foamy material where parasitic organisms intermingled with surfactants, fibrin and cell debris were observed. In the cases of bronchioloalveolar carcinoma, the septa were thickened by associated fibrosis or lymphangitis, with the alveolar walls internally lined with tumor cells, and production of mucus. In the patient with alveolar proteinosis, the septa were thickened by edema, with alveolar filling determined by lipoprotein. In the patient with mineral oil aspiration, the histopathological findings included alveolar septa thickening caused by cell proliferation, with presence of fat vacuoles in alveolar septa. Conclusion: The crazy-paving pattern offers a wide range of differential diagnoses and must be evaluated in conjunction with clinical findings. Materiais e Métodos: Foram estudados sete pacientes, com diagnósticos de proteinose alveolar, carcinoma bronquíolo-alveolar, pneumonia lipídica e pneumocistose, e é discutida a correlação dos aspectos tomográficos com os achados anatomopatológicos. Resultados: Nos casos de pneumocistose, os espaços alveolares estavam cheios de material espumoso, no qual eram encontrados parasitas de permeio a surfactante, fibrina e restos celulares. Nos casos de carcinoma bronquíolo-alveolar, os septos estavam espessados por fibrose ou por linfangite associada, com células tumorais revestindo internamente as paredes alveolares, e produção de muco. No paciente com proteinose alveolar foram observados septos espessados por edema, com enchimento alveolar determinado por material lipoproteico. No paciente com aspiração de óleo mineral, os achados histopatológicos foram de espessamento dos septos alveolares determinado por proliferação celular, observando-se vacúolos de gordura no interior dos septos alveolares. Conclusão: O padrão de pavimentação em mosaico oferece um amplo diagnóstico diferencial, necessitando ser avaliado em conjunto com os dados clínicos. A proposta do trabalho é apresentar os achados radiológicos observados na tomografia computadorizada de alta resolução do tórax de dois pacientes com pneumonia intersticial linfocítica e correlacioná-los com os aspectos anatomopatológicos, obtidos a partir de biópsias a céu aberto. Um dos pacientes mostrou, na tomografia, basicamente opacidades em vidro fosco difusas, e o outro tinha infiltração ao longo das bainhas conjuntivas peribroncovasculares. Na anatomopatologia o padrão predominante foi o de infiltração intersticial, especialmente ao longo dos septos alveolares, por linfócitos policlonais. O estudo mostrou estreita correlação entre os achados anatomopatológicos e aqueles observados na tomografia computadorizada de alta resolução do tórax. Unitermos: Pneumonia intersticial linfocítica. Lymphocytic interstitial pneumonia: correlation of high-resolution computed tomography findings with anatomopathology.The purpose of this study is to report the main high-resolution computed tomography findings of two patients with lymphocytic interstitial pneumonia. High-resolution computed tomography findings were correlated with pathology findings of material obtained from open biopsies. One patient had diffuse ground glass opacities seen on high-resolution computed tomography whereas the other patient had peribroncovascular thickening. Anatomopathology studies showed that the main pattern was polyclonal lymphocyte interstitial infiltration, particularly along alveolar septa. The current study demonstrated a close correlation between high-resolution computed tomography and anatomopathology findings.
Objective: To alert the clinician suspecting of an autoimmune disorder like Sjogren Syndrome for children affected by recurrent parotitis, particularly in case of bilateral involvement in order to timely recognize this disease and to facilitate the treatment and the screening for complications. Materials and Methods:All patients with recurrent parotid swelling between 0 and 14 years, 7 in total referred to the Multidisciplinary Department of Medical and Dental Specialities, Division of Oral and Maxillofacial Surgery (Università degli Studi della Campania Luigi Vanvitelli, Naples, Italy) in the last 2 years, were subjected to clinical, serological, microbiological and ultrasound screening for excluding tumors or infectious diseases. In patients thus selected were diagnosed with Juvenile Recurrent Parotitis and were treated with sialoendoscopy (intraductal wash of saline solution and steroids) or other anti inflammatory drugs. From the formulation of diagnosis, patients were subjected to careful follow-up checks with 6, 12 and 18 months during which they proceeded to repeat initial serological screening if the endoscopic or systemic therapy became ineffective.Results: Patient 1: Following interventional sialoendoscopy treatment with mechanical dilation and washing with physiological solution and cortisone, howed remission at 18 months. Patient 2: Following interventional sialoendoscopy treatment with mechanical dilation and washing with physiological solution and cortisone showed remission at 18 months. Patient 3: She took prednisone and hydroxychloroquine, with complete resolution of parotid swelling and no further recurrences. Patient 4: Following endoscopic treatment with mechanical dilation and washing with physiological solution and cortisone did not show relapse after 18 months. Patient 5: She took hydroxychloroquine 12 months later with resolution of parotitis symptoms. Patient 6: Following interventional sialoendoscopy treatment with mechanical dilation and washing with physiological solution and cortisone showed remission at 18 months. Patient 7: Following the interventional sialoendoscopy treatment with mechanical dilation and washing with physiological solution and cortisone showed remission of relapse at 18 months.Conclusions: Recurrent parotitis should alert the clinician to the possibility of pSS especially if it does not respond to treatment with anti-inflammatory therapy and sialoendoscopic washing.
