CEUS represents a useful method in clinical practice for differentiating between malignant and benign FLLs detected on standard ultrasonography, and the results of this study are in concordance with previous multicenter studies: DEGUM (Germany) and STIC (France).
"Objective. This narrative review sought to identify the most frequently used types of exercises in cardiopulmonary rehabilitation programs and to verify the level of efficiency and feasibility of this treatment method in cardiovascular diseased patients. Methods. We conducted research via Pubmed over two months using specific keywords related to cardiorespiratory rehabilitation, cardiovascular rehabilitation, endurance exercises, and respiratory exercises. In addition, we included studies that involved physical training for patients diagnosed with coronary artery disease, chronic heart failure, valvular disease or valvular surgery, asthma, and chronic obstructive pulmonary disease. Results. A total of fifteen articles were included in this narrative review. Eight studies investigated the effect of aerobic training, alone or combined, in the rehabilitation process for patients with cardiovascular disease and/or pulmonary disease. It showed that this type of training is the most recommended and used for these specific pathologies. Seven studies showed the effects of the resistance training and progressive resistance training, alone or combined, and proved the importance of their integration in patients with skeletal muscle weakness and altered functional capacity. High-intensity interval training was not superior to moderate-intensity continuous training in improving aerobic capacity and changing ventricular remodeling in patients with chronic heart failure. However, the results were promising in patients with coronary artery disease. Combined training had the best results in improving the aerobic capacity, muscle strength, functional capacity, ventilatory responses, and enhancing the quality of life for cardiovascular patients. Conclusions. Combined training was found superior to aerobic training, resistance training, respiratory training, or inspiratory muscles training applied alone as a treatment in cardiopulmonary rehabilitation programs. Thus, every rehabilitation program should be individualized for every patient to increase the effort capacity using aerobic training and functional capacity of the skeletal muscles by increasing the muscle mass (hypertrophy) as an effect of resistance training or progressive resistance training "
Hypertension poses a significant burden in the general population, being responsible for increasing cardiovascular morbidity and mortality, leading to adverse outcomes. Moreover, the association of hypertension with dyslipidaemia, obesity, and insulin resistance, also known as metabolic syndrome, further increases the overall cardiovascular risk of an individual. The complex pathophysiological overlap between the components of the metabolic syndrome may in part explain how novel antidiabetic drugs express pleiotropic effects. Taking into consideration that a significant proportion of patients do not achieve target blood pressure values or glucose levels, more efforts need to be undertaken to increase awareness among patients and physicians. Novel drugs, such as incretin-based therapies and renal glucose reuptake inhibitors, show promising results in decreasing cardiovascular events in patients with metabolic syndrome. The effects of sodium-glucose co-transporter-2 inhibitors are expressed at different levels, including renoprotection through glucosuria, natriuresis and decreased intraglomerular pressure, metabolic effects such as enhanced insulin sensitivity, cardiac protection through decreased myocardial oxidative stress and, to a lesser extent, decreased blood pressure values. These pleiotropic effects are also observed after treatment with glucagon-like peptide-1 receptor agonists, positively influencing the cardiovascular outcomes of patients with metabolic syndrome. The initial combination of the two classes may be the best choice in patients with type 2 diabetes mellitus and multiple cardiovascular risk factors because of their complementary mechanisms of action. In addition, the novel mineralocorticoid receptor antagonists show significant cardio-renal benefits, as well as anti-inflammatory and anti-fibrotic effects. Overall, the key to better control of hypertension in patients with metabolic syndrome is to consider targeting multiple pathogenic mechanisms, using a combination of the different therapeutic agents, as well as drastic lifestyle changes. This article will briefly summarize the association of hypertension with metabolic syndrome, as well as take into account the influence of antidiabetic drugs on blood pressure control.
The administration of an anticoagulant in patients with liver disease (nonalcoholic steatohepatitis—NASH, nonalcoholic fatty liver disease—NAFLD, chronic hepatitis, or cirrhosis) who have an indication (atrial fibrillation, venous thrombosis, or pulmonary embolism) is challenging because there is an imbalance between thrombosis and bleeding. There is a need to focus our attention on preventing risk factors because diabetes, obesity, dyslipidemia, smoking, and sedentary behavior are risk factors for both NASH/NAFLD and AF, and these patients require anticoagulant treatment. Patients with advanced liver disease (Child–Pugh C) were excluded from studies, so vitamin K antagonists (VKAs) are still recommended. Currently, VKAs are recommended for other conditions (antiphospholipid syndrome, mitral valve stenosis, and mechanical valve prosthesis). Amongst the patients under chronic anticoagulant treatment, especially for the elderly, bleeding as a result of the improper use of warfarin is one of the important causes of emergency admissions due to adverse reactions. DOACs are considered to be efficient and safe, with apixaban offering superior protection against stroke and a good safety profile as far as major bleeding is concerned compared to warfarin. DOACs are safe in the Child–Pugh A and B classes (except rivaroxaban), and in the Child–Pugh C class are contraindicated. Given that there are certain and reliable data for chronic kidney disease regarding the recommendations, in liver function impairment more randomized studies must be carried out, as the current data are still uncertain. In particular, DOACs have a simple administration, minimal medication interactions, a high safety and effectiveness profile, and now a reversal agent is available (for dabigatran and idarucizumab). Patients are also statistically more compliant and do not require INR monitoring.
Resistance exercise training is well documented as having cardiovascular benefits, but paradoxically, it seems to increase arterial stiffness, favoring the development of high blood pressure. The present study investigates the potential effects of oral supplementation with arginine in healthy individuals performing exercise resistance training. We studied 70 non-smoking male subjects between the ages of 30 and 45 with normal or mildly increased blood pressure on ambulatory monitoring (for 24 h) and normal blood samples and echocardiography, who performed regular resistance exercise training for at least five years with a minimum of three workouts per week. They were divided into two groups in a random manner: 35 males were placed in the arginine group (AG) that followed a 6-month supplementation of their regular diets with 5 g of oral arginine powder taken before their exercise workout, and the control (non-arginine) group (NAG) consisted of 35 males. All subjects underwent body composition analysis, 24 h blood pressure monitoring and pulse wave analysis at enrollment and at six months. After six months of supplementation, blood pressure values did not change in the NAG, while in the AG, we found a decrease of 5.6 mmHg (p < 0.05) in mean systolic blood pressure and a decrease of 4.5 mmHg (p < 0.05) in diastolic values. There was also a 0.62% increase in muscle mass in the AG vs. the NAG (p < 0.05), while the body fat decreased by 1% (p < 0.05 in AG vs. NAG). Overall, the AG gained twice the amount of muscle mass and lost twice as much body fat as the NAG. No effects on the mean weighted average heart rate were recorded in the subjects. The results suggest that oral supplementation with arginine can improve blood pressure and body composition, potentially counteracting the stress induced by resistance exercise training. Supplementation with arginine can be a suitable adjuvant for these health benefits in individuals undertaking regular resistance training.
The COVID-19 pandemic has produced social and economic changes that are still affecting our lives. The coronavirus is proinflammatory, it is replicating, and it is quickly spreading. The most affected organ is the lung, and the evolution of the disease can degenerate very rapidly from the early phase, also known as mild to moderate and even severe stages, where the percentage of recovered patients is very low. Therefore, a fast and automatic method to detect the disease stages for patients who underwent a computer tomography investigation can improve the clinical protocol. Transfer learning is used do tackle this issue, mainly by decreasing the computational time. The dataset is composed of images from public databases from 118 patients and new data from 55 patients collected during the COVID-19 spread in Romania in the spring of 2020. Even if the disease detection by the computerized tomography scans was studied using deep learning algorithms, to our knowledge, there are no studies related to the multiclass classification of the images into pulmonary damage stages. This could be helpful for physicians to automatically establish the disease severity and decide on the proper treatment for patients and any special surveillance, if needed. An evaluation study was completed by considering six different pre-trained CNNs. The results are encouraging, assuring an accuracy of around 87%. The clinical impact is still huge, even if the disease spread and severity are currently diminished.
Coronary artery anomalies may occur during embryogenesis and can lead to changes in the vascularization of the heart, possible ischemia, and an increased risk of sudden death. A retrospective study was conducted with the aim of assessing the prevalence of coronary anomalies in a Romanian sample of patients, investigated with computed tomography angiography for coronary artery disease. The objectives of the study were to identify the anomalies of the coronary arteries and to conduct an anatomical classification according to Angelini. The study also consisted of evaluations regarding coronary artery calcification in the sample of patients by the Agatston calcium score and assessments regarding the presence of cardiac symptoms and their association with coronary abnormalities. The results showed a prevalence of coronary anomalies of 8.7%, of which 3.8% were origin and course anomalies and 4.9% were coronary anomalies with intramuscular bridging of the left anterior descending artery. Recommendations for practice include the widespread use of coronary computed tomography angiography for the diagnosis of coronary artery anomalies and coronary artery disease in larger patient groups and encouraging this investigation across the country.
