El sarcoma sinovial pleuropulmonar primario es una entidad rara y agresiva. Existen pocos trabajos publicados de esta patología en especial durante el embarazo. Clínicamente presentan síntomas como dolor torácico, disnea, tos seca, febrícula; hemotórax, hidrotórax o neumotórax. El diagnóstico requiere una combinación de factores clínicos, radiológicos, patológicos, inmunohistoquímicos y moleculares. La translocación equilibrada SS18-SSX (previamente llamado SYT-SSX) entre los cromosomas X y 18, t (X; 18) (p11.2; q11.2) se encuentra presente el 80-90% de los casos y se obtiene mediante FISH o RT-PCR, se considera el "Gold estándar" si está disponible. El manejo es quirúrgico, seguido de radioterapia y quimioterapia en dependencia del tamaño del tumor, bordes quirúrgicos de resección y número de mitosis. Describimos el caso de una mujer de 21 años, con diagnóstico de sarcoma sinovial pleuropulmonar primario durante el embarazo y los desafíos terapéuticos involucrados.
