O líquen plano oral (LPO) é um processo inflamatório crônico mediado por células T que acomete a mucosa bucal ou pele, com incidência maior em mulheres adulta, tendo uma prevalência global baixa em torno de 1,01%. Normalmente assintomáticos, tem aspecto clínico diverso, podendo se apresentar em forma de bolhas, placas, pápulas, atróficas, erosiva ou reticular. Em alguns casos pode se apresentar de forma sintomática, com sensação de queimação e desconforto. Para um diagnóstico assertivo do LPO, é necessário o exame de biopsia, sendo os aspectos histopatológicos caracterizados por hiperqueratose, cristas epiteliais ausentes ou hiperplásicas, degeneração hidrópica da camada basal, presença de corpos hialinos e infiltrado inflamatório crônico na camada epitelial basal e camadas adjacentes, sendo em alguns casos necessário diagnóstico diferencial de outras lesões brancas da cavidade bucal. O objetivo do presente estudo é relatar um caso clínico muco-cutâneo de líquen plano acometendo paciente do gênero masculino, leucoderma, sexo masculino, 58 anos, onde a condução do tratamento e a evolução da lesão foi decisiva para se firmar o diagnóstico diferencial de líquen plano da leucoplasia. Os autores concluem que o diagnóstico diferencial de lesões brancas da cavidade bucal nem sempre é fácil, exigindo do profissional um conhecimento profundo das patologias e o discernimento de fazer o diagnóstico diferencial de lesões com similaridades, de modo a diagnosticar e tratar as lesões de forma adequada.
