IntroduzioneL'osteomalacia è una osteopenia metabolica demineralizzante caratterizzata dalla presenza di tessuto osteoide non calcificato in misura superiore alla norma. Esistono sostanzialmente tre categorie etiologiche di osteomalacia: le forme vitamina D-dipendenti, quelle correlate ad alterazioni del metabolismo del fosfato e quelle dove non sono presenti disturbi del metabolismo della vitamina D e del fosfato. Tra queste ultime rientra una rara sindrome chiamata osteomalacia oncogenica ("tumor-induced osteomalacia", TIO). La TIO è una rara patologia paraneoplastica nella quale il tumore causale, in genere di origine mesenchimale, produce un fattore circolante che è in grado di determinare un quadro clinico di osteomalacia. Il meccanismo patogenetico della sindrome è la produzione, da parte della neoplasia, di un fattore fosfaturico che agisce a livello del tubulo renale, determinando una riduzione del riassorbimento tubulare di fosfati con conseguente perdita renale di fosfati ("renal phosphate leak"). Il primo fattore fosfaturico a essere identificato è stato il "fibroblast growth factor"-23 (FGF-23). Recenti studi avrebbero individuato nel FGF-23 la sostanza responsabile dell'inibizione sodiodipendente del riassorbimento dei fosfati a livello renale da cui dipendono l'ipofosfatemia e i ridotti livelli di 1,25 diidrossivitamina D, "marker" caratteristici del quadro clinico che contraddistingue la TIO. Non tutti i pazienti con TIO presentano però un aumento del livello di FGF-23, suggerendo la possibile implicazione anche di altri fattori. A tutt'oggi sono state identificate altre due sostanze con caratteristiche analoghe al FGF-23: MEPE (matrix extra cellular phosphoglycoprotein) e FRP4 ("secreted fizzled-related protein" 4): queste proteine nel loro insieme sono state definite fosfatonine. Caratteristicamente il quadro clinico presenta una remissione completa dopo asportazione della neoplasia, dimostrando che è secondario all'increzione di un fattore umorale da parte del tumore. Si tratta generalmente di tumori a crescita lenta, poco invasivi e di piccole dimensioni. Nella maggioranza dei casi il tumore è localizzato a livello delle ossa lunghe o del massiccio facciale, ma non è rara l'evenienza in cui non sia possibile localizzare la neoplasia, nonostante un approfondito iter diagnostico. Nelle osservazioni riportate in letteratura, l'intervallo temporale tra l'esordio della sindrome e la scoperta della neoplasia causale è mediamente di 5-7 anni. La TIO è di difficile diagnosi ed è caratterizzata da specifiche alterazioni di laboratorio (ipofosforemia, iperfosfaturia e bassi livelli sierici di 1,25 diidrossivitamina D). A livello osseo si osserva una aumentata deposizione di matrice ossea non sufficientemente mineralizzata e una riduzione della resistenza al carico meccanico. La sintomatologia, frequentemente aspecifica e che può precedere anche di molti anni la diagnosi corretta, è caratterizzata da ipostenia, mialgia ingravescente e dolori ossei, soprattutto nelle aree sottoposte al carico, come cavigli...
