La hipertensión portal es un síndrome hemodinámico de la circulación venosa portal que condiciona múltiples complicaciones potencialmente mortales, debido a las alteraciones generadas en la circulación esplácnica y sistémica. En las últimas décadas se ha identificado la presencia de hipertensión portal en ausencia de cirrosis con un fenómeno de vasculopatía característico. Esta entidad puede clasificarse como prehepática, hepática y posthepática, según la localización del compromiso, identificado mediante estudios hemodinámicos. Se presentan cuatro casos de pacientes con hipertensión portal no cirrótica secundaria a trombosis portal. Adicionalmente, se realiza una revisión breve de la literatura, haciendo énfasis en las dos causas más frecuentes de hipertensión portal no cirrótica; la trombosis venosa portal crónica y la enfermedad vascular porto-sinusoidal.
Introducción: la osteoporosis es el trastorno endocrino óseo más prevalente en adultos mayores y genera a nivel mundial una gran morbimortalidad. Las causas secundarias en hombres representan entre el 50?% y el 80?% de los casos. En el abordaje inicial es importante el estudio del metabolismo del calcio. Objetivo: presentar el caso de un paciente con osteoporosis secundaria y normocalcemia asociada a hipercalciuria significativa. Presentación del caso: presentamos un paciente masculino de 55 años, con antecedente de nefrolitiasis que consulta por dolor lumbar y en quien se hallan fracturas vertebrales por fragilidad a nivel torácico y lumbar. Se realiza diagnóstico de osteoporosis severa y la densitometría ósea sugiere causa secundaria, confirmando al final la presencia de mieloma múltiple. Discusión y conclusión: las fracturas por fragilidad siempre obligan a la búsqueda de osteoporosis. Cuando se presenta en un paciente masculino debe recordarse que en el 80?% de los casos se presenta una causa secundaria que obliga a la búsqueda de la enfermedad subyacente. La hipercalciuria es un trastorno que siempre debe ser adecuadamente caracterizado y los valores elevados extremos de velocidad de eritrosedimentación (VSG) tienen un significado patológico, especialmente en relación con enfermedades que generan una gran carga de inflamación.
La pericarditis tuberculosa es una rara manifestación extrapulmonar que afecta a menos del 4% de los pacientes con tuberculosis (TB), con alta morbimortalidad ante diagnósticos tardíos, especialmente en aquellos pacientes inmunocomprometidos. Su presentación es inespecífica y la sintomatología deriva de la velocidad de instauración del derrame pericárdico, registrándose casos mortales debido a taponamiento cardiaco. La confirmación diagnóstica se obtiene con la identificación de la micobacteria en muestra de pericardio o líquido pericárdico, con una alta tasa de sospecha fundamentada en pruebas adicionales como la cuantificación del interferón gamma no estimulado, lisozima pericárdica y adenosina desaminasa (ADA) en liquido pericárdico. El tratamiento consiste en una combinación de terapia antituberculosa, drenaje pericárdico, y corticoesteroides como adyuvantes. Se presenta el caso de un varón de 64 años inmunocompetente, fumador frecuente en el pasado, con una alta sospecha diagnostica para TB, confirmada con resultado positivo para ADA (55,8 UI/L) en fluido pericárdico permitiendo iniciar un tratamiento adecuado con mejoría clínica posterior durante el seguimiento.
