Background: Estimation of usual dietary intake poses a challenge in epidemiological studies. We applied a blended approach that combines the strengths provided by repeated 24-h food lists (24HFLs) and a food frequency questionnaire (FFQ).Methods: At least two web-based 24HFLs and one FFQ were completed by 821 participants in the KORA FF4 study. Consumption probabilities were estimated using logistic mixed models, adjusting for covariates and the FFQ data on consumption frequency. Intake amount of a consumed food item was predicted for each participant based on the results of the second Bavarian Food Consumption Survey (BVS II). By combining consumption probability and estimated consumption amount, the usual food intake for each participant was estimated. These results were compared to results obtained without considering FFQ information for consumption probability estimation, as well as to conventional FFQ data.Results: The results of the blended approach for food group intake were often higher than the FFQ-based results. Intraclass correlation coefficients between both methods ranged between 0.21 and 0.86. Comparison of both methods resulted in weighted kappa values based on quintiles ranging from fair (0.34) to excellent agreement (0.84). Omission of FFQ information in the consumption probability models distinctly affected the results at the group level, though individual intake data were slightly affected, for the most part.Conclusions: Usual dietary intake data based on the blended approach differs from the FFQ-based results both in absolute terms and in classification according to quintiles. The application of the blended approach has been demonstrated as a possible tool in nutritional epidemiology, as a comparison with published studies showed that the blended approach yields reasonable estimates. The inclusion of the FFQ information is valuable especially with regard to irregularly consumed foods. A validation study including biomarkers of dietary intake is warranted.
Background. The rapid scale-up of antiretroviral therapy in resource-limited settings has greatly increased demand for antiretroviral medicines and raised the importance of good forward planning, especially in the context of the new 2010 WHO treatment guidelines. Methods. Forecasting of the number of people receiving antiretroviral therapy from 2010 to 2012 was produced using three approaches: linear projection, country-set targets, and a restricted scenario. Two additional scenarios were then used to project the demand for various antiretroviral medicines under a fast and slower phase-out of stavudine. Results. We projected that between 7.1 million and 8.4 million people would be receiving ART by the end of 2012. Of these, 6.6% will be on second-line therapy. High variation in forecast includes reductions in the demand for d4T and d4T increases in the demand for tenofovir, emtricitabine followed by efavirenz, ritonavir, zidovudine and lopinavir; lamivudine, atazanavir, and nevirapine. Conclusion. Despite the global economic crisis and in response to the revised treatment guidelines, our model forecasts an increasing and shifting demand for antiretrovirals in resource-limited settings not only to provide treatment to new patients, but also to those switching to less toxic regimens.
The German National Cohort (NAKO) is a multidisciplinary, population-based prospective cohort study that aims to investigate the causes of widespread diseases, identify risk factors and improve early detection and prevention of disease. Specifically, NAKO is designed to identify novel and better characterize established risk and protection factors for the development of cardiovascular diseases, cancer, diabetes, neurodegenerative and psychiatric diseases, musculoskeletal diseases, respiratory and infectious diseases in a random sample of the general population. Between 2014 and 2019, a total of 205,415 men and women aged 19–74 years were recruited and examined in 18 study centres in Germany. The baseline assessment included a face-to-face interview, self-administered questionnaires and a wide range of biomedical examinations. Biomaterials were collected from all participants including serum, EDTA plasma, buffy coats, RNA and erythrocytes, urine, saliva, nasal swabs and stool. In 56,971 participants, an intensified examination programme was implemented. Whole-body 3T magnetic resonance imaging was performed in 30,861 participants on dedicated scanners. NAKO collects follow-up information on incident diseases through a combination of active follow-up using self-report via written questionnaires at 2–3 year intervals and passive follow-up via record linkages. All study participants are invited for re-examinations at the study centres in 4–5 year intervals. Thereby, longitudinal information on changes in risk factor profiles and in vascular, cardiac, metabolic, neurocognitive, pulmonary and sensory function is collected. NAKO is a major resource for population-based epidemiology to identify new and tailored strategies for early detection, prediction, prevention and treatment of major diseases for the next 30 years.
Zusammenfassung Hintergrund Mit Beginn der SARS-CoV-2-Pandemie und der nachfolgenden Maßnahmen zu ihrer Eindämmung im Frühjahr 2020 ist rasch die Frage nach Auswirkungen der Beschränkung sozialer Kontakte auf die psychische Gesundheit der Bevölkerung aufgekommen. Einsamkeit beschreibt eine wahrgenommene Qualität der eigenen Kontakte und Beziehungen zu anderen Menschen. Zahlreiche Studien haben einen Zusammenhang von Einsamkeit mit somatischen und psychischen Erkrankungen aufgezeigt. Ziel Auswertung der Häufigkeit von Einsamkeit und ihrer Beziehung zu Angst- und Depressionssymptomen in der ersten Welle der Pandemie im Mai 2020. Methoden Zwischen 2014 und 2019 hat die NAKO-Gesundheitsstudie 205.000 Personen im Alter zwischen 20 und 69 Jahren in 18 Studienzentren in Deutschland rekrutiert und untersucht. Die nachfolgende Zweituntersuchung musste aufgrund der Pandemie im Frühjahr 2020 unterbrochen werden. In dieser Zeit wurde ein COVID-19-bezogener Fragebogen entwickelt und an alle Teilnehmenden verschickt. Ausgewertet wurden die 113.928 Fragebögen, die innerhalb der ersten 30 Tage zurückgeschickt wurden. Einsamkeit wurde mit der 3‑Item UCLA Loneliness Scale, Angst und Depression mit den PHQ-9- und GAD-7-Skalen des Patient Health Questionnaire erhoben. Ergebnisse Im Mai 2020 nahmen sich 31,7 % der NAKO-Teilnehmenden als einsam wahr. Frauen und junge Menschen waren häufiger als Männer und ältere Personen betroffen. Mit steigender Wahrnehmung von Einsamkeit nahm der Schweregrad von Depressions- und Angstsymptomen stetig zu. Einsame Personen während der Pandemie hatten bereits zur NAKO-Basisuntersuchung mehr depressive und Angstsymptome angegeben als NAKO-Teilnehmende, die sich in der Pandemie nicht einsam fühlten. Schlussfolgerung In der NAKO-Gesundheitsstudie zeigte sich während der ersten Phase der Pandemie eine Zunahme von Einsamkeit und ihr deutlicher Zusammenhang mit schlechterer, psychischer Gesundheit.
The developed screening method fulfilled the specifications of the National Cohort. The used pinhole visual acuity examination was fast and cheap. Image quality and usability of the retina photographies could be improved with prolonged pupil recovery times. The quality of the anterior segment images could not fulfill the expectations and were taken out of the further examinations of the ophthalmological screening. The written SOP showed good acceptance within the investigators' daily routine. The ophthalmological screening within the National Cohort generates information (e. g., pathologies of the vessels or of the retina) which are useful not only from an ophthalmological point of view.
Messung der körperlichen Fitness in der NAKO Gesundheitsstudie-Methoden, Qualitätssicherung und erste deskriptive Ergebnisse Einleitung Der körperlichen Fitness wird eine besondere Rolle für die Gesundheit des Menschen zugesprochen. Dabei umfasst die gesundheitsbezogene körperliche Fitness im Allgemeinen die Leistungsfähigkeit des Herz-Kreislauf-Systems (Cardiorespiratory Fitness, CRF), die Muskelausdauer, die Muskelkraft, die Körperzusammensetzung sowie die Beweglichkeit [1]. Von diesen Komponenten der körperlichen Fitness haben sich insbesondere die Muskelkraft sowie die CRF als bedeutungsvolle prognostische Indikatoren für die Gesundheit des Menschen erwiesen. Bezogen auf die Muskelkraft konnte gezeigt werden, dass die Handgreifkraft (Grip Strength, GS) ein sehr guter Parameter für die Vorhersage von Gesundheitsrisiken ist [2-4]. So konnte z. B. in der PURE-Studie ein inverser Zusammenhang zwischen der GS und der allgemeinen und kardiovaskulären Mortalität gezeigt werden [2].
