Chronic increase in the intravesical pressure secondary to bladder outlet obstruction can lead to the formation of bladder diverticulum. Bladder diverticulum may get pulled into the hernial sac and may become a component of the hernia. Here, we report the case of an elderly male who had an unusual presentation of urinary bladder diverticulum as the content in an obstructed inguinal hernia. Upon exploration, the bladder diverticulum was released from the inguinal canal and returned to the peritoneal cavity, following which conventional hernioplasty was done. Inguinal herniation of bladder diverticulum is an uncommon condition and can be perilous during surgery if not diagnosed preoperatively.
Endometriosis is presence of functioning endometrial tissue outside the uterine cavity. Endometriosis can sometimes occur in a previous surgical scar. Scar endometriosis is rare and difficult to diagnose. It mostly follows obstetrical and gynecological surgeries. This condition is often confused with other surgical conditions. We are reporting 5 cases of scar endometriosis following cesarean section, some of which were misdiagnosed as stitch granuloma initially. Medical treatment was not helpful. All 5 patients required wide surgical excision of the lesion. The pathogenesis, diagnosis and treatment of this condition are being discussed.
Subdiaphragmatic abscess is rather common itself but a tubercular subdiaphragmatic abscess in the absence of coexisting tuberculous abdomen presenting as thoracic complication is extremely rare. A cold abscess is an abscess that commonly accompanies tuberculosis. It develops so slowly that there is little inflammation and it becomes painful only when there is pressure on the surrounding area. Here we report a case of isolated subdiagphragmatic tuberculous cold abscess in a 38-year-old female whose imaging of the abdomen revealed a hyperechogenic area involving segment 7 of liver adherent to rib cage and diaphragm, S/O ruptured liver abscess, subdiagphragmatic abscess. The pus drained from the abscess cavity at diagnostic laparoscopy proved Multidrug Resistant Tuberculosis. This case has been reported for the rarity of isolated subdiaphragmatic tuberculous abscess and the importance of microscopic or culture diagnosis in a suspected case of tuberculous abscess.
A lump in the neck is a common problem presenting to clinicians. In most individuals, the new lump in the neck is noticed casually by the patient himself or by others. This lump without any associated symptoms may be the only indicator of underlying malignancy in the adult population.Here, we report one such case of a vague neck lump turning out to be lymphocyte-rich classical Hodgkin's lymphoma (HL). Lymphocyte-rich classical Hodgkin's lymphoma (LRCHL) is considered a rare subtype of HL having a relatively good prognosis.
