Introdução: Baseado em estudos de perfil de expressão gênica, o câncer gástrico (CG) foi categorizado recentemente em subtipos moleculares, os quais incluem tumores Epstein-Barr (EBV)-positivo e microssatélite instável (MSI). A definição destes, além de fornecer maiores informações prognósticas, permite a identificação de vias moleculares envolvidas na progressão tumoral e potenciais alvos terapêuticos. Nesse contexto, a identificação de biomarcadores como o PD-L1 pode auxiliar na estratificação dos pacientes e adoção de terapia personalizada. Objetivo: Avaliar a presença do EBV, MSI e expressão de PD-L1 no CG e suas relações com características clínicopatológicas e prognósticas. Método: Foram avaliados retrospectivamente 287 pacientes com CG submetidos à gastrectomia com linfadenectomia através da construção de tissue microarray. A expressão das proteínas de reparo do DNA (MLH1, MSH2, MSH6, PMS2) e de PD-L1 foram avaliadas por imuno-histoquímica. O EBV foi detectado por hibridização in situ. Resultados: A presença de infecção por EBV e MSI foi identificada em 10,5% e 27% dos pacientes, respectivamente. A maioria dos CG com MSI apresentaram perda simultânea da expressão de MLH1 e PMS-2 (60%). A positividade para EBV foi mais frequente em homens (p=0,032), tumores proximais (<0,001), Lauren indeterminado/intestinal (p<0,001), pouco diferenciado (p=0,043) e com acentuado infiltrado inflamatório (p<0,001). Os CG MSI foram associados à idade mais avançada (p=0,002), localização distal (p=0,004), baixa incidência de metástase linfonodal (LNM) (p=0,024), infiltração linfática (p=0,039) e estádio menos avançado (p=0,020). A expressão do PD-L1 foi vista em 8,8% dos casos, com predomínio nas células tumorais (PD-L1TU) (6,3%). CG PD-L1TU(+) foram associados ao tipo indeterminado de Lauren (p<0,001), pouco diferenciados (p=0,015) e a presença de acentuado infiltrado inflamatório (p=0,010). A expressão de PD-L1 foi associada à infeção por EBV (p<0,001). Entre os CG PD-L1(+), 50% eram EBV(+) e 27% MSI. Na análise de sobrevida, CG com MSI foram associados a melhor sobrevida livre de doença (SLD) (p=0,011). Enquanto pior SLD foi observada nos casos com LNM PD-L1TU(+) comparado aos PD-L1TU(-) (p=0,049). Conclusão: A MSI foi significantemente associada a melhor SLD, confirmando sua utilização como marcador prognóstico no CG. Tumores EBV(+) foram associados à expressão de PD-L1, sugerindo que a identificação de subgrupos pode auxiliar na indicação à terapia direcionada.
