Dans ce texte, l’auteure réactualise son expérience de chercheuse, analyste de groupe et des institutions, au sein de lieux « cachés » et exceptionnels. Les salles de garde des bâtiments de l’internat en médecine des centres hospitaliers universitaires conservent en leur sein l’apanage d’un monde traditionnel appartenant à l’apprentissage de la médecine et soumis aux évolutions de la contemporanéité hypermoderne. Les butées de la transmission intergénérationnelle et le dévoilement sur la scène publique des pratiques « carabines » en salle de garde participent de leur effraction et font bouger la tradition. La finitude de cette culture traditionnelle dans ces espaces groupaux questionne et s’éprouve. Cet article tente, entre autres, d’apporter des éléments de réponse quant aux difficultés et limites rencontrées pour penser le déclin de la tradition carabine.
En s’étayant sur les concepts de la psychanalyse des groupes et des institutions, cet article s’attache à comprendre les processus psychiques inconscients au sein d’espaces groupaux traditionnels peu communs. La problématique s’intéresse aux fonctions qui soutiennent le travail de culture de la tradition carabine pour ces sujets apprentis médecins. L’auteure propose une lecture du malêtre contemporain de René Kaës (2012) à travers l’ethnographie du dispositif traditionnel appartenant aux origines de l’internat de médecine. L’internat est la période par laquelle les internes en médecine deviennent des médecins. La liminarité constitutive de ce moment de passage s’accompagne et s’encadre en salle de garde grâce à la perpétuation des rituels de sa tradition originelle. Ce dispositif traditionnel n’échappe pas aux bouleversements hypermodernes. Néanmoins, l’analyse du déclin de la tradition carabine questionne autant qu’elle éclaire la complexité des besoins des nouvelles générations d’internes en médecine.
Les actualisations du code de déontologie réduisent l’asymétrie entre le médecin et le malade et régissent les devoirs des médecins envers les patients, dont celui de l’exercice du consentement libre et éclairé. En neurologie, une jeune patiente est dans l’attente d’un diagnostic et de réponses vis-à-vis de ses souffrances physiques. Des tensions surgissent entre elle et l’équipe médicale, elle réclame un changement d’hôpital ainsi que son dossier médical. Une seconde observation expose les discussions des médecins en réanimation concernant l’arrêt thérapeutique d’une patiente et les informations qui ne seront pas communiquées à ses proches. Dans ces deux cas, face au réel du travail, les médecins laissent les ayants droit dans l’ignorance d’éléments. Par affiliation à l’approche psychodynamique du travail, l’auteure montre notamment que la phronèsis médicale ou sagesse pratique peut être étroitement contrôlée par les stratégies collectives de défense des médecins.
