C eliac disease (CeD) is an autoimmune disorder that occurs in genetically predisposed individuals as a result of an immune response to gluten. It is present in approximately 1% of the population. Diarrhea has become a less common mode of presentation (<50% of cases) than it once was. We reported a female patient with previously unrecognized celiac disease and multiple extra intestinal manifestations, presenting with gastrointestinal bleeding and severe anemia, and misdiagnosed as Behcet's disease because of her oral aphthous ulcers. The patient who admitted to hospital with dizziness and fatigue had severe anemia. Laboratory findings revealed hypocalcemia, hypokalemia, hypomagnesemia. Vitamin A, vitamin B12, folic acid and vitamin D levels were low. In the light of this malabsorptive state anti tissue transglutaminase and anti endomysial antibodies were positive. Esophagogastroduodenoscopic findings were compatible with CeD. When we evaluated her for the accompanying manifestations we found osteoporosis, epilepsy, hypothyroidism, partial empty sella syndrome, oral aphthous ulcers, short stature, hematological abnormalities such as coagulation disorders and anemia. Baş dönmesi ve halsizlik yakınmaları ile hastaneye başvuran hastada derin anemi mevcuttu. Hipokalemi, hipokalsemi, hipomagnezemi bulgularının yanı sıra vitamin A, vitamin B12, folik asit ve vitamin D seviyeleri düşüktü. Bu malabsorptif zeminde anti doku transglutaminaz ve endomisyal antikorları pozitifti. Özofagogastroduodenoskopik görünümü ÇH için tipikti. Eşlik eden durumları araştırdığımızda osteoporoz, epilepsi, hipotiroidizm, parsiyel boş sella, oral aftlar, boy kısalığı ve koagülasyon bozuklukları ve anemi gibi hematolojik anormallikler saptadık. SUMMARY INTRODUCTION Celiac disease (CeD) is an autoimmune disease that can be delayed in diagnosis due to the presence of atypical and asymptomatic cases in adulthood. Herein we aimed to study the frequency of CeD and evaluate whether magnified endoscopy and magnified/FICE (flexible spectral imaging color enhancement) techniques contribute to the diagnosis in patients with serum iron and vitamin B12 deficiency. METHODS We evaluated 50 adult patients (10 males and 40 females) who had serum iron and vitamin B12 deficiency, prospectively. All the patients had undergone upper gastrointestinal system endoscopy by the same endoscopist. The second part of the duodenum was evaluated with white light, magnified, and magnified/FICE endoscopy. Biopsy specimens were evaluated by the same pathologist. The specimens diagnosed as CeD were classified according to the Modified Marsh-Oberhuber criteria. RESULTS 10 of 50 patients (20%) were diagnosed as CeD. The average age was 41±11 years (20-67 years). Thirty percent of CeD diagnosed patients had typical CeD symptoms. Six of 10 patients (60%) who were diagnosed as CeD had typical endoscopic images under white lighted endoscopy. All of these 10 patients (100%) showed villous irregularity, partial villous atrophy, or total villous atrophy consistent with CeD with magnified and magnified/FICE endoscopy. CONCLUSION The practical use of magnified/FICE endoscopy allows us to differentiate mucosal abnormalities of the duodenum and minimize false-negative results that indicate normal mucosal findings with conventional endoscopy.

Retrospektif olarak kliniğimizde takipli HCC'li hastalarını karakterize etmeyi, sağkalımı araştırmayı ve sağkalım için öngörücü faktörleri belirlemeyi amaçladık. Gereç ve Yöntemler: Manisa Celal Bayar Üniversitesi Tıbbi Onkoloji Polikliniğinde 2014-2019 yılları arasında HCC teşhisi konan tüm hastalar geriye dönük olarak çalışmaya kaydedildi. Genel sağkalım Kaplan -Meier yöntemi kullanılarak hesaplandı. Sağkalım için öngördürücü faktörleri belirlemek için çok değişkenli Cox regresyon analizi yapıldı. Bulgular: Kliniğimizde takipli 40 HCC hastasının medyan tanı yaşı 66 olup %90'ı erkektir. Hepatit B virüs (HBV) enfeksiyonu en sık etyolojik sebeptir. C-Reaktif Protein (CRP) seviyesi daha yüksek olan hastaların sağkalım sürelerinin daha kısa olduğu görülmüştür. AFP düzeyi ile sağkalım arasında anlamlı korelasyon izlenmemiştir. Sonuç: Bölgemizde HBV'ye bağlı HCC'nin daha sık görüldüğü saptanmıştır. Hedefe yönelik tedavilerin umut vadettiği ve bu yönde prediktif belirteçlere ihtiyaç duyulduğu düşünülmektedir.
