Background: Nosocomial infections are a global threat to human health worldwide.Aim: This study aimed to investigate the change of nosocomial infection factors in equivalent historical periods in pediatric patients without COVID-19 before and during the pandemic in the pediatric intensive care unit.Method: The study was planned retrospectively. Data on hospital infection rates, incidence densities, invasive device-associated infections, infectious agents, comorbid diseases, and invasive procedures in non-COVID-19 pediatric patients were obtained from the medical records for the periods of April-September 2019 and April-September 2020 in the pediatric intensive care unit. Hand hygiene compliance rates of healthcare workers were evaluated.Results: Prior to the pandemic, the number of patients was 332, comprising 2,377 patient days with a nosocomial infection rate of 5.12, and an incidence density of 7.15. During the pandemic, the number of patients was 221, comprising 2,260 patient days with a nosocomial infection rate of 4.52, and incidence density of 4.43. Prior to the pandemic, there were 28.80% cases of Klebsiella pneumoniae, 23.81% of Pseudomonas aeruginosa, 9.52% of Enterococcus faecium, and 4.76% of Enterococcus faecalis. During the pandemic, there were decreased 14.29% cases of Klebsiella pneumoniae while Pseudomonas aeruginosa, Enterococcus faecium, and Enterococcus faecalis was not seen. Prior to the pandemic, the hand hygiene compliance rate was 94.83%, and during the pandemic, it was found to be 99.44%. Conclusion:This study showed that the spread of bacteria such as Klebsiella pneumoniae, Pseudomonas aeruginosa, vancomycin-resistant enterococci, and Stenotrophomonas maltophilia, which are a major public health threat, can be decreased by applying simple standard methods.
Hastane enfeksiyonları hastalar için risk olduğu kadar sağlık çalışanları içinde büyük önem taşımaktadır. Bu çalışma hastane enfeksiyonlarının yayılmasında önemli yeri olan hemşirelerin görevleri gereği enfeksiyonların gerek kendilerine bulaşmasını, gerekse hastane içerisinde yayılmalarını engelleyici enfeksiyon kontrolü ve koruyucu önlemler hakkında bilgileri ve klinik içi uygulamalarını belirlemek amacıyla gerçekleştirilmiştir. Yöntem: Araştırma kesitsel ve tanımlayıcı özelliktedir. Araştırmanın evrenini üniversite hastanesinde kliniklerde çalışan 330 hemşire oluşturmaktadır. Araştırma minumum örneklem büyüklüğü 178 olarak belirlenmiştir. Araştırma verileri araştırmacılar tarafından hazırlanan anket aracılığı ile yüz yüze uygulanarak toplanmıştır. Araştırmaya hastanede bulunan kliniklerden 10 hemşire rastgele seçilmiş, hemşirelere gündüz ve gece mesainde ulaşılmıştır. Araştırmaya katılmayı kabul eden 130 hemşire araştırmaya dahil edilmiştir. Veriler anket yöntemi ile toplanmıştır ve SPSS 22.0 programında analiz edilmiştir. Veriler %95 güven aralığında, p<0.05 olarak anlamlı kabul edilmiştir.Bulgular: Hemşirelerin yaş ortalamaları 26.0±0.5 olup, %84.6'sı kadındır ve %55'i lisans mezunudur. Meslekte çalışma yılı ortalaması 5.0±0.3'tür. Hemşirelerin kişisel koruyucu önlemler ve el hijyenine yönelik bilgi düzeylerinin standart önlemler çerçevesinde yüksek olduğu belirlenmiştir. Hemşirelerin hastane enfeksiyonları için sıklıkla aldığı önlem eldivendir (%84.2). Hemşirelerin %95.4'ü tarafından maske, bone ve gözlük gibi diğer araçların kullanılması enfeksiyona karşı koruyucu bariyer olarak bildirilmiştir. Hemşirelerin %42.6'sı okul müfredatından, %39.5'i hastane hizmet içi eğitimden hastane enfeksiyonları ile ilgili bilgi edindiğini bildirmiştir. Hastane enfeksiyonlarını önleme çalışmalarının yeterli olmadığını düşünenler %67, kendisinin yeterli bulmayanlar ise %48'dir. Hastanelerde enfeksiyon kontrolü için yetersizliklerin en önemli nedeni sorulduğunda birinci sırada malzeme eksikliği (%68.7) gelmektedir. Sonuç: Hemşirelerin hastane enfeksiyonları korunma ve önlenmesinde önemli rol oynadıkları bilinmektedir. Hemşirelerin bilgi düzeylerini arttırmak önemlidir ancak tek başına yeterli değildir.Hastane enfeksiyonları için koruyucu önlemlerin alınabilmesi için gerekli malzemelerin sağlanması ve hastane enfeksiyon önleme kurallarına uyulması gerekmektedir.
Introduction: Vancomycin-resistance in enterococci (VRE) poses a major threat in hospitals. It is well known that patients in the neonatal intensive care unit (NICU) is especially at risk of nosocomial VRE acquisition. In this study, it was aimed to present the VRE outbreak analysis identified in NICU. Materials and Methods: Patients with VRE colonization and infection were included in the study between 17.10.2016-30.03.2017 in NICU. Results: There are a total of 30 beds in the NICU, including five beds in the level 1 ICU, eight beds in the level 2 ICU and 17 beds in the level 3 ICU. Newborns in the NICU are hospitalised in the same ICU (room) in a mixed form. Enterococci were detected in blood culture samples of four patients in the NICU. Three of these cases were VRE and the other was VSE (vancomycin-sensitive enterococci). The most important problem identified in the NICU was the inability to use the interior architecture effectively. In addition, inadequate nurse both in number and level of education and lack of night observations were identified as other problems. Conclusion: As a result, the outbreak in NICU was controlled thanks to educated nurse support, frequent visits and change in interior architecture.
