İntihar girişimi sonucu yoğun bakıma yatırılarak takip ve tedavi edilen çocuk ve ergenlerde intihar girişimlerinin özelliklerinin değerlendirilmesi ve konuya dikkat çekilmesi amaçlanmıştır. Yöntemler: Erciyes Üniversitesi Tıp Fakültesi Çocuk Yoğun Bakım Ünitesi'ne 2010-2015 yılları arasında intihar girişimi nedeniyle yatırılan çocuk ve ergenlerin sosyo-demografik ve klinik özellikleri, tetikleyici faktörler, intihar girişiminin niteliği, ilaç alımı ile olan intiharlarda kullanılan ilaçlar, olguların çocuk ve ergen ruh sağlığı ve hastalıkları tarafından yapılan takiplerinin değerlendirilmesi amaçlanmıştır. Bulgular: Çalışmaya yaş ortalaması 14,5±1,2 yıl (min:10,5-maks: 17) olan, 46'sı kız (%88,5) toplam 52 olgu dahil edildi. Hastaların 24'ü (%46,1) tek ilaç alarak intihar girişiminde bulunurken, 28'i (%53,9) çoklu ilaç alarak intihar girişiminde bulunmuştur. Analjezikler (%42,3) en sık alınan ilaç grubu olup, bunu sırasıyla anti-depresanlar (%28,8), ve antihipertansifler (%15,3) izlemektedir. Hastanede yattıkları dönemde psikiyatrik değerlendirmesi yapılabilen 52 olgudan 24'ünde (%46,1) herhangi bir ruhsal problem saptanmamış olup, impulsif intihar olarak değerlendirilmiştir. Olgularda majör depresyon (%28,8) saptanan en sık ruhsal bozukluk olup bunu sırasıyla davranım bozukluğu (%11,5), uyum bozukluğu (%7,6), dürtü-kontrol bozukluğu (%3,8) izlemiştir. En sık karşılaşılan tetikleyici faktörün aile çatışması (%53,8) olduğu belirlenmiştir. Sonuç: İntihar girişimlerinin özelliklerini belirlemek, risk faktörlerini saptamak olguların izlemi ve koruyucu önlemlerin alınması açısından yol gösterici olacaktır. Özellikle depresyonu olan ergenlerin yakından izlemi ve tedavisi önemlidir. Ayrıca impulsif intihar girişimlerinin önlenmesi açısından ergenlerin ve ailelerin desteklenmesi gerekmektedir. Ergenlerin ev içinde ve ev dışında ilaçlara ulaşımının engellenmesi için gerekli önlemler alınmalıdır.
1): Humoral and cellular immunological functions were evaluated in 4 children with CVI. All the patients showed low levels of Ig, except one with normal IgM level, at the time of diagnosis. Secretory IgA were present and the basal production of Ig in vitro were lower than the control in all patients. Ab production did not improve by the modulation of the autnlogus lymphocytes. Delayed hypersensibility test was negative in 2 and lymphocyte proliferative response to mitogens was absent in 3 patients. Immunological phenotype demonstrated a deficit of CD4+ lymphocytes in 3 patients, with an inversion of CD4+/CDS+ ratio in two of them. Absolute, CD4+ CD29+ lymphocytes deficit was detected in all patients. These cases belong to a variant of CVI characterized by a deficit of CIM+ CD29+ lymphocytes that could have an important role in differentation processes of B cells and Ab production. ABSTRACT (2):A boy, actually 10 years old, with X-linked Chronic Granulomatous Disease CGD and present Cytochrome b has been studied, History of CGD began at two months of age with recurrent pyogenic infections due to Staphylococcus aureus and Aspergilhis fumigatas (recurrent suppurative cervical lymphadenitis at 2 and 6 months of age, a perianal abscess at 2 years of age and a fistula which remained open for about 4 years, broncopnenmonia at 4 years and 6 years of age and Aspergillus fumigatus pneumonia invading the thoracic wall at 7 years of age).His polymorphonaclear cells were defective in tests of Nitro Blue Tetrazolium (NBT) reduction and O 2-generation with a normal Cytochrome b level as judged by difference spectroscopy.The maternal granulocyte function tests showed a mosaicism in the NBT-test of NBT positive and negative cells indicating X-linked trasmission, but all her neutrophils stained for cytochrome b on the cell surface by using moneclonal antibody 7D5.This discrepant results is highly suggestive for a dysfunctional Cytochrome b in this family. The mutated X-linked gene coding for the heavy chain of Cytochrome b is currently being analyzed in detail.ABSTRACT (5): The X-tinked bleeding disorder hemophilia A results from mutation in the factor VIII gene. We used the denaturant gradient gel alectrophoresis (DGGE) to find mutations in amplified DNA from 80 Italian hemophilic patients. To date we screened exons 4, 8, 14. The mutations detected were sequenced on single strand DNA. We found, in exon 4, a microdelection at codon 162 (GAG->-G) causing a severe disease; in exon 14 a C->T change at codon 1689 in a patient with mild disease. In exon 8 two mutations at the same codon 373 were observed: one is a C->A change producing serine to stop codon transition, the second is a C->T transition producing a missense mutation seriue-> Ieucine. The patients show severe and mild disease, respectively. The patients with severe diease showed a cross reacting material (CRM) negative hemophilia A.
