Cutaneous tuberculosis: two case reports and reviewCutaneous tuberculosis is a chronic infectious disease caused by Mycobacterium tuberculosis. It is uncommon (1% of all cases of tuberculosis), but has increased due to the human immunodefi ciency virus epidemic and to pharmacologic immunosuppression. It presents a wide variety of clinical forms depending on how bacteria reach the skin and on the immune status of the patient. We present two cases of cutaneous tuberculosis diagnosed in the Hospital Clínico de la Pontifi cia Universidad Católica de Chile. We emphasize the diffi culty in diagnosis and classifi cation of this disease and briefl y discuss on the different diagnostic and therapeutic approaches.Key words: Cutaneous tuberculosis, extrapulmonary tuberculosis. Caso 1P aciente de 56 años, sexo femenino, con antecedente de dermatomiositis diagnosticada un año previo a la consulta, en tratamiento con hidroxicloroquina (Plaquinol ® ) 200 mg, metotrexato 10 mg y prednisona 30 mg diarios. Fue hospitalizada por cuadro de 10 días de evolución, con fi ebre, compromiso del estado general y pérdida de peso, encontrándose crepitaciones bilaterales al examen pulmonar. La TAC de tórax mostró un infi ltrado nodular bilateral, extenso, con cavitaciones biapicales. La baciloscopia obtenida por lavado bronquio-alveolar (LBA) resultó positiva y la reacción de polimerasa en cadena (RPC) positiva para Mycobacterium tuberculosis. El resto del estudio de LBA fue negativo (cultivo corriente, hongos, Nocardia spp, inmunofl uorescencia, RPC para Pneumocystis jiroveci, panel viral respiratorio, cultivo de citomegalovirus, galactomanano). El Departamento de Dermatología fue consultado por úlceras en los pulgares aparecidas cuatro semanas previas al ingreso. Al examen físico se observó una úlcera de bordes bien delimitados con fondo sucio y bordes necróticos, no dolorosa, ubicada en el pulpejo del dedo pulgar izquierdo ( Figuras 1A y B). Se le realizó biopsia que mostró una necrosis reciente extensa, sin evidencia de vasculitis ni elementos histopatoló-gicos sugerentes de TBC, con inmunofl uorescencia directa para C3, IgA, IgG, IgM y fi brina negativos. En el cultivo corriente crecieron S. aureus y Candida parapsilosis. La tinción de Ziehl Neelsen en tejido fue positiva. Se inició tratamiento con isoniacida, (HIN) rifampicina (RFP), pirazinamida (PZ) y etambutol, además de ceftriaxona y fl uconazol. El cultivo acelerado de Mycobacterium spp de la úlcera del pulgar fue negativo a los 60 días; sin embargo, el cultivo obtenido en LBA resultó positivo a los 30 días. La paciente fue trasladada al Hospital del Tórax para continuar su tratamiento.Dado que se trataba de una paciente inmunosuprimida que cursaba con un cuadro de tuberculosis (TBC) pulmonar, la úlcera del pulgar pudo corresponder a un absceso frío. The free-living amebas Naegleria and Acanthamoeba are recognized as causal agents of central nervous system infections. Recently a third free-living ameba, Balamuthia mandrillaris, was identified as the causal agent of granulomatous encephalitis in humans. We report a case of Balamuthia encephalitis in an immunocompetent school-age girl who presented cutaneous lesions that compromised the central portion of the face. The skin biopsy revealed granulomatous lesion with positive PCR for non-tuberculous Mycobacterium. We started treatment for atypical extrapulmonary mycobacteriosis. Nevertheless, the child was readmitted six months later, with progressive neurological involvement, dying about one year after the onset of cutaneous symptoms. The brain necropsy showed the presence of B. mandrillaris cysts and trophozoites. Balamuthia mandrillaris infection should be considered in the differential diagnosis of a patient with chronic granulomatous disease with neurologic symptoms.
Skin necrosis must be considered as a syndrome, because it is a clinical manifestation of different diseases. El diagnóstico precoz y la pronta identificación de la etiología son claves para un tratamiento oportuno y mejorar la sobrevida del paciente.Se recolectaron los casos más demostrativos de necrosis cutánea en pacientes hospitalizados. Se registró la historia clínica, se tomaron fotografías y en algunos casos se realizaron biopsias de las lesiones. Casos clínicosEn la Tabla 1 se presentan los casos clínicos más demostrativos de necrosis cutánea en pacientes hospitalizados en nuestra institución. Se detalla sexo del paciente, edad, antecedentes mórbidos, fármacos utilizados, clínica con la que se presentaron al momento de la consulta y el estudio de anatomía patológica (AP). Finalmente, se describe el diagnóstico y se menciona el tratamiento que se realizó. Caso 1: Necrosis por cumarínicosSe ha descrito necrosis cutánea asociada al uso de warfarina, reportándose una prevalencia entre 0,01 y 0,1% de los pacientes tratados. Aunque el mecanismo no está aclarado, se describe un efecto procoagulante transitorio por una rápida caída de los niveles de proteína C 1,2 . El cuadro clínico se manifiesta como edema y eritema de inicio súbito, asociado a dolor o parestesias. En 90% de los casos, la aparición de necrosis cutánea se produce de 3 a 6 días de iniciado el tratamiento, aunque se han descrito casos que se han desarrollado semanas o meses después 3 . El estudio histopatológico revela trombosis y depósitos de fibrina en las vénulas postcapilares sin inflamación 4,5 . El tratamiento es la suspensión del fármaco, transfusión de plasma fresco congelado y aporte de vitamina K endovenosa.
