Esophageal perforation is a condition associated with high morbidity and mortality rates; it requires early diagnosis and treatment. The most common complication of esophageal rupture is mediastinitis. There are several case reports in the literature of mediastinitis secondary to esophageal perforation and development of aortic pseudoaneurysm as a complication. We report the case of a patient with an 8-day history of esophageal perforation due to foreign body (fishbone) with mediastinitis and aortic pseudoaneurysm. The diagnosis was made using Computed Tomography (CT) with intravenous and oral water-soluble contrast material. An esophagogastroduodenoscopy did not detect the perforation.
La vía visual comprende estructuras de alta complejidad anatómica y funcional, que pueden ser agrupadas por localización en tres grupos: prequiasmáticas, quiasmáticas y retroquiasmáticas. Las lesiones prequiasmáticas más frecuentes son el retinoblastoma y el melanoma uveal al interior del globo ocular, y el glioma del nervio óptico, el meningioma de la vaina del nervio óptico y la neuritis óptica que comprometen el nervio óptico. Las lesiones que más frecuentemente comprometen el quiasma son los gliomas, meningiomas, craneofaringiomas y adenomas hipofisarios. Finalmente, en las lesiones retroquiasmáticas el PRES (síndrome de encefalopatía posterior reversible, por sus siglas en inglés) es una de las más representativas. El objetivo de este artículo es presentar una revisión de la anatomía radiológica y de los principales hallazgos anormales en neuroimagen.
Introducción: El sistema de reporte de imágenes e información radiológica del hígado (LI-RADS) desarrollado por el Colegio Americano de Radiología, hasta la actualización 2018 presenta limitaciones para realizar seguimiento por imágenes a los pacientes que reciben tratamiento sistémico para hepatocarcinoma. Métodos: Se presentan cinco casos de pacientes con observaciones LI-RADS iniciales compatibles con hepatocarcinoma y la respectiva evolución radiológica de las mismas después de la administración del inhibidor de tirosin-cinasa, Sorafenib. Resultados: A partir los casos presentados se realizó una revisión de los diferentes sistemas de evaluación para el seguimiento: los criterios de la Organización Mundial de la Salud, RECIST versión 1.1, EASL, mRECIST, RECICL, Choi, PERCIST y EORTC. Conclusiones: Los criterios de evaluación propuestos permiten un seguimiento radiológico más preciso para determinar la respuesta al tratamiento con terapia sistémica, en especial los criterios mRECIST, permitiéndole al clínico tratante realizar los ajustes terapéuticos correspondientes y al equipo médico obtener mejores resultados en estos pacientes.
