Pulp infections leading to total pulp necrosis occur as a result of caries, dental procedures and trauma. The tooth may remain asymptomatic until inflammation spreads to the periradicular tissues; spontaneous pain and percussion can be seen when spread. Radiographic changes may occur, ranging from a thickening of the periodontal ligament space to the appearance of a periapical radiolucent lesion. Treatment options for large periapical lesions range from nonsurgical root canal treatment and apical surgery to tooth extraction. This case report describes a nonsurgical management of a large mandibular cyst-like periapical lesion involving mandibular incisors. The patient returned for clinical and radiographic controls every 3 months during the first year, radiographic evidence of lesion healing was observed at the 12months follow-up. Tomography showed that the lesion was filled with bone like tissue. With appropriate treatment planning under ideal conditions, no matter how large the lesion is, improvement can be achieved without the need of surgical intervention.
scite is a Brooklyn-based organization that helps researchers better discover and understand research articles through Smart Citations–citations that display the context of the citation and describe whether the article provides supporting or contrasting evidence. scite is used by students and researchers from around the world and is funded in part by the National Science Foundation and the National Institute on Drug Abuse of the National Institutes of Health.
