Outpatient excision under local anesthesia of a thrombosed external hemorrhoid can be safely performed with a low recurrence and complication rate while offering a high level of patient of acceptance and satisfaction.
Zusammenfassung Ziel: Eine retrospektive Studie wurde durchgeführt um die Rezidivrate, Komplikationsrate, Langzeitergebnisse und Patientenzufriedenheit nach ambulanter Operation (in Lokalanästhesie) einer Perianalthrombose bzw. segmentären thrombosierten Anal-/Hämorrhoidalprolapses festzustellen. Patienten und Methode: 1995 bis 1999 wurden 340 Patienten operiert. Mindestens 9 Monate nach der Operation erhielt jeder Patient einen Fragebogen, in dem nach Rezidivoperationen, Restbeschwerden, Zufriedenheit mit der Lokalanästhesie/Wundbehandlung gefragt wurde.Ergebnisse: 20 Patienten entwickelten ein Rezidiv (5,9%), drei eine postoperative Blutung (0,9%), sieben einen Abszess oder Fistel (2,1%). Die Rücklaufquote der Fragebögen betrug 86%, die durchschnittliche Nachbeobachtungszeit 16,3 Monate. 67% der Patienten gaben Beschwerdefreiheit an. 81% würden sich wieder ambulant in Lokalanästhesie operieren lassen, 5% nicht, 13% waren sich unsicher.Schlussfolgerung: Die Ergebnisse zeigen, dass sich die ambulante Exzision in Lokalanästhesie gut durchführen lässt mit akzeptablen Rezidiv-und Komplikationsraten. Die Langzeitergebnisse bestätigen, dass diese Operation auch von den Patienten gut toleriert wird.
