This research examined the relationship between nursing managers' power and nursing group power in public health service organizations. Approach/Methods: A cross-sectional survey of first-line nurse managers (n ¼ 264) from 11 Finnish hospitals was conducted. The power of nurse managers and nursing groups was examined through two reliable instruments: the Nursing Managers' Power instrument and the Sieloff-King Assessment of Group Power within Organizations instrumentß. Findings: The power of nurse managers was perceived to be moderate on a 5-point Likert scale. Managers reported that they gained the most power from their role and position as a nursing manager, and lastly, from their resources. The power of nurse managers correlated no more than moderately with nursing group power. Implications for Nursing Management: Organizations need to ensure that nursing managers have the power and opportunity to influence nursing goals and outcomes.
The results of the present study indicated that the nurses perceived the lowest levels of group power in relation to the subscales of controlling the effects of environmental forces, resources, communication competency and group supervisor's outcome attainment competency.
Different organizational climates and cultures appear within healthcare organizations and vary within their associated subgroups. The healthcare and nursing work environments have been studied from the individualistic perspectives rather than from organizational or nurse managers’ group perspectives. The aim of this cross-sectional study was to examine the differentiation of organizational social contexts in hospitals. The organizational social context (OSC) measurement system was used to assess characteristics of culture, climate and morale. The data have been statistically analyzed using within-group consistency (rwg), ANOVA-based eta-squared values and interclass correlation coefficient (ICC). The results revealed that at the divisional level, the organization’s social context is different.
Organisaatiokulttuuri ja -ilmapiiri ovat yhteydessä palvelun laatuun, työntekijöiden tyytyväisyyteen ja organisaation menestymiseen. Aikaisemman tutkimuksen perusteella johtajien ja eturivin työntekijöiden välillä ei ole ollut jaettua käsitystä vallitsevasta organisaatiokulttuurista ja -ilmapiiristä. Hoitotyön kontekstissa vastaavaa tutkimusta ei ole tehty. Yhteinen kulttuuri- ja ilmapiirikäsitys esimiesten ja työntekijöiden välillä voi edistää organisaation kehitystä. Tutkimuksen tarkoituksena on verrata julkisen terveydenhuollon hoitohenkilökunnan sekä heidän lähiesimiestensä organisaatiokulttuuri- ja -ilmapiirinäkemyksiä. Tutkimukseen osallistui julkisen terveydenhuollon hoitohenkilökuntaa (n=266) sekä heidän lähiesimiehiään (n=246) 11 sairaanhoitopiiristä. Kohdeorganisaatioiden kulttuuria ja -ilmapiiriä tutkittiin Organisaation sosiaalinen konteksti -mittarilla. Aineistoa kuvailtiin standardoitujen T-arvojen keski- ja hajontaluvuilla, analysoinnissa hyödynnettiin lineaarista regressioanalyysiä sekä sekamallinnusta. Lähiesimiehet arvioivat organisaatiokulttuurin kokonaisuudessaan sekä organisaatioilmapiirin osallisuuden ja funktionaalisuuden hoitohenkilökuntaa positiivisemmin. Operatiivisella ja konservatiivisella toimialueella lähiesimiehet kokivat vähemmän organisaatiokulttuurin joustamattomuutta. Lisäksi konservatiivisen toimialueen lähiesimiehet kokivat vähemmän organisaatioilmapiirin stressiä sekä enemmän osallisuutta ja funktionaalisuutta. Operatiivisen sekä psykiatrisen toimialueen lähiesimiehet arvioivat organisaatiokulttuurin hoitohenkilökuntaansa pätevämmäksi. Sairaanhoitopiireittäin tarkasteltuna hoitohenkilökunnan ja lähiesimiesten näkemykset erosivat organisaatiokulttuurin joustamattomuuden sekä organisaatioilmapiirin stressin ja funktionaalisuuden osalta. Eri sairaanhoitopiireissä ryhmän vaikutus korostui organisaatiokulttuurin joustamattomuuden ja vastarinnan kokemisessa. Ilmenneet lähiesimiesten ja hoitohenkilökunnan näkemyserot osoittavat tarpeen organisaatiokulttuurin ja -ilmapiirin kehittämiselle yhteisen näkemyksen saavuttamiseksi organisaation sosiaalisesta kontekstista. Organisaatiomuutoksissa on tarpeen ottaa huomioon työyhteisöjen erilaiset näkemykset ympäröivästä kulttuurista ja ilmapiiristä sekä työntekijöiden rooli ja asema organisaatiossa.
BackgroundFinnish on-line Health village environment, so-called HUS-MAP©, was used to develop on-line information for rheumatic patients.ObjectivesTo face the challenge of increasing demand for rheumatological care without additional resources a new virtual client-orientated service was developed.MethodsIn Finland, an on-line Health village environment, so-called HUS-MAP©, has been developed to offer virtual digital information to citizens, patients, and healthcare professionals. By using this HUS-MAP© environment, we provide information and support to citizens and patients who need information on different rheumatic diseases and various unspecified aches and musculoskeletal pains. The study group consisted of participants from every university hospital in Finland, from local authorities; rheumatologists, other specialists, general practitioners, nurses, special professionals such as physical therapists, and patients. Information provided was based on updated international and Finnish guidelines on rheumatic diseases. After the information was reviewed and accepted by our study group, it was fed into a special program and published on-line in the Health village environment.ResultsE-service for citizens and patients was opened in March 2017. Detailed description of symptoms, diagnosis and therapy of various rheumatic diseases, such as rheumatoid arthritis, spondyloarthritis, connective tissue diseases and vasculitis are available. Early diagnosis and treatment to remission when possible is emphasised. The service aids to identify, by using symptom navigator and questionnaires for back pain, as well as swollen and painful joints those who may have a rheumatic disease and guide patients to find medical services without delay. Already 60 500 persons have visited our website.ConclusionsFinnish e-service, rheumaticdiseases.fi, offers an easy way to reach people and increase citizen’s awareness on their own health.
