BackgroundTorsional malalignment syndrome (TMS) is a well defined condition consisting of a combination of femoral antetorsion and tibial lateral torsion. The axis of knee motion is medially rotated. This may lead to patellofemoral malalignment with an increased Q angle and chondromalacia, patellar subluxation and dislocation. Conservative management is recommended in all but the most rare and severest cases. In these cases deformity correction requires osteotomies at two levels per limb.Materials and methodsFrom 1987 to 2002 in our institution three patients underwent double femoral and tibial osteotomy for TMS bilateral correction (12 osteotomies). All patients were reviewed at mean follow-up of 16 years.ResultsAt final follow-up no patients reported persistence of knee or hip pain. At clinical examination both lower limbs showed a normal axis and a normal patella anterior position. Pre-operative femoral version measurement showed an average hip internal rotation of 81.5° (range 80°–85°) and average hip external rotation of 27.2° (10°–40°). Thigh–foot angle measurement showed an average value of 38.6° (32°–45°). At final follow-up femoral version measurement showed an average hip internal rotation of 49° (range 45°–55°) and average hip internal rotation of 44.3° (20°–48°) (Figs. 1, 2, 3, 4, 5, 6). Thigh–foot angles measurement showed an average value of 21.6° (18°–24°) outward.ConclusionWe recommend a clinical, radiographical and CT scan evaluation of all torsional deformity. In cases of significant deformity, internally rotating the tibia alone is not sufficient. Ipsilateral outward femoral and inward tibial osteotomies are our current recommendation for TMS, both performed at the same surgical setting.
Kudo's pit pattern classification is an accurate diagnostic method for the differentiation of neoplastic colorectal lesions. Publication bias is significant in the current available literature.
Surface treatment remained a key solution to numerous problems of synthetic hard tissues. The basic methods of implant surface modification include various physical and chemical deposition techniques. However, most of these techniques have several drawbacks such as excessive cost and surface cracks and require very high sintering temperature. Additive mixed-electric discharge machining (AM-EDM) is an emerging technology which simultaneously acts as a machining and surface modification technique. Aside from the mere molds, dies, and tool fabrication, AM-EDM is materializing to finishing of automobiles and aerospace, nuclear, and biomedical components, through the concept of material migrations. The mechanism of material transfer by AM-EDM resembles electrophoretic deposition, whereby the additives in the AM-EDM dielectric fluids are melted and migrate to the machined surface, forming a mirror-like finishing characterized by extremely hard, nanostructured, and nanoporous layers. These layers promote the bone in-growth and strengthen the cell adhesion. Implant shaping and surface treatment through AM-EDM are becoming a key research focus in recent years. This paper reports and summarizes the current advancement of AM-EDM as a potential tool for orthopedic and dental implant fabrication. Towards the end of this paper, the current challenges and future research trends are highlighted.
Biomaterials play a significant role in revolutionizing human life in terms of implants and medical devices. These materials essentially need to be highly biocompatible and inert to the human physiological conditions. This paper provides an in-depth, critical and analytical review on the previous research work and studies conducted in the field of metals and alloys used as implant materials including stainless steel, titanium and its alloys, cobalt chromium and others. Since the manufacturing of medical implants relies on selected grades of biomaterials, metals play a significant role in biomaterials market. This paper focuses on highlighting some basic principles of manufacturing implant materials underlying composition, structure and properties of these materials. Finally, attention is also given to the role of these implant materials on the betterment of human life in terms of their failures by critically analysing these materials.
Both EIS and EVL produce excellent results, are safe, effective, feasible, and acceptable for EVB with minimum complications and obviate need for subsequent procedures in the short term. To make better choice, we should consider the hospital conditions, operator experience, and the characteristics of esophageal varices.
Due to the high-measuring cost, the monitoring of power quality (PQ) is nontrivial. This paper is aimed at reducing the cost of PQ monitoring in power network. Using a real-world PQ dataset, this paper adopts a learn-from-data approach to obtain a device latent feature model, which captures the device behavior as a PQ transition function. With the latent feature model, the power network could be modeled, in analogy, as a data-driven network, which presents the opportunity to use the well-investigated network monitoring and data estimation algorithms to solve the network quality monitoring problem in power grid. Based on this network model, algorithms are proposed to intelligently place measurement devices on suitable power links to reduce the uncertainty of PQ estimation on unmonitored power links. The meter placement algorithms use entropy-based measurements and Bayesian network models to identify the most suitable power links for PQ meter placement. Evaluation results on various simulated networks including IEEE distribution test feeder system show that the meter placement solution is efficient, and has the potential to significantly reduce the uncertainty of PQ values on unmonitored power links.Index Terms-Bayesian networks (BNs), conditional entropy (CE), Monte Carlo (MC) simulations, power quality (PQ) monitoring.
Multidrug resistance (MDR) has been a potentiator for the exploration of antibiotics. Nano drug delivery systems have opened new avenues to overcome this challenge. Although antibacterial nanocarriers are extensively realized, their effect on the bacteria residing inside the tissues and their toxicity is rarely explored. This study investigated the effects of flavonoid coated gold nanoparticles (FAuNPs) on the colonization of Enterococcus faecalis in the mouse liver and kidneys. Flavonoids were extracted from the leaves of Berberis lycium Royle and used to stabilize gold following a green synthesis approach. FAuNPs were characterized by ultraviolet-visible (UV-Vis) spectroscopy, Fourier-transform infrared spectroscopy (FTIR), scanning transmission electron microscopy (STEM), X-ray powder diffraction (XRD), and energy-dispersive X-ray spectroscopy (EDS). FAuNPs showed significantly higher reduction in bacterial counts in in-vitro and in-vivo in mice organs as compared to the free flavonoids owing to their biocompatibility and effectiveness.
