ObjectivesPatients with autoimmune inflammatory rheumatic diseases (AIRD) often have lower vaccination coverage rates compared with the general population, despite being disproportionately affected by infectious complications. We aim to systematically review the literature regarding vaccination willingness and hesitancy in AIRD.MethodsA scoping review was conducted in PubMed, EMBASE and the Cochrane Library in June 2021. Study selection was performed by two independent reviewers and data were extracted using a standardised form. Risk of bias was assessed using instruments from McMaster University. Identified barriers were categorised into the WHO’s measuring behavioural and social drivers (BeSD) of vaccination conceptual model.ResultsThe search yielded 1644 hits of which 30 publications were included (cross-sectional studies based on interviews (n=27) and intervention studies (n=3)). The majority of studies reported barriers to influenza and pneumococcal vaccination only (n=9) or in combination with another vaccination (n=8) from the patients’ perspective. Only one study assessed the view of rheumatologists. Coverage of domains matched to the BeSD model suggests a lack of awareness of infection risk by both patients and physicians. Patients mainly mentioned behavioural and social factors that negatively influenced their willingness to be vaccinated while physicians mentioned organisational deficits as major barriers.ConclusionsThe view on vaccination in patients with AIRD diverges between patients and rheumatologists. Our results show that in-depth counselling on vaccines is important for patients, whereas physicians need support in implementing specific immunisation recommendations. The themes identified provide a starting point for future interventions to improve vaccine rates in patients with AIRD.
Zusammenfassung Hintergrund Die Psychotherapie-Richtlinie (PT-RL) gestaltet die Rahmenbedingungen der Psychotherapie in der vertragsärztlichen Versorgung in Deutschland. Aufgrund der Diskussion um Wartezeiten und inadäquate Versorgung von Betroffenen trat 2017 eine Reform in Kraft, um den Zugang zur Psychotherapie sowie den gesamten Versorgungs- und Behandlungsverlauf zu verbessern. Ziel des Projektes ist die Evaluation der neu eingeführten Versorgungselemente in der PT-RL sowie die Identifikation möglicher Implementierungsschwierigkeiten. Methoden Zur Evaluation der Reform der PT-RL finden sowohl quantitative als auch qualitative Methoden Anwendung. Um einen Vergleich der Versorgungssituation vor und nach der Reform durchzuführen, wird eine vergleichende retrospektive, kohortenbasierte Analyse auf Basis anonymisierter Routinedaten der BARMER und der AOK durchgeführt. Zusätzlich wird mit Hilfe einer Querschnittsanalyse erfasst, wie die neuen Versorgungselemente in der Praxis umgesetzt werden. Dazu erfolgt eine Befragung von Psychotherapeut*innen, Hausärzt*innen und Patient*innen. Fokusgruppen mit relevanten Interessensgruppen stellen zusammen mit einer Literaturrecherche die Basis für die Befragung dar. Zusammen mit den Routinedaten zur Versorgungssituation nach Reform der PT-RL und aggregierten, anonymisierten Daten der KBV zur vertragsärztlichen Versorgung sowie zur Inanspruchnahme der Terminservicestellen ergeben die Befragungsergebnisse die Datenbasis für die Querschnittsanalyse. Schlussfolgerung Durch die Verbindung von quantitativen und qualitativen Daten können die Auswirkungen der Strukturreform der PT-RL von 2016 auf individueller und struktureller Ebene sowie in Bezug auf den gesamten Versorgungs- und Behandlungsprozess analysiert werden. Darauf aufbauend werden Vorschläge für eine bedarfsgerechte Weiterentwicklung der PT-RL unter Berücksichtigung der Perspektiven verschiedener Interessensgruppen ausgearbeitet.
Zusammenfassung Hintergrund Psychische Erkrankungen verursachen sowohl individuelle als auch gesellschaftliche Belastungen. Zentrales Ziel der Strukturreform der Psychotherapie-Richtlinie (Inkrafttreten 2017) war die verbesserte Steuerung des Zugangs zur psychotherapeutischen Versorgung. Die hier vorgestellten Fokusgruppen, die im Rahmen des durch den Innovationsfonds geförderten Forschungsprojekts „Evaluation der Psychotherapie-Richtlinie (Eva PT-RL)“ durchgeführt wurden, adressieren die Umsetzung der Ziele der Reform und der einzelnen neu eingeführten Versorgungselemente sowie Hürden bei der Implementierung aus der Perspektive von Leistungserbringer*innen, Patient*innen und Kostenträgern. Methoden Es wurden sechs Fokusgruppen und fünf Einzelinterviews mit Personen der drei genannten Stakeholder-Gruppen durchgeführt. Grundlage war ein an die jeweilige Personengruppe angepasster auf Basis einer strukturierten Literaturrecherche erstellter halbstrukturierter Gesprächsleitfaden. Die Gespräche wurden von einem Moderatorinnenteam per Videokonferenz durchgeführt, aufgezeichnet und transkribiert. Die Auswertung erfolgte über eine qualitative Inhaltsanalyse in Anlehnung an Mayring. Ergebnisse Eine allgemein positive Beurteilung insbesondere hinsichtlich des Erstzugangs zur psychotherapeutischen Versorgung erfuhr die psychotherapeutische Sprechstunde, wobei gleichzeitig ein verzögerter Übergang zur gegebenenfalls folgenden Richtlinien-Psychotherapie insbesondere aufgrund Kapazitätenmangels kritisch diskutiert wurde. Es wurden günstige Effekte der telefonischen Erreichbarkeitszeiten sowie einer verstärkten Vernetzung der psychotherapeutischen Versorgung auch mit anderen psychosozialen Angeboten angesprochen. Die Umsetzung der Akutbehandlung und der Rezidivprophylaxe wurde jedoch eher kritisch diskutiert. Als zentrales Ergebnis zeigt sich, dass der durch die Reform angestrebte Anstoß zum Paradigmenwechsel von Psychotherapeut*innen unterschiedlich wahrgenommen und umgesetzt wird – von einem Fokus auf individuelle Therapieprozesse hin zur Public Mental Health-Perspektive mit einer Verpflichtung zur Versorgungssteuerung. Schlussfolgerung Angesichts der variablen Strategien zur Umsetzung der Strukturreform durch die Psychotherapeut*innen, der Kritik an der Ausgestaltung einzelner Reformelemente und der Diskussion um Verzögerungen beim Zugang zur Richtlinientherapie aus der psychotherapeutischen Sprechstunde besteht weiterer Forschungsbedarf. Die Ergebnisse der Fokusgruppen dienen als Grundlage für die folgenden Projektschritte, u. a. eine Befragung von Psychotherapeut*innen, Patient*innen und hausärztlich tätigen Ärzt*innen.
