Background: Hirsutism is a common clinical condition encountered in day-to-day practice. The androgenic causes account for more than 80% of these patients and include polycystic ovary syndrome (PCOS), which affects about 70%-80% of hirsute women. The second most common cause is idiopathic hirsutism. Omentin-1 is an adipokine mainly produced by visceral adipose tissue. Aim:The current study aimed at evaluating omentin-1 levels in hirsute females with PCOS and in idiopathic hirsutism.Patients and Methods: Eighty-five females were included in this study. They were classified into three groups: thirty hirsute patients with PCOS, thirty females with idiopathic hirsutism, and twenty-five healthy control females. The participants were subjected to history taking, physical and dermatological examination. A gynecological history and radiological examination of the ovary also were done. Serum testosterone and omentin-1 were measured by ELISA.Results: Serum testosterone was statistically elevated in PCOS than other groups.Serum omentin-1 in females with idiopathic hirsutism was statistically significantly higher than control and PCOS. There was a significant inverse correlation between serum testosterone level and serum omentin-1 level. Conclusion:Omentin-1 may be involved in the pathogenic process of hirsutism.
Background: Multiple epidemiological data showed a significant higher prevalence of hypovitaminosis D in patients with Parkinson's disease (PD). Objectives: To assess the serum vitamin D level in patients with Parkinson's disease and to investigate the possible relationship between the serum vitamin D level and both motor and cognitive symptoms in Parkinson's disease Materials and methods: A case-control study was conducted on 25 patients who fulfilled the criteria for diagnosis of idiopathic Parkinson's disease based on the British Brain Bank criteria, and 25 healthy volunteers. Selected PD patients were submitted for assessment of cognitive function using the PD-Cognitive Rating Scale (PD-CRS) and assessment of motor function using the Unified Parkinson's Disease Rating Scale (UPDRS). Serum 25 hydroxy vitamin D level was measured for all the included patients and controls. Results: PD patients were found to have a significantly lower level of serum vitamin D than controls (P value = 0.001). There was a statistically significant negative correlation between the serum vitamin D level and the scores of motor, mentation, activities of daily living, medication complication, other complications, and the total score of UPDRS (P value = 0.01, < 0.001, 0.012, 0.017, 0.039, and 0.002 respectively). There was a statistically significant positive correlation between the serum vitamin D level and the scores of attention, working memory, immediate recall, delayed recall, naming, visuoperceptual abilities, visuoconstructional abilities, alternating verbal fluency, action verbal fluency, and the total score of PD-CRS (P value < 0.001 in all parameters). Conclusion: Vitamin D deficiency is evident in PD patient, and such deficiency significantly affected both motor and cognitive symptoms.
Objective: To compare the early recovery after surgery (ERAS) pprotocol with the conventional one in women undergoing elective CS. Patients and Methods: The study included 96 women undergoing elective cesarean section for different reasons. They were randomly divided into two groups 48 patients each, Group (A) received the ERAS regimen and Group (B) was managed with the conventional care. Women with major medical or obstetric disorders were excluded. Results: Cases' age ranged between 18-35 years without significant difference between groups. Also, gestational age, haemoglobin concentration and platelet count were comparable between groups. Intra and post-operative nausea and vomiting were significantly higher (p value <0.0366) in the control group (8 vs 17). Group A had significantly shorter interval to oral intake, ambulation, first intestinal sound and first motion. Moreover, the need to use opiate for pain control with overall pain scores were significantly lower in study group with significantly better satisfaction rates and shorter hospital stay. Conclusion: ERAS protocol for women planned for elective CS is effective in controlling perioperative gastrointestinal symptoms, pain control and encourages early ambulation with offering earlier resumption of intestinal motility, higher satisfaction and fewer days of admission.
Background: Hidradenitis suppurativa is a chronic inflammatory condition that affects apocrine gland-bearing areas‚ causing abscesses and sinuses. Multimodality treatment is suggested for management. The surgical option is becoming more widely used, especially in drug-resistant cases. In this article, we describe a series of bilateral axillary hidradenitis cases which we treated with wide local excision and immediate reconstruction with lateral chest flap. Methods: Fourteen patients presented to our clinic with bilateral hidradenitis suppurativa of the axilla. The cases were all resistant to medical treatment. They were managed by excision and simultaneous reconstruction with lateral chest flaps. Results: At 3 months postoperatively, all patients had full shoulder range of motion and were completely satisfied with the aesthetic outcome, except for one patient who complained of the bulky look of his axilla. Liposuction was performed for him‚ with a pleasant resultant outcome. Conclusions: Our patients underwent wide local excision of bilateral disease plus reconstruction with lateral chest flaps in the same session. Our aim was to introduce a treatment option for moderate to severe axillary hidradenitis suppurativa that offers good aesthetic and functional outcomes.
