Celoviti tretman afektivnih poremećaja, i to posebno različitih formi depresije još uvek predstavlja značajan izazov u kliničkom radu. Aktuelni prikaz slučaja ima za cilj upravo razmatranje izazova u svakodnevnoj kliničkoj praksi u slučajevima kompleksne dijagnostike, diferencijalne dijagnostike i tretmana rekurentne, teške depresivne epizode, kao i razmatranje uloge dodatne terapije. U aktuelnom radu prikazana je pacijentkinja srednjih godina, lečena na Klinici za psihijatriju Univerzitetskog kliničkog centra Srbije, u okviru rekurentne, teške depresivne epizode, sa prethodnim višestrukim pokušajima suicida. Na prethodno primenjeni antidepresiv iz grupe inhibitora ponovnog preuzimanja serotonina i noradrenalina i stabilizator raspoloženja litijum karbonat, pacijentkinji je u terapiju uveden atipični antipsihotik kariprazin što je dovelo do značajnog kliničkog poboljšanja i subjektivnog zadovoljstva pacijentkinje. Upotreba atipičnih antipsihotika, među njima i kariprazina, kao psihofarmaka sa multidimenzionalnim delovanjem u lečenju teških i izazovnih oblika afektivnih poremećaja, od posebne je važnosti sa aspekta svakodnevne kliničke prakse. Samo detaljno, integrativno i individualno prilagođeno sagledavanje terapijskih mogućnosti, bazirano na naučnim saznanjima i praktičnim iskustvima, može dovesti do njihove adekvatne, pravovremene i utemeljene kliničke primene, čiji je cilj pre svega bolje funkcionisanje pacijenata sa afektivnim poremećajima kako po objektivnim kriterijumima, tako i prema subjektivnom/ličnom doživljaju funkcionisanja.
Temperament podrazumeva genetsko-biološke komponente ponašanja u koje spadaju raspoloženje, način emocionalnog reagovanja u smislu brzine, intenziteta, trajanja i vrste reakcije, odnos prema okolini, i osetljivost na spoljašnje stimulus. Pokušaji klasifikacije temperamenata datiraju još iz vremena Stare Grčki, a savremeni koncept temperamenta zasniva se na radu Krepelina i Krečmera. Hagop Akiskal je nastavio razvoj savremenog koncepta temperamenta i podelio vrste afektivnog temperamenta na: depresivni, hipertimni, ciklotimni, iritabilni i anksiozni temperament. U zdravoj populaciji temperament predstavlјa vrlo važnu, biološku, osnovu ličnosti. Međutim, danas se smatra da se dominantni afektivni temperamenti potencijalno nalaze na kontinuumu između zdrave emocionalne reaktivnosti i afektivnih poremećaja. Takodje, dominantni afektivni temperamenti u naglašenoj formi mogu predstavljati subkliničke manifestavije afektivnih poremećaja, te i prekursore za nastanak ovih poremećaja. Zasnovani na snažnim genetičkim i neurohemijskim osnovama, afektivni temperamenti su se razvili i zadržali tokom tokom evolucije jer nose izrazite evolutivne i socijalne prednosti. Tako, pored patološke, temperamentalne karakteristike mogu imati i adaptivnu vrednost. Dodatna istraživanja na temu sagledavanja afektivnih poremećaja kao kontinuuma nasuprot njihovoj kategorijalnoj klasifikaciji, uz uvažavanje prelaznih oblika koji zauzimaju od zdravlja ka bolesti bi doprinela ne samo boljem razumevanju emocionalnog života čoveka nego i pravovremenom, ciljanom i kvalitetnijem tretmanu različitih psihopatoloških oblika.
Pacijenti na hemodijalizi su u povećanom riziku od kognitivnog propadanja i razvoja demencije. Sam tok i priroda hronične bubrežne insuficijencije predstavljaju faktore rizika za kognitivno propadanje. Pored toga pacijenti na hemodijalizi su izloženi čestim hemodinamskim stresovima koji utiču na cerebralnu perfuziju na koje su posebno vunerabilni usled čestih komorbiditeta koji nose kardiovaskularni rizik. Takođe kod ove populacije su česte metaboličke abnormalnosti, poremećaji acido-bazne ravnoteže, disbalans vode i elektrolita, pojava inflamacije i oksidativnog stresa i nakupljanje toksičnih materija koji deluju kao faktori rizika za poremećaje kognicije. Adekvatno kognitivno funkcionisanje ovih pacijenata je bitno kako bi oni bili sposobni da vode računa o svom zdravlju i usvajaju medicinske savete, i kako bi imali što bolji kvalitet života. Iz tih razloga veoma je bitna rana detekcija faktora koji dovode do kognitivne disfunkcije kod ove populacije i blagovremena intervencija kako bi se ovi faktori rizika ublažili.
