This study compared the antipyretic effectiveness of acetaminophen, ibuprofen, and dipyrone in young children with fever. The results were based on a modified double-blind, randomized, multinational trial that evaluated 628 febrile children, aged 6 months to 6 years. All three drugs lowered temperature in the 555 patients completing the study. Temperature normalization rates in the ibuprofen and dipyrone groups (78% and 82%, respectively) were significantly higher than the acetaminophen group (68%, P = 0.004). After 4 to 6 hours, mean temperature in the dipyrone group was significantly lower than the other groups, demonstrating longer temperature normalization with dipyrone. All three drugs showed comparable tolerability profiles.

A identificação desta bactéria direcionou a investigação para a síndrome MIM. O diagnóstico de deficiência do receptor da interleucina-12 (IL-12Rß1) foi confirmado através da dosagem de IL-12 e do interferon (IFN)-γ produzido pelas células do paciente em meio de cultura. O resultado demonstrou ausência de produção de IL-12 e do IFN-γ, mesmo após estímulo adequado.Comentários: a identificação da Salmonella enteritidis como agente etiológico de septicemia sugere uma disfunção do sistema imunológico. Foi realizada avaliação laboratorial das imunidades humoral, celular e inata. Após avaliação laboratorial direcionada para síndrome MIM, foi confirmada a deficiência do receptor da interleucina-12 (IL-12Rß1). O uso do IFN-γ é recomendado nos casos graves, assim como o tratamento de suporte e o aconselhamento genético.
Carcinogenesis induced by high-risk human papillomavirus (HPV) involves inflammatory phenomena, partially mediated by cyclooxigenase-2. In pre-clinical models of HPV-induced cancer, cyclooxygenase-2 inhibitors have shown significant efficacy, but also considerable toxicity. This study addresses the chemopreventive effect and hepatic toxicity of a specific cyclooxigensase-2 inhibitor, parecoxib, in HPV16-transgenic mice. Forty-three 20 weeks-old female mice were divided into four groups: I (HPV16−/−, n = 10, parecoxib-treated); II (HPV16−/− n = 11, untreated); III (HPV16+/−, n = 11, parecoxib-treated) and IV (HPV16+/−, n = 11, untreated). Parecoxib (5.0 mg/kg once daily) or vehicle was administered intraperitoneally for 22 consecutive days. Skin lesions were classified histologically. Toxicological endpoints included genotoxic parameters, hepatic oxidative stress, transaminases and histology. Parecoxib completely prevented the onset of epidermal dysplasia in HPV16+/− treated animals (0% versus 64% in HPV16+/− untreated, p = 0.027). Parecoxib decreases lipid peroxidation (LPO) and superoxide dismutase (SOD) activity and increases the GSH:GSSG ratio in HPV16+/− treated animals meaning that oxidative stress is lower. Parecoxib increased genotoxic stress parameters in wild-type and HPV16-transgenic mice, but didn’t modify histological or biochemical hepatic parameters. These results indicate that parecoxib has chemopreventive effects against HPV16-induced lesions while maintaining an acceptable toxicological profile in this model.

AbstractObjective: to present a case report of a child who developed sepsis by Salmonella enteritidis associated with the diagnosis of primary immunodeficiency.Description: a twenty-one months old boy presenting fever and skin lesions, bilateral pneumonia with pleural effusion and septic shock. Salmonella enteritidis was isolated in blood cultures and pleural fluid. The identification of the bacteria suggested the presence of the MIM syndrome. The diagnosis of IL-12Rß1 was confirmed after IL-12 and IFN-γ levels were measured using patient cells in a culture medium. The results showed absence of IL-12 and the IFN-γ post stimulation using BCG.Comments: a severe infection by Salmonella enteritidis is strongly suggestive of an immune system dysfunction. Laboratory tests for humoral, cellular and innate immunity were performed. Interleukin 12 receptor ß1 (IL-12 Rß1) deficiency was confirmed after specific laboratory evaluation. ResumoObjetivo: Avaliar a eficácia e a tolerabilidade de duas cefalosporinas orais -cefprozil e cefaclor -no tratamento da OMA em crianças.Métodos: Quarenta crianças com diagnóstico clínico de otite média aguda foram randomizadas em dois grupos: vinte e uma crianças foram medicadas com cefprozil na dose de 30mg/kg/ dia em duas tomadas diárias; dezenove crianças receberam cefaclor na dose de 40mg/kg/dia em três tomadas diárias; ambos por dez dias. A avaliação da eficácia do antibiótico foi analisada pela presença ou ausência de sintomas, de febre e de alterações otoscópicas, e a da tolerabilidade, pela informação espontânea de possíveis eventos adversos.Resultados: Houve diminuição significante do número de alterações entre o 3º e 5º dia de tratamento para o grupo cefprozil em relação ao cefaclor, enquanto entre o 10º e 14º após o início do tratamento o índice de cura foi clinicamente semelhante. Ambos os medicamentos foram bem tolerados. Apenas três crianças que receberam cefprozil apresentaram eventos adversos leves (naúseas e vômitos), sendo que nenhuma precisou interromper o tratamento.Conclusões: A eficácia clínica das duas cefalosporinas foi semelhante, com início de ação mais rápida do cefprozil em relação ao cefaclor, entre o 3º e 5º dias de tratamento. Os resultados favoráveis obtidos, a baixa incidência de eventos adversos e a cômoda posologia (a cada 12 horas) sugerem ser o cefprozil mais um antimicrobiano adequado para tratamento de OMA em crianças, quando essa conduta terapêutica for indicada.J. pediatr. AbstractObjective: To evaluate the efficacy and safety of two oral cephalosporins, cefprozil and cefaclor, in the treatment of acute otitis media in children Methods: Forty children with clinical diagnosis of acute otitis media were randomized in two groups. Twenty-one children were given cefprozil at the dose of 30/mg/kg/day, in two equally divided doses every twelve hours, for ten days. Nineteen children were given cefaclor at the dose of 40/mg/kg/day in three divided doses every eight hours, for ten days. Efficacy evaluation of the antibiotic was analyzed by the presence or absence of symptoms, fever and otoscopic abnormalities while the safety and tolerability were evaluated by spontaneous information for possible adverse events.Results: There was a significant decrease in the number of observed alterations between days 3 and 5 of treatment among those of the cefprozil group in comparison to the cefaclor group, while between days 10 and 14 of treatment the observed cure rate was clinically similar in both groups. Both medications presented adequate safety profiles. Only three children receiving cefprozil presented mild adverse events, such as nausea and vomit, none of which required treatment interruption.Conclusions: The clinical efficacy of both cephalosporins was similar although a more rapid activity was observed for cefprozil between days 3 and 5 of treatment.
