Background: Little is known about the cognitive signature of bipolar disorder (BD) in elderly brains. The neuropsychological features of depressive elderly with early-onset BD are largely unknown. This issue is relevant because cognitive impairment can produce an additional impact on the already compromised functionality of elderly with BD. The aim of this study is to assess executive functions (EFs) in the depressive phase of elderly outpatients with early-onset BD.Methods: Forty-nine elderly outpatients with early-onset BD were assessed with several neuropsychological tests for EF in the depressive phase of the disorder.Results: Executive dysfunction is very common in old age bipolar depression. Thirteen patients (26.5%) had a pseudodementia presentation. The worst performances were observed in the following tests: Trail Making B, Stroop Test 3, Backward Digit Span and Wisconsin Card Sorting Test.Conclusion: Executive dysfunction profile in elderly BD is complex and heterogeneous, but most cases display difficulties in working memory, inhibitory control, mental flexibility, and information processing speed. The performance of elderly with bipolar depression in executive assessment can be divided into two main categories: (1) Single EF domain impairment; and (2) Multiple EF domain impairment with or without a pseudodementia syndrome. Executive dysfunction in old age bipolar depression may be explained by lack of sufficient mental energy to run those cognitive processes that require larger amounts of effort to be performed.
Introduction: TMS is said to be an effective technique for motor and cognitive rehabilitation for acquired neurological lesions. This study aims to evaluate the effect of TMS in the cognition of patients after stroke. Methods: This prospective, longitudinal and interventional study was approved by the Ethics Committee (Protocol No. 54977216.3.0000.5078) and included 16 stroke victims aged from 24 to 74 years. The Montreal Cognitive Assessment (MoCA) test was used before and after the stimulation sessions and TMS was administered according to treatment protocols for a motor goal, with inhibitory (1 hz) TMS stimulation over the right and left primary motor cortex; and according to protocol for the prefrontal cortex involved in humor processing, with stimulation (10 hz) of the left dorsolateral prefrontal cortex, and inhibitory (1 hz) stimulation of the right dorsolateral prefrontal cortex. The patients underwent fifteen treatment sessions, on average. Results: Memory subtests showed improvement, and average and standard deviation values for the pre- and post-intervention periods were [2.06 (1.6) and 3.5 (1.5)], respectively. In terms of total performance, MoCA results were [18.7(3.4) and 21.1(4.03)]. Student’s t test indicated p=0.006 for performance differences in memory and p=0.003 for total performance. Conclusion: TMS was shown to be effective in achieving cognitive rehabilitation after strokes, most notably in terms of the recovery of mnemonic functions
The non-linear analysis of the heart rate variability (HRV) is a potential correlation tool between physiological and emotional states. The aim of this study was correlate changes in non-linear parameters of the HRV with anxiety and depression indices in higher education entrants from Federal University of Goiás (UFG). To record HRV, the Polar Frequency Metermodel Polar® RS800cx (Electro Oi, Finland) was used. To analyze the levels of depression, the Beck Depression Inventory-I (BDI) was used, and for analysis of anxiety, the Inventories of State and Trace (IDATE T and E). Altogether, 50 (19.4±2.48 years) volunteers participated in the study. The scores of the depression and anxiety inventories indicated that the public had low and moderate indices of both conditions, respectively. Statistically significant correlations were found between the SD2 parameter (from the Poincaré plot in the HRV analysis), entropy data (ApEn and SampEn) and fractal property (DFA: α1) and trait and state anxiety levels. Although ApEn e SampEn still do not have well-defined physiological correspondences, DFA: α1 has been associated with parasympathetic modulation of heart rate. Therefore, it is concluded that chronic negative emotional states, such as anxiety and humor disorders, can be accompanied by parasympathetic increase in HRV, so the organism would be able to not wear out against long-term stressors. New studies are necessary to understand the physiological and evolutional bases of depression and anxiety.
A Síndrome de Prader-Willi (SPW) é uma doença rara de origem genética que cursa com alterações endócrinas, metabólicas e neurológicas que trazem grande prejuízo funcional aos pacientes e familiares. O objetivo deste estudo é apresentar um caso clínico cujo desfecho foi o diagnóstico de Síndrome de Prader-Willi. Destaca-se aqui o processo de avaliação neuropsicológica que investigou as manifestações comportamentais e cognitivas. Participou deste estudo uma criança de 9 anos, sexo masculino com queixas de baixo rendimento e adaptação escolar. RESUMOA Dislexia é definida como uma defasagem na habilidade de leitura e escrita em relação à capacidade de compreensão e inteligência geral, e suposta educação formal de qualidade. Além dessas dificuldades, há concordância entre autores em relação a existência de um déficit metalinguístico envolvendo a dificuldade de nomeação rápida, a falta de consciência metafonológica e o déficit de memória de trabalho e dificuldade de sequencialiazação, fatores que dificultam a aquisição da leitura e da escrita. O objetivo do trabalho é comparar o desempenho em nomeação de figuras de crianças sem queixas escolares e crianças com Dislexia. Participaram da pesquisa 271 alunos do grupo controle sem histórico de repetência ou queixa escolar e 16 do grupo clínico, na faixa etária de 8 a 12 anos, com diagnóstico formalizado de dislexia. Utilizou-se o Teste de Nomeação de Figuras e realizou-se estatística descritiva (média) e inferencial (Mann-Whitney). Os resultados sugerem que os disléxicos de fato têm dificuldade ao nomear figuras corroborando dados da literatura. O estudo possibilitou ainda afirmar que algumas figuras são mais consensualmente nomeadas e que há itens do teste de figuras que devem ser melhor estudados. Estudos futuros poderão ainda discutir qualitativamente a natureza das dificuldades encontradas no grupo com dislexia. Estudos relacionados à déficits de categoria de figuras distintas devem ser implementados. O estudo aponta ainda para a possibilidade de, conhecidas as dificuldades de nomeação encontradas no grupo com dislexia, implementar projeto de reabilitação que envolva esta habilidade, bem como estudo de rastreio que possa, a partir da inabilidade de nomear figuras, oferecer suporte adicional com vistas a minimizar o impacto destas dificuldades no processo de aquisição de leitura e escrita.
Objective: Analyzes the coverage of the digital inclusion programs in health units. Method: The public data of the Cidades Digitais and Wi-fi Brasil programs were analyzed, extracting the number and type of health units supported by the programs, for each Brazilian region and state. Results: The North and Northeast regions have the largest number of health units attended by the programs. The collected data shows that 4.4% of the Wi-fi Brasil’s total access points are in health units and mainly in Indigenous Health Centers (28%), whilst Cidades Digitais corresponds to 4.3%, supporting the Basic Health Units (33%). Conclusion: To recognize the community’s level of access to the internet it is important to propitiate the digital inclusion in health units and then contribute to the National Health Data Network, following the Digital Health Strategy 2020-2028 guidelines.
CLÁUDIA SANTOS GONÇALVES BARRETO BEZERRA; MIRIAM DO ROCCIO GUADAGNINI RESUMO Introdução: Os prejuízos atencionais, bem como o transtorno de déficit de atenção/hiperatividade -TDAH constituem um verdadeiro desafio no ambiente escolar. O Pay Attention é um sistema de treinamento de processos atencionais que foi modelado de acordo com o Programa de Treinamento de Processos de Atenção. Foi traduzido para o português sendo mantido seu nome original e inclui atividades adequadas à reabilitação de problemas de processamento de atenção de crianças com idade de cinco a 14 anos. Evidências de melhora em grupos clínicos e não clínicos têm sido apresentadas. Objetivo: O objetivo deste trabalho foi implementar o programa Pay Attention como estratégia de treino atencional para um estudante com diagnóstico formalizado de TDAH. O projeto foi apresentado e aprovado pelo comitê de ética sob o número 40013520.4.0000.5083 e foi aprovado com o parecer 4.447.441. Relato do caso: Participou deste estudo uma criança de nove anos, sexo masculino, frequentando o 3º ano do Ensino Fundamental -Anos Iniciais, com diagnóstico formalizado de TDAH. Realizaramse 20 sessões de estimulação com duração de 20 minutos cada durante cinco semanas. A eficácia do programa foi monitorada pela Bateria de Provas de atenção -BPA e pelos subtestes de dígitos -Ordem direta (OD) e indireta -OI da Escala Wechsler de Inteligência para crianças -WISC-IV. Resultados e Discussão: Dentre as medidas aferidas encontrou-se uma pontuação de 121 para Atenção geral no período pré-intervenção, posicionando o participante no percentil <25, classificação médio inferior contra uma pontuação 178 no período pós-intervenção, o que posiciona o participante no percentil <60, classificação médio superior. Discutiu-se que os resultados obtidos após a mediação com o programa de estimulação da atenção Pay Attention indicaram a eficácia do programa. Do ponto de vista quantitativo, os ganhos em pontuações em testes padronizados foram robustos; do ponto de vista qualitativo, o estudante reportou melhora na concentração e na regulação do comportamento. Espera-se ampliar o número de atendimentos. Conclusão: pode-se afirmar que o prognóstico no uso do treino é favorável e que o programa apresentou evidência de eficácia.
