Objective: to discuss the ethical challenges faced by managers in the SUS in the process of distribution and allocation of health resources. Methodology: this is about a qualitative study based on literature research method, in the universe of publications in the field published from 1997 to 2009, with the following keywords: ethics, health resources, health manager, unified health system. It lists the tools used in decision-making in health. Subsequently brings the placement of managers facing this reality. Results: some decisions in health need to be based on clear criteria established after broad social debate. Such decisions are taken by representatives of the people, being essential to the existence of tools, like the principles of the SUS and bioethical issues, guiding policies, thus avoiding discretionary choices. Conclusion: bioethical principles and the principles of the SUS represent the main elements of the scheme, also representing the justifications for public health efforts. Because they are generalizable, and opposed in certain situations, the principles have applicability related to the worldview of those who are legally responsible for decision making. However, the aspect of guiding decisions must be grounded in the possibility of success of measures and contributing to the welfare of people. Metodologia: estudo qualitativo com método de investigação bibliográfica, com universo de publicações na área divulgadas entre 1997 e 2009, adotando os descritores: ética; recursos em saúde; gestor de saúde; SUS. Elenca ferramentas utilizadas no processo decisório na saúde. Posteriormente, traz o posicionamento dos gestores em relação a esta realidade. Resultados: decisões em saúde necessitam ser baseadas em critérios claros definidos após amplo debate social. Tais decisões são tomadas por representantes do povo, sendo essencial a existência de ferramentas, a exemplo dos princípios do SUS e bioéticos, que norteiam as políticas, evitando assim escolhas discricionárias. Conclusão: os princípios bioéticos e do SUS configuram peças fundamentais nas articulações, colocando-se como justificativas para ações em saúde. Por serem generalizáveis, e se contraporem em determinadas situações, os princípios tem sua aplicabilidade relacionada com a visão de mundo dos que legalmente são responsáveis pela tomada de decisão. Contudo, o aspecto orientador das decisões deve estar ancorado na possibilidade de êxito das medidas e na contribuição para o bem estar das pessoas. Metodología: estudio cualitativo basado en un método de investigación de la literatura, en el universo de las publicaciones en el área publicados entre 1997 y 2009, con las siguientes palabras clave: la ética, los recursos de salud, gerente de salud, Sistema Único de Salud. En él se enumeran las herramientas utilizadas en la toma de decisiones en salud. Posteriormente trae la colocación de los administradores frente a esta realidad. Resultados: las decisiones de salud deben basarse en criterios claros establecidos después de un debate social amplio. Estas decisiones son adoptadas por los representantes del pueblo, siendo esencial para la existencia de herramientas, como los principios del Sistema Único de Salud y las cuestiones de bioética, orientar las políticas, evitando así decisiones discrecionales.
