A provisão adequada de serviços de atenção primária à saúde e o acesso aos serviços especializados, também usualmente referidos como de média/alta complexidade, apresentam-se como uma dificuldade em vários municípios brasileiros, tendo em vista as questões gerenciais envolvidas que incluem a força de trabalho, os custos e a densidade tecnológica exigida. Este estudo objetivou compreender a prática discursiva de gestores em relação à articulação entre os níveis de atenção primária e de média/alta complexidade dos serviços públicos de saúde na Região Metropolitana de Curitiba (RMC). Realizouse um estudo exploratório de abordagem qualitativa com 17 gestores de saúde da RMC. Os dados discursivos foram coletados por meio de entrevista semiestruturada, sendo processados pelo método de análise de discurso. A interpretação do material permitiu a classificação dos dados discursivos, que foram agrupados em categorias: atenção primária municipal, acesso na média complexidade, atendimento hospitalar, saúde bucal e articulação política. A análise demonstrou que os municípios se encontram em diferentes estágios de implantação e organização da atenção primária. A dificuldade no acesso aos serviços de média/alta complexidade promove interrupção na continuidade das linhas de cuidado, as quais visam à integralidade da atenção em saúde e implicam a adequada coordenação da atenção primária articulada à atenção especializada de segundo e/ou terceiro níveis de densidade tecnológica.Pa l a v r a s -c h a v e : políticas públicas de saúde; acesso aos serviços de saúde; níveis de cuidado em saúde; iniquidades em saúde.Artigo recebido em 29 out. 2012 e aceito em 16 abr. 2013.
This study, conducted on public roads in Curitiba, shows a noticeable proportion of drivers voluntarily self-reporting drinking and driving. However, a smaller percentage was confirmed to have positive blood alcohol content, likely due to the high breathalyzer refusal rate.
The aim of this study was to identify and analyze inequities in the access to specialized services in the municipalities of the metropolitan area of Curitiba, Paraná, Brazil. This is an ecological study. In its preparatory stage, this study focused on the socioeconomic, epidemiologic, healthcare network and sectoral financing network profiles of the 26 municipalities comprising this area. Factor analysis was employed to obtain the six principal components, and a synthetic index was calculated from them, allowing municipalities to be ranked according to living conditions and health situation. Primary data was collected from 24 municipalities, regarding their capacity, directed and repressed demand of specialized healthcare services. The context analysis revealed accentuated intermunicipal inequities. The synthetic index allowed municipalities to be classified in four relatively homogeneous groups regarding living and health conditions. Municipalities located in Vale do Ribeira obtained the worse outcomes for the Living Conditions and Health Situation Synthetic Index, as well as the higher repressed demand for specialized healthcare services. The geographical distance from the capital showed to contribute to worse living and health conditions and greater difficulties in access to healthcare services.
Objective: To explore high-risk behaviors and experiences with traffic law among night drivers in Curitiba, Brazil. Method: Data from 398 drivers on sociodemographic parameters, high-risk behaviors, experiences with traffic law, and traffic law violations were collected through interviews conducted at sobriety checkpoints. Exploratory-descriptive and analytical statistics were used. Results: The mean age of the participants was 32.6611.2 years (range, 18 to 75 years). Half of the drivers reported having driven after drinking in the last year, predominantly single men aged 18 to 29 years who drive cars and drink alcohol frequently. Only 55% of the drivers who had driven after drinking in the last year self-reported some concern about being detected in a police operation. Conclusions: A significant association was found between sociodemographic variables and behavior, which can help tailor public interventions to a specific group of drivers: young men who exhibit high-risk behaviors in traffic, such as driving after drinking alcohol, some of whom report heavy alcohol consumption. This group represents a challenge for educational and enforcement interventions, particularly because they admit to violating current laws and have a low perception of punishment due to the low risk of being detected by the police.
Objetivo: Descrever o perfil epidemiológico dos pacientes com câncer bucal tratados em um hospital de referência regional para tratamento de câncer no Paraná. Métodos: Este estudo retrospectivo, com delineamento transversal, envolveu uma população de 274 pessoas com câncer bucal, tratados no período de janeiro de 2007 a janeiro de 2014. Os dados foram coletados a partir dos prontuários hospitalares e complementados com as fichas de admissão hospitalar. Resultados: Identificou-se que 75,2% eram indivíduos do gênero masculino, e 81,5% de cor/raça branca. A média de idade foi 59 anos, variando de 19 a 88 anos, com maior acometimento para a faixa etária com mais 60 anos com 48,2% dos casos. As pessoas mais acometidas pela doença exerciam atividades relacionadas à agricultura (21,9%) e atividades relacionadas à produção de bens e serviços industriais (20,0%). O tipo histológico mais prevalente foi o carcinoma espinocelular em 90,2% casos, seguido pelo carcinoma basocelular (3,3%) e adenocarcinoma (2,9%). Conclusões: Os indivíduos mais acometidos foram homens, de raça branca, com idade média de 59 anos, com profissão ligada a agricultura, com lesões localizadas principalmente em lábio.
Objetivo: Estabelecer o perfil de mortalidade de centenários no município de Curitiba-PR. Métodos: Trata-se de um estudo epidemiológico descritivo, retrospectivo, a partir dos óbitos de idosos na faixa etária de 100 anos ou mais, residentes no município de Curitiba. Os dados secundários foram coletados no Sistema de Informação de Mortalidade (SIM). As variáveis consideradas foram idade, sexo, raça/cor, escolaridade, ocupação habitual, local de ocorrência do óbito, causas básicas do óbito (CID-10), de 2010 a 2019. A análise dos dados foi descritiva por meio de distribuição de frequências. Resultados: No banco de dados, constavam 498 óbitos de centenários e a idade variou de 100 a 115 anos. As principais causas de óbito foram doenças do aparelho circulatório (32,7%), do aparelho respiratório (21,3%), do sistema nervoso (12%), neoplasias (7,2%) e causas externas (6,8%). Para as mulheres, as principais causas foram doenças do aparelho circulatório (37%) e, para os homens, destacaram-se as doenças do aparelho respiratório (29,8%). Conclusão: O perfil de mortalidade entre os centenários de Curitiba identificou que predominaram as mulheres, donas de casa, de cor branca, com escolaridade entre 4 e 11 anos e com uma média de 101 anos, sendo doenças cardiovasculares a principal causa de morte.
Background Road accidents are an important cause of mortality in big cities in Brazil mainly in metropolitan areas like Curitiba. Aims/Objectives/Purpose This study aims to analyse epidemiological characteristics of victims and traffic accidents and identify space distribution of traffic accident's occurrences. Methods The analysis of traffic accident's space distribution (georeferencial) was accomplished to enlarge the knowledge of occurrence standard's and its distribution regularity related to each Regional Administrative Office, neighbourhood and streets of Curitiba. An index of accidents by population and an index of accidents by demographic density were generated to determine criteria risk of accidents in each neighbourhood and allowing comparability. To characterise victims and accidents, a descriptive and analytical epidemiological study was accomplished, based on an institutional secondary data base evaluation supplied by Epidemiology Centre of Curitiba's Municipal Health Office. Results/Outcomes The main findings reaffirm accident's predominance in the male young adult gender, but for each age group and each type of traffic user (pedestrian, cyclist, motorcyclist, driver, passenger) there are specificities which contribute to determine the risk of suffering a traffic accident and they need differentiated specific intervention strategies to become efficient. It's necessary to establish partnership with traffic related parts, registration standards, interaction among institutions which compose the system and to develop analyses and evaluation methodologies of generated data. Significance/Contribution to the Field This work supplies elements to the construction of a reliable information system which gives subsides to elaborate prevention and intervention actions, allowing better orientation of public policies of the sector.
