Thoracic spinal psammomatous meningioma is a rare subtype of meningioma. Among diverse types of mesenchymal differentiation, osseous metaplasia is found to be still rarer. We are presenting a new case of thoracic psammomatous spinal meningioma with osseous metaplasia in a middle aged female which that gives a sense of cancellous bone in the spinal canal. To conclude, meningiomas with osseous metaplasia are very rare tumors that complicate the surgical removal in certain cases. Ossification, if predicted prior to operation with computed tomography reconstruction, makes planning of removal easier. In our case, maintained cerebrospinal fluid spaces despite hard consistency of tumor made its removal easier once cerebrospinal fluid was drained. We have submitted this article because it is very rare and curable pathology and preoperative diagnosis helps in prevention of neurological injury during its excision.
Cystic hygromas are uncommon congenital lymphatic malformations that commonly involve the lateral aspect of the neck. The treatment of choice of these swellings is surgical excision but at times, it becomes quite difficult to perform a complete removal because of the tendency of the tumor to infiltrate the surrounding structures. We encountered a similar case of cystic hygroma that was infiltrating the carotid sheath. We were able to achieve complete surgical excision without damaging the surrounding structures.
Background: Lower extremity reconstruction is an essential part of plastic surgery and focuses on the treatment of wounds. The restoration of an intact covering is the primary surgical requisite following trauma of the lower extremity because deep healing can be no better than the surface covering. Present study will review the field of lower extremity reconstruction focusing on the micro-vascular tissue transfer for severe limb traumatic injuries.Methods: Patients were fully evaluated and defect was assessed depending on the defect requirement free micro- vascular flap was planned. 20 micro vascular flaps in 18 males and 2 females were used for the wound / defect of lower limbs following trauma.Results: There were 18 men and 2 women with a mean age of 43.6 years (range, 18-62 years) in the present study. The overall survival of the flaps was 95% (19/20).Conclusions: The free flap is an effective method of lower-extremity reconstruction. Good outcomes can be achieved with complete debridement and the selection of appropriate recipient vessels and flaps according to the recipient site.
Background: Over 1 lakh people are affected by burn every year in India and 20 thousands of them die per year. Post burn contracture is a common sequele occurring after burn. Upper limb contractures are also occurring more commonly because it is most mobile part of body and likely to be involved in burn. There are many studies on management of post burn contractures but literature about prevention of contracture is little, hence this study was conducted. The aim of this study was to recognise various preventive measures to prevent post burn contractures of upper extremity.Methods: This study was conducted in NSCBM Subharti Medical College and Hospital located in Meerut (North India) from October 2012 to October 2014 in Department of Surgery. It was a prospective observational study consisted of 80 cases who presented as acute burn of upper limb admitted in the hospital.Results: In our study early excision with skin grafting was done in 20 patients (25%) while delayed skin grafting was done in 25 patients (31%) while 35 patients (44%) were managed conservatively. In our study 20 patients reported back with a post burn contracture. The reason found was non-compliance to antideformity splint and physiotherapy.Conclusions: Early surgical management of deep burns, physiotherapy, anti-deformity position and proper splintage can significantly reduce the development of post burn contracture.
