Objective: Patent ductus arteriosus is associated with multiple complications and high neonatal mortality. This study aimed to evaluate the efficacy and benefits of paracetamol compared to ibuprofen and indomethacin, non-steroidal anti-inflammatory drugs usually used to close the ductus arteriosus in premature newborns. Data source: We searched the PubMed, SciELO, Lilacs, Bireme, ClinicalTrials.gov and Embase databases for articles published between January 2005 and December 2020. The descriptors used were: premature newborn, patent ductus arteriosus, pharmacotherapy, closure and paracetamol / acetaminophen. The quality of the articles was evaluated by the classification of the Oxford Centre for Evidence based Medicine. Data synthesis: The efficacy of paracetamol is comparable to that of ibuprofen and indomethacin, and it is associated with lower rates of gastrointestinal and intracerebral hemorrhage, spontaneous intestinal perforation, thrombocytopenia, hepatic and renal intolerance, and lower serum values of urea, creatinine, and bilirubin. Furthermore, it is more advantageous than placebo or nonintervention and may be the best option in cases with contraindications for other drugs. Gestational age and birth weight were factors that influenced the efficacy of paracetamol and showed reduced efficacy in extremely premature newborns with extremely low and very low birth weights. Conclusions: Other clinical and prospective studies are needed to support the indication of paracetamol in patent ductus arteriosus. However, this drug proves to be a promising pharmacological alternative, having similar efficacy to classic non-steroidal anti-inflammatory drugs, with fewer adverse effects.
Background: ST-segment elevation acute myocardial infarction (STEMI) is a pathological process that involves cardiac muscle tissue death. Intravenous thrombolysis with fibrinolytics or primary percutaneous coronary intervention (PCI), an invasive technique, can be performed for tissue revascularization. PCI has been preferred as compared to non-invasive methods, although few studies have described its use in Brazil. Objectives:The aim of the present study was to analyze data on the use of primary PCI and investigate the relevance of hospitalizations for the treatment of STEMI in the country.Methods: A descriptive, cross-sectional analysis of data from the Brazilian Unified Health system (SUS) Department of Informatics (DATASUS) from 2010 to 2019 was conducted.Results: Hospitalizations for STEMI represented 0.6% of all hospital admissions in Brazil in the analyzed period, 0.9% of hospital costs, and 2.1% of deaths. The number of hospitalizations due to STEMI was 659,811, and 82,793 for PCIs. Length of hospital stay was 36.0% shorter and mortality rate was 53.3% lower in PCI. The mean cost of PCI was 3.5-fold higher than for treatment of STEMI.Conclusions: Data on hospitalizations for STEMI treatment in Brazil revealed high hospitalization and mortality rates, elevated costs, and long hospital stay. Although primary PCI is a more expensive and less used technique than other methods, it can reduce the length of hospital stay and mortality in the treatment of STEMI.

Introdução: Em caso de Infarto Agudo do Miocárdio (IAM), o tempo entre o início dos sintomas e a chegada ao serviço de saúde é um relevante preditor de mortalidade. Essa assertiva é agravada pela apresentação tardia (AT) dos pacientes, visto que o benefício da terapia de reperfusão miocárdica é tempo-dependente. Objetivos: O objetivo deste estudo consiste em investigar e analisar os preditores de AT em pacientes com IAM, bem como os fatores associados a estes. Metodologia: Este estudo constitui uma revisão sistemática entre os anos 2000 e 2020, através das bases de dados - LILACS, MEDLINE, PubMed e SciELO, com os seguintes descritores usados concomitantemente: Myocardial Infarction; Time Factors; First Aid; Health Services. Além disso, foram selecionadas fontes secundárias, totalizando 16 artigos. Discussão: Sexo feminino, não brancos, idade avançada, baixa escolaridade, baixo nível socioeconômico, ausência de seguro de saúde privado, ser solteiro ou viúvo e estar desacompanhado durante a deflagração do IAM foram considerados preditores de AT. Somado a isso, em relação ao histórico de saúde, apresentaram-se tardiamente pacientes com Diabetes Mellitus, IAM sem supradesnivelamento do segmento ST ou manifestação clínica intermitente. Além disso, o desconhecimento sobre sinais e sintomas de IAM foi um preditor de AT, assim como determinados comportamentos praticados com o início do quadro, tais como esperar ou orar pelo desaparecimento dos sintomas e negar o ataque cardíaco. Por outro lado, presença de IAM com supradesnivelamento do segmento ST, doença cardiovascular prévia, insuficiência renal crônica e, principalmente, dor torácica foram fatores protetores. Condições estruturais e demográficas apontadas foram pacientes provenientes de áreas suburbanas, conduzidos ao serviço de saúde por meios próprios (desconsiderando a possibilidade de tratamento pré-hospitalar em ambulância), submetidos a triagem ineficiente, atendidos por não-cardiologistas ou aqueles transferidos para hospital especializado em caso de necessidade. Considerações finais: Diante dos preditores de AT identificados, reforça-se o impacto da apresentação precoce do paciente ao serviço de saúde e evidencia-se a importância da melhoria estrutural da rede hospitalar e da conscientização sobre IAM, sobretudo, à população de alto risco para AT. Essas medidas possuem grande potencial para evitar atrasos e modificar a realidade das altas taxas de morbimortalidade das doenças cardiovasculares.
Dentre as maiores causas de óbito no Brasil, figuram as que possuem como desfecho final a parada cardiorrespiratória, a qual ainda representa e permanece como um dos maiores problemas de saúde em nível mundial. Estimativas apontam que ocorram cerca de 200 mil paradas cardiorrespiratórias anualmente no Brasil, sendo que metade ocorre em ambiente extra-hospitalar, dependendo, assim, da ação de cidadãos leigos. Este artigo tem como objetivo relatar as experiências com o Projeto de Extensão "Ensinando a Salvar Vidas", realizado para capacitar e ampliar os conhecimentos da população a respeito das práticas de ressuscitação cardiopulmonar, por meio de intervenções teórico-práticas nas escolas de São João del-Rei. Entretanto, devido à pandemia do vírus SARS-CoV-2, o projeto sofreu alterações nos seus planos e metodologias iniciais, sem a possibilidade dos encontros presenciais. Foram adotadas estratégias de ensino a distância, com o intuito de se manterem os objetivos do projeto, entendendo-se a importância das intervenções.
