To investigate the adjunctive role of some peripheral blood parameters to serum prostate specific antigen (PSA) in predicting the patients with prostate cancer (PCa) prior to prostate biopsy, and clinically significant PCa among those. Material and Methods: This prospective study included men aged ≥45 years, who were scheduled to undergo a prostate biopsy due to gray-zone PSA levels. The levels of free and total PSA (fPSA and tPSA), total testosterone (TT), PSA density (PSAD), C-reactive protein (CRP), De Ritis ratio (aspartate aminotransferase/alanine transaminase) and hemograms were recorded. Patients were divided into 2 groups as benign prostatic hyperplasia (Group 1) and PCa (Group 2) groups. The PCa group was further divided into 2 subgroups as clinically significant and clinically insignificant PCa. The pre-biopsy values of the variables were compared between the groups. Results: A total of 210 patients were included in the study (Group 1, n=105; Group 2, n=105). The mean age was 65.1±7.4 years, the mean tPSA level was 6.14±2.05 ng/mL, and the mean TT level was 15.46±5.47 nmol/L. Age, prostate volume, the values of fPSA, PSAD, CRP, and the ratios of fPSA/tPSA, CRP/albumin and De Ritis were statistically different between the groups. In the PCa group, only the tPSA level was significantly different between the subgroups (p=0.005). The area under curve and the cut-off value for tPSA to predict clinically significant PCa were 0.669 and 5.8 ng/mL, respectively. Conclusion: CRP, the CRP/albumin ratio and De Ritis ratio may further help predict a diagnosis of PCa.
ÖZET Küçük hücreli prostat karsinomu (KHPK) oldukça nadir görülmekte olup, tüm prostat kanserlerinin yaklaşık %1'ini oluşturur. Agresif seyirli olup tanı anında hastaların yaklaşık yarısı metastatik evrededir. KHPK, klasik prostat adenokarsinomuyla kıyaslandığında akciğer ve karaciğer başta olmak üzere hızla gelişen viseral metastazlar, osteolitik kemik metastazları, beyin tutulumu ve paraneoplastik sendromlar gibi belirgin olarak farklı klinik özelliklere sahiptir. Tümör yüküyle orantısız olarak serum prostat spesifik antijen (PSA) düzeyi düşük seviyededir. CD56, sinaptofizin, kromogranin A gibi biyobelirteçler ayırıcı tanıda önemli yere sahiptir. Tedavinin temelini kemoterapi ve radyoterapi oluşturur. It has an aggressive course and about half of the patients are in the metastatic stage at the time of diagnosis. SCCP has distinctly different clinical features such as rapidly developing visceral metastases, especially lung and liver, brain and osteolytic bone metastases, paraneoplastic syndromes compared to classical adenocarcinoma of the prostate. It has low serum prostate specific antigen (PSA) level disproportionately to the tumor burden. Biomarkers such as CD56, synaptophysin and chromogranin A play an important role for the differential diagnosis. The mainstay of treatment is chemotherapy and radiotherapy. In this paper, we aimed to present a case of SCCP detected in a patient with normal serum PSA level, who underwent surgical treatment for lower urinary tract symptoms, in the light of current literature.
Kemoterapi sonrası retroperitoneal lenf nodu diseksiyonu (RPLND) uygulanan metastatik seminom dışı germ hücreli testis tümörü (NSGCT) hastalarında, preoperatif inflamasyon belirteçleri (nötrofil/lenfosit oranı (NLR), platelet/lenfosit oranı (PLR)) ve görüntüleme bulgularının RPLND patolojisi ile ilişkili olup olmadığını araştırmaktır. Gereç ve Yöntemler: Kliniğimizde Ocak 2016-Şubat 2022 tarihleri arasında RPLND yapılmış hastalar geriye dönük tarandı. Preoperatif hematolojik ve biyokimyasal parametreler, bilgisayarlı tomografi (BT) bulguları, orşiektomi ve RPLND histopatoloji bulguları kaydedildi. Hastalar patolojisi canlı tümör/teratom olanlar Grup 1 ve benign (nekroz-fibroz) olanlar Grup 2 şeklinde ayrıldı. Gruplar arasında NLR, PLR, De Ritis oranı, serum tümör belirteçleri ve abdominopelvik BT'de saptanan ek büyük boyuttaki retroperitoneal lenf nodunun uzunluğu, hem orşiektomi öncesi, hem kemoterapi öncesi, hem de RPLND öncesi dönemlerde karşılaştırıldı. Bulgular: Top-lamda13 hasta çalışmaya dahil edildi (Grup 1: 10 hasta, Grup 2: 3 hasta). Gruplar arasında yaş ortalaması benzerken, orşiektomi öncesi hematolojik ve serum belirteçleri açısından fark saptanmadı (p>0,05). Kemoterapi öncesi hematolojik ve serum belirteçleri 2 grupta da benzerken, abdominopelvik BT'deki en büyük lenfadenopati (LAP) boyutları Grup 1'de anlamlı düzeyde yüksekti (p=0,03). Benzer şekilde RPLND öncesi hematolojik ve serum belirteçleri 2 grupta da benzerken, abdominopelvik BT'deki en büyük LAP boyutları Grup 1'de anlamlı düzeyde yüksekti (p=0.03). Hidronefroz, yüzeyel ve kas invaziv mesane kanseri (MK) hastalarında sırasıyla %7,5 ve %54'e varan oranlarda görülebilmektedir. MK hastalarında tıkalı bir böbreğin double J stentleme (DJS) yöntemiyle drenajının sağlanması, olası metakron üst üriner sistem tümörü (ÜÜST) gelişimi korkusu nedeniyle tartışmalıdır. MK hastalarında, transüretral rezeksiyon sırasında perkütan nefrostomi ya da DJS takma yöntemlerini, metakron ÜÜST gelişimi açısından karşılaştırmayı amaçladık. Gereç ve Yöntemler: Ocak 2016-Aralık 2021 tarihleri arasında eş zamanlı olarak mesane kanserinin transüretral rezeksiyonu ve retrograd/antegrad DJS veya perkütan nefrostomi yerleştirilmesi ile opere edilen MK hastalarının verileri geriye dönük olarak analiz edildi. Hastaların takiplerinde, metakron ÜÜST oluşumu açısından, tümör gelişen ve gelişmeyen olarak 2 gruba ayrıldı. Hastaların yaş, cinsiyet gibi demografik özellikleri ile tedavi öncesi hematolojik parametreleri ve serum biyokimyasal parametre seviyeleri, ameliyat öncesi görüntülemede hidronefroz derecesi, drenaj yöntemi ile ameliyat sonrası patoloji sonuçları kaydedildi ve gruplar arasında karşılaştırıldı. Bulgular: Çalışmaya dâhil edilen 133 hastanın 8'inde (%6) metakron ÜÜST gelişti [perkütan nefrostomi (n=2), DJS (n=6)]. Nefrostomi takılan hastalar ile DJS takılan hastalar arasında metakron ÜÜST gelişim oranı istatistiksel olarak farklı saptanmadı (p=0,27). MK'nin yerleşimi, patolojisi, sayısı ve hidronefroz dereceleri her iki grup arasında benzerdi (p>0,05). Ameliyat öncesi nötrofil/lenfosit oranı, platelet/lenfosit oranı ve De Ritis oranında her iki grup arasında fark saptanmadı (p>0,05). Drainage of an obstructed kidney with double J stenting (DJS) method in BC patients is controversial due to the fear of possible metachronous upper urinary tract urothelial cancer (UUTUC) development. We aimed to compare the methods of percutaneous nephrostomy or DJS insertion during transurethral resection in BC patients in terms of metachronous UUTUC development. Material and Methods: Data of BC patients operated by concomitant transurethral resection of bladder tumor and retrograde/antegrade DJS or percutaneous nephrostomy placement between January 2016 and December 2021 were analyzed retrospectively. The patients were divided into 2 groups as UUTUC and non-UUTUC.
