Introduction: This study investigated the influence of Erbium-Doped Yttrium Aluminum Garnet (Er:YAG) laser on microhardness, chemical composition and subsurface morphology of dentin cavity walls. Methods: Forty sound human premolars were selected and randomly assigned into four groups. Class V cavities were prepared either with an Er:YAG laser (groups 1 and 2; 15 Hz, 250 mJ for enamel, 10 Hz, 200 mJ for dentin) or with a high speed handpiece (groups 3 and 4). The specimens in groups 1 and 3 served as the control, whereas those in groups 2 and 4 were exposed to subablative laser irradiation following cavity preparation (10 Hz, 50 mJ). After bisecting the specimens, one half was subjected to microhardness assessment and the other half was evaluated by SEM-EDS analysis. Results: Microhardness was significantly greater in the specimens prepared by both ablative and subablative laser irradiation (group 2) than that of the bur-prepared cavities (groups 3 and 4) (P < 0.05). The quantity of calcium ion was significantly greater in cavities prepared by the Er:YAG laser (groups 1 and 2) compared to that of the bur cavities (groups 3 and 4) (P < 0.05). Subablative irradiation improved microhardness and weight percentage of calcium ion in both laser and bur cavities, but the difference was not significant compared to that of the relevant control group (P > 0.05). Conclusion: Cavity preparation with an Er:YAG laser could be considered as an alternative to the conventional method of drilling, as it enhances the mechanical and compositional properties of lased dentin, especially when combined by subablative irradiation.
Sinus tract (or fistula) is a common manifestation of pulpal necrosis with periapical pathosis that requires conventional or rarely surgical- endodontic treatment in order to heal. It is mainly identified intraorally and in rare cases manifestation is extra orally, which may frequently be misdiagnosed with cutaneous lesion and incorrectly treated. New technologies such as lasers have been developed as adjuncts to standard endodontic antimicrobial procedures in order to increase the success rate of endodontic therapy while complying with the desire of saving a natural tooth. Herein, we present a clinical case of extra oral sinus tract with periodic pus drainage that was successfully treated by combining conventional endodontic therapy, intra canal photodynamic therapy (PDT), extra oral low level laser therapy (LLLT) and laser treatment for extra oral fistula. Treatment of the odontogenic lesion among with the extra oral lesion was an ingenious and successful technique which was achieved via laser technology and winded up to patient's consent and satisfactory results.
Objectives: This study aimed to assess the effect of DentalVibe on the level of pain experienced during anesthetic injections using 2 different techniques. Materials and Methods: This randomized crossover clinical trial evaluated 60 patients who required 2-session endodontic treatment. Labial infiltration (LI) anesthesia was administered in the anterior maxilla of 30 patients, while inferior alveolar nerve block (IANB) was performed in the remaining 30 patients. 1.8 mL of 2% lidocaine was injected at a rate of 1 mL/min using a 27-gauge needle. DentalVibe was randomly assigned to either the first or second injection session. A visual analog scale was used to determine participants' pain level during needle insertion and the anesthetic injection. The paired t-test was applied to assess the efficacy of DentalVibe for pain reduction. Results: In LI anesthesia, the pain level was 12.0 ± 15.5 and 38.1 ± 21.0 during needle insertion and 19.1 ± 16.1 and 48.9 ± 24.6 during the anesthetic injection using DentalVibe and the conventional method, respectively. In IANB, the pain level was 14.1 ± 15.9 and 35.1 ± 20.8 during needle insertion and 17.3 ± 14.2 and 39.5 ± 20.8 during the anesthetic injection using DentalVibe and the conventional method, respectively. DentalVibe significantly decreased the level of pain experienced during needle insertion and the anesthetic injection in anterior LI and mandibular IANB anesthesia. Conclusions: The results suggest that DentalVibe can be used to reduce the level of pain experienced by adult patients during needle insertion and anesthetic injection.
