A total of 363 patients underwent 48-hour Bravo testing and of those patients, 205 were eligible for the study. Ninety-two patients reported symptoms as being «same/better» and 113 as being «worse» after stopping PPIs. Of the 92 patients with improved symptoms, 44 (48%) had documented acid reflux during the Bravo study, compared with 65 of 113 (58%) patients with worsening symptoms that also complained of acid reflux. Of the 109 patients found to have confirmed GERD upon pH monitoring, 65 (59.6%) reported a worsening of symptoms, compared with 48 of 96 (50.0%) patients without GERD (p=0.043). Main symptoms stated to be worse included heartburn, chest pain, regurgitation, nausea, and belching (p<0.05). Of the 205 patients, 103 were off PPIs for 7 days. Seventy-two of them (68.9%) reported a worsening of symptoms, compared with 40 of the 102 (41.2%) patients that were off PPIs for>7 days (p=0.042) CONCLUSION: Symptom exacerbation following PPI cessation for at least 7 days correlated with acid reflux severity assessed by Bravo testing. Patients off PPIs for 7 days had a higher likelihood of experiencing worsening symptoms, compared with those off PPIs for more than 7 days. These findings suggest that when PPIs are held for 7 days or less prior to Bravo testing, acute worsening of upper GI symptoms due to the abrupt discontinuation of therapy may influence the Bravo results. The etiology of this may be related to rebound acid hypersecretion and needs to be further elucidated in future studies.
Background: Spinal cord stimulation can be an effective treatment modality in patients suffering from postherpetic
neuralgia who have failed first-line pharmacotherapy and continue to struggle from debilitating pain.
Appropriate patient selection and having a wide array of stimulation waveforms can enhance the success
of spinal cord stimulator trials.
Case Report: In this article, we present a case report of a patient suffering from refractory thoracic postherpetic neuralgia
who underwent a successful high-frequency 10-kHz spinal cord stimulator trial. Lead tips were
successfully placed at the midline and left paramedial side of the top of T1 vertebral bodies at the source
of the pain. The patient was followed up in our clinic 7 days post procedure. At the time of follow-up,
our patient reported an 85% to 90% reduction in his pain symptom scores and a significant improvement
in his quality of life.
Conclusion: Similar successful trials for postherpetic neuralgia have been reported in small studies using traditional
low-frequency stimulation waveforms. However, to our knowledge, this is the first report of a successful
spinal cord stimulator trial using high-density 10 kHz.

Мета статті – визначити роль композиторки Франчески Каччіні в італійській музичній культурі епохи Бароко, виявити вокально-стилістичну специфіку першої жіночої опери Ф. Каччіні «Звільнення Руджеро з острова Альчина».
Результати дослідження. Проаналізовано роботи провідних західних вчених щодо музичної культури Італії часів Бароко. Обґрунтовано, що виникнення жіночих вокальних ансамблів змінило музичний ландшафт Італії та вплинуло на жанрову стилістику, вокально-виконавські традиції та музично-виразні прийоми. Визначено, що вклад композиторки Ф. Каччіні у вокально- оперне мистецтво є незаперечним. Доведено, що опера «Звільнення Руджеро з острова Альчина» зіставляється з протосалонними музично-театралізованими виставами Європи та Британії.
Наукова новизна – зроблено спробу визначити ступінь впливу культових жіночих ансамблів на статус жінок у музичній культурі Італії кінця XVI – початку XVIІ століття, а також провести паралель між першою «жіночою» італійською оперою епохи Бароко та протосалонними жанрами Європи та Британії (англійська придворна маска, французький придворний балет).
Висновки. Доведено, що в XVI столітті гендерні норми в суспільстві давали змогу виконувати музику жінкам тільки в монастирях. Зазначено, що діяльність ансамблю «Concerto Delle Donne» стає поворотним моментом в італійській музичній культурі, який можна прирівняти до гендерно-музичної революції. Акцентовано на першій «жіночій» опері Ф. Каччіні «Звільнення Руджеро з острова Альчина» як рідкісному зразку ранньої барокової опери та балету. Схарактеризовано жанрову специфіку та стилістичні особливості опери. Визначено паралель між оперою Ф. Каччіні, протосалонними жанрами Європи й Британії та «інтелектуальним театром» придворних зібрань у Візантії. Позначено масштаб культурного впливу творчості Ф. Каччіні на гендерний фактор у музичній культурі Італії.
