BackgroundSelf-management support can improve health and reduce health care utilization by people with long-term conditions. Online communities for people with long-term conditions have the potential to influence health, usage of health care resources, and facilitate illness self-management. Only recently, however, has evidence been reported on how such communities function and evolve, and how they support self-management of long-term conditions in practice.ObjectiveThe aim of this study is to gain a better understanding of the mechanisms underlying online self-management support systems by analyzing the structure and dynamics of the networks connecting users who write posts over time.MethodsWe conducted a longitudinal network analysis of anonymized data from 2 patients’ online communities from the United Kingdom: the Asthma UK and the British Lung Foundation (BLF) communities in 2006-2016 and 2012-2016, respectively.ResultsThe number of users and activity grew steadily over time, reaching 3345 users and 32,780 posts in the Asthma UK community, and 19,837 users and 875,151 posts in the BLF community. People who wrote posts in the Asthma UK forum tended to write at an interval of 1-20 days and six months, while those in the BLF community wrote at an interval of two days. In both communities, most pairs of users could reach one another either directly or indirectly through other users. Those who wrote a disproportionally large number of posts (the superusers) represented 1% of the overall population of both Asthma UK and BLF communities and accounted for 32% and 49% of the posts, respectively. Sensitivity analysis showed that the removal of superusers would cause the communities to collapse. Thus, interactions were held together by very few superusers, who posted frequently and regularly, 65% of them at least every 1.7 days in the BLF community and 70% every 3.1 days in the Asthma UK community. Their posting activity indirectly facilitated tie formation between other users. Superusers were a constantly available resource, with a mean of 80 and 20 superusers active at any one time in the BLF and Asthma UK communities, respectively. Over time, the more active users became, the more likely they were to reply to other users’ posts rather than to write new ones, shifting from a help-seeking to a help-giving role. This might suggest that superusers were more likely to provide than to seek advice.ConclusionsIn this study, we uncover key structural properties related to the way users interact and sustain online health communities. Superusers’ engagement plays a fundamental sustaining role and deserves research attention. Further studies are needed to explore network determinants of the effectiveness of online engagement concerning health-related outcomes. In resource-constrained health care systems, scaling up online communities may offer a potentially accessible, wide-reaching and cost-effective intervention facilitating greater levels of self-management.
The Decentralised Web (DW) has recently seen a renewed momentum, with a number of DW platforms like Mastodon, Peer-Tube, and Hubzilla gaining increasing traction. These offer alternatives to traditional social networks like Twitter, YouTube, and Facebook, by enabling the operation of web infrastructure and services without centralised ownership or control. Although their services differ greatly, modern DW platforms mostly rely on two key innovations: first, their open source software allows anybody to setup independent servers ("instances") that people can sign-up to and use within a local community; and second, they build on top of federation protocols so that instances can mesh together, in a peer-to-peer fashion, to offer a globally integrated platform.In this paper, we present a measurement-driven exploration of these two innovations, using a popular DW microblogging platform (Mastodon) as a case study. We focus on identifying key challenges that might disrupt continuing efforts to decentralise the web, and empirically highlight a number of properties that are creating natural pressures towards recentralisation. Finally, our measurements shed light on the behaviour of both administrators (i.e., people setting up instances) and regular users who sign-up to the platforms, also discussing a few techniques that may address some of the issues observed.
This paper aims to shed light on alternative news media ecosystems that are believed to have influenced opinions and beliefs by false and/or biased news reporting during the 2016 US Presidential Elections. We examine a large, professionally curated list of 668 hyper-partisan websites and their corresponding Facebook pages, and identify key characteristics that mediate the traffic flow within this ecosystem. We uncover a pattern of new websites being established in the run up to the elections, and abandoned after. Such websites form an ecosystem, creating links from one website to another, and by 'liking' each others' Facebook pages. These practices are highly effective in directing user traffic internally within the ecosystem in a highly partisan manner, with right-leaning sites linking to and liking other right-leaning sites and similarly left-leaning sites linking to other sites on the left, thus forming a filter bubble amongst news producers similar to the filter bubble which has been widely observed among consumers of partisan news. Whereas there is activity along both left-and right-leaning sites, right-leaning sites are more evolved, accounting for a disproportionate number of abandoned websites and partisan internal links. We also examine demographic characteristics of consumers of hyper-partisan news and find that some of the more populous demographic groups in the US tend to be consumers of more right-leaning sites.
Disruptions resulting from an epidemic might often appear to amount to chaos but, in reality, can be understood in a systematic way through the lens of “epidemic psychology”. According to Philip Strong, the founder of the sociological study of epidemic infectious diseases, not only is an epidemic biological; there is also the potential for three psycho-social epidemics: of fear, moralization, and action. This work empirically tests Strong’s model at scale by studying the use of language of 122M tweets related to the COVID-19 pandemic posted in the U.S. during the whole year of 2020. On Twitter, we identified three distinct phases. Each of them is characterized by different regimes of the three psycho-social epidemics. In the refusal phase, users refused to accept reality despite the increasing number of deaths in other countries. In the anger phase (started after the announcement of the first death in the country), users’ fear translated into anger about the looming feeling that things were about to change. Finally, in the acceptance phase, which began after the authorities imposed physical-distancing measures, users settled into a “new normal” for their daily activities. Overall, refusal of accepting reality gradually died off as the year went on, while acceptance increasingly took hold. During 2020, as cases surged in waves, so did anger, re-emerging cyclically at each wave. Our real-time operationalization of Strong’s model is designed in a way that makes it possible to embed epidemic psychology into real-time models (e.g., epidemiological and mobility models).
