Background: Obstructive ileus and peritonitis are the most frequent complication of colorectal cancer, manifested in 8-40% cases. Anastomotic leakage (AL) is a fatal complication in colorectal surgery. In our study, we aim to analyse the safety, outcomes in patients with colon obstruction after resection with new anastomosis technique implementation and to identify prognostic value of each risk factor on the development of AL. Methods: A retrospective analysis of prospectively acquired data of 155 consecutive emergency patients with cancer of the proximal rectum and sigmoid colon complicated by chronic bowel obstruction in the stage of subcompensation and decompensation were included in our study and divided into two groups. In the basic group (n=95), primary nonfunctional anastomosis (PNA) was performed after resection of the rectum. In the comparing group (n=60), resection of the colon or rectum with primary anastomosis was performed. Results: Postoperative complications have arisen in 5(5,3%) patients of group I and 11 (18,4%) patients of group II. AL was developed only in 1 patient in the basic group (1,1%) and in 7 (11,7%) patients of the compared group on the 5 th -12 th day after the operation. We have worked out the rating score of risk factors of AL.A score was given for each AL factor which we had selected for the elaboration of this scale. Conclusions: Based on AL risk factors scale we highlighted five levels of risk which have significant clinical meaning and help surgeons decide finally how to finish operation in a patient with bowel obstruction to avoid AL. PNA with preventive transversostoma was shown to provide adequate decompression of the colon, reliable prevention of colorectal AL and avoid complex reconstructive operations.KEYWORDS: colorectal cancer, bowel obstruction, anastomotic leakage.

В економічно розвинутих країнах світу система охорони здоров'я функціонує в умовах жорстких правил страхової медицини. Як правило, при наданні тих чи інших медичних послуг враховують також і економічну доцільність необхідних операційних втручань чи будь-якої іншої лікарської маніпуляції, вираховують можливі втрати як для пацієнта, так і для дер-жави. Мета дослідження -вивчити та економічно обґрунтувати додаткові витрати держави та особистих витрат пацієнтів, котрим була виконана операція Гартмана, та провести порівняння даних показників із економічними втратами, які несуть пацієнти, прооперовані за методикою, розробленою в нашій клініці. Матеріали і методи. За даними Міністерства охорони здоров'я України, в 2015 р. чисельність інвалідів ІІ групи (захворюван-ня органів травлення), яких вперше визнали, становила 3423 особи, в тому числі 3219 осіб у працездатному віці або 94,0 % [2]. Доходи населення -важливий результативний показник, що характеризує загальну суму грошових і натуральних (у гро-шовій оцінці) доходів громадян протягом року. За даними Державної служби статистики України, в 2015 р. загальний серед-ньомісячний дохід у розрахунку на одного працюючого становив 10 362 грн. Результати досліджень та їх обговорення. За умов, якщо інвалід ІІ групи офіційно не працював, тобто не отримував зар-плати, а лише отримував соціальну допомогу та інші соціальні трансферти, то у цій ситуації втрати з державного бюджету в 2015 р. для таких осіб становили 5958,3 грн. щомісячно, або на 11,7 % більше, ніж для працюючих інвалідів. Крім цього, інва-ліди ІІ групи понесли особисті додаткові витрати, пов'язані з придбанням калоприймачів та засобів догляду за стомою. Для забезпечення одного пацієнта засобами догляду за стомою становить 10 300 грн/1 рік. Висновки. Пацієнти, котрим виконано операцію Гартмана, потребують тривалої соціальної адаптації та психологічної допо-моги для пристосування до нових умов існування, необхідні додаткові витрати коштів як з державного бюджету, так і особис-тих заощаджень, збільшення обсягів соціальної допомоги від держави та ін.Ключові слова: економічна обґрунтованість; операція Гартмана; стома; національний дохід.
Background. Reversal of Hartmann’s procedure (HP) is one of the most complex surgical interventions in abdominal surgery with high morbidity rates of up to 58% in the patients after HP and mortality of up to 3.6%. Objective. This was a retrospective observational study to analyze the Hartmann’s reversal (HR) outcomes. Methods. The study involved 31 patients (16 males and 15 females), average age 59.6±10.31 (range 26-80), who underwent HR at Ternopil Regional Hospital in 2010 - 2021. The reasons for the HP were: cancer in 20 (64.5%) cases, perforation of the diverticulum – 6 (19.3%), traumatic rupture – 3 (9.7%) and Crohn’s disease - 2 (6.4%) cases. Results. Median time from Hartmann’s procedure to reversal was 11.13±9.24 months (interval 3-38). Intraoperative time was 210.33±56.91 minutes (range 120-330). HR was performed in 30 (96.8 %) patients. Dense pelvic adhesions of the stump of the rectum was diagnosed in 13 (41.9%) patients. Hand-sewn anastomosis was performed for 22 (71%) patients, stapler anastomosis – 4 (12.9%), pull-through technique – 3 (9.7%) patients. AL occurred in 3 (9.7%) patients on the 15th, 23rd and 35th postoperative days. pseudomembranous colitis was diagnosed in 2 (6.4%) patients with AL. The mortality rate was 1 (3.3%) as a result of septic complications due to AL; this case was not operated by a colorectal surgeon due to administrative issue in the hospital. Conclusions. Hartmann’s reversal is still one of the most difficult operations in colorectal surgery with high incidence of postoperative complications. Sound selection of patients with low comorbidity and in suitable time period is crucial for successful HR.
