Аннотация. Целью работы явилась разработка антирубцового крема с верапамилом и морфофункциональное обоснование его фармакологических эффектов в коррекции патологических рубцов кожи. Впервые сконструирована мягкая лекарственная форма для наружного применения-антирубцовый крем, содержащий в качестве базового компонента верапамил, пенетратора-димексид, в качестве основы-вода-вазелин-ланолин в определенном количественном соотношении компонентов крема. На основании результатов технологических и биофармацевтических исследований с использованием диализа через полупроницаемые мембраны и метода обращенно-фазовой высокоэффективной жидкостной хроматографии высокого давления был осуществлен выбор наиболее оптимальной концентрации верапамила. Были исследованы различные составы основ: вода-вазелин-эмульгатор Т2; вода-вазелин-ланолин; стеарин-вазелиновое масло-карбонат натрия-карбонат калия-вода; воск-спермацет-ланолин-персиковое масло-вода. Установлено, что основа вода-вазелин-ланолин в соотношении 3:6:1 обеспечивает самую оптимальную скорость и полноту высвобождения входящих в ее состав лекарственных веществ. Обоснована эффективность использования оригинального антирубцового крема в коррекции патологических рубцов кожи. Экспериментальную терапию проводили антирубцовым кремом с верапамилом, в группе сравнения применяли ланолин-вазелиновую основу. Исследовали соотношение патологических и нормотрофических рубцов, сравнивали особенности строения рубцовой ткани в зависимости от способа лечения и длительности применения лекарственных препаратов. Сделано заключение, что под действием верапамила происходит целенаправленная трансформация гипертрофических патологических рубцов в нормотрофические. Патогенетически обоснованный состав разработанного антирубцового крема на основе верапамила позволяет использовать его на всех этапах медицинской реабилитации больных, перенесших ожоговую травму для профилактики и лечения гипертрофических и келоидных рубцов.
The data in respect to efficacy of verapamil-containing scar cream for treatment of hypertrophic and keloid scars are presented in the article. Skin scars were simulated in rats by excision of a full-thickness skin flap size 5x4 cm2 in the center of the back. Upon wound healing and scar formation a daily twofold fomentation of verapamil-containing cream was carried out. Treatment efficacy was checked after 10, 30 and 60 days. The following indices were studied: an area of the scar, its type, consistence, color, sensitivity, and scar microcirculation according to biomicroscopy. The proposed novel method for treatment of hypertrophic and keloid scars with verapamil-containing cream allowed to obtain a sustained clinical result demonstrating a reducing area of the scar by 25%, a physiological scar formation, improving the consistence and scar colour changing to the normal skin colour, its softening and flattening, and pain disappearance. Due to the dermal application, the method was painless, safety and did not affect on the surrounding tissues. According to biomicroscopy a scar remodeling approximated the indices to the level of healthy skin in 93% of rats. The most significant clinical effect the cream with verapamil was observed in experimental animals in 60 days after treatment.

Верапамил и дилтиазем снижают чрезмерную активность фибробластов на стадии формирования спаек путем регуляторного воздействия на их пролиферативную и коллагенсинтетическую функции, уменьшая число митозов на 58,5 %, количество Ki-67 + клеток на 59,2 %, избыточную продукцию оксипролина на 66,7 %. Нормализующий эффект верапамила на фибробласты в 1,8 раза выше, чем у дилтиазема. Нифедипин антипролиферативное действие на клетки не оказывает. Verapamil and diltiazem reduce the excessive activity of fibroblasts at the stage of formation of adhesions by regulating their proliferative and collagen-synthetic functions, reducing the number of mitoses by 58.5 %, the number of Ki-67 + cells by 59.2 %, the excess production of oxyproline by 66.7 %. The normalizing effect of verapamil on fibroblasts is 1.8 times higher than that of diltiazem. Nifedipine has no antiproliferative effect on cells.
