This study did not find superiority of 3DATG-EAM image merge to guide AF ablation when compared to CT-EAM fusion. Both merging techniques result in similar navigation accuracy.
Цель. Установить клинические и электрофизиологические предикторы реци-дивирования желудочковых тахиаритмий (ЖТА) после катетерной гомогениза-ции постинфарктных рубцовых полей. Материал и методы. Включались пациенты с инфарктом миокарда (ИМ) в анамнезе и документированной устойчивыми ЖТА, несмотря на эффектив-ную терапию имплантируемым кардиовертером-дефибриллятором. Включено 72 пациента (средний возраст 64±13 лет), из них 63 мужчины с постинфаркт-ными ЖТА. В 12 случаях регистрировались "электрические штормы", потребо-вавшие экстренного катетерного лечения. У данных пациентов применялась расширенная катетерная гомогенизация рубца, то есть аблация всех каналов проведения, анормальных потенциалов и окружной аблации инфарктной зоны. Пациенты были разделены на две группы в зависимости от рецидивирования желудочковой тахикардии (ЖТ) в послеоперационном периоде. Первую группу составили 27 (37%) пациентов с рецидивами ЖТА, средний возраст которых составил 62±10 лет. Во вторую группу вошло 45 пациентов (63%) без рециди-вов тахиаритмий, средний возраст которых составил 63±12 лет. Проводилась оценка выделенных параметров с целью определения предикторов рецидиви-рования нарушений ритма сердца. Результаты. Основными клиническими предикторами рецидивирования постинфарктных ЖТА после катетерной аблации являлись давность перене-сенного ИМ и его передняя локализация. Полная площадь поверхности рубца (биполярный вольтаж менее 1,5 мВ) была сопоставима у пациентов с рециди-вом ЖТ и без рецидивов (66±51 см 2 против 82±49 см 2 ). Однако площадь поверхности плотного рубца (биполярная амплитуда ≤0,5 мВ) и процент плот-ного рубца относительно общего рубца был значительно меньше в группе рецидивов (в I группе 23±22 см 2 и 24±18%, во II группе -41±22 см 2 и 45±21%; p<0,05). Различия были выявлены при определении длины цикла клинических ЖТА в I и II группах (290±62 и 330±93 мс, соответственно, p<0,05 ). However the area of the dense scar (bipolar amplitude ≤0,5 mV) and percent of the dense scar in relation to entire scar was significantly smaller in the recurrence group (group 1 -23±22 cm 2 and 24±18%, in group 2 -41±22 cm 2 and 45±21%; p<0,05).The differences were found in the cycle length of clinical VTA assessment in groups 1 and 2 (290±62 and 330±93 ms, respectively, p<0,05). Conclusion. The analysis that was done, made it to define the following variables and VT predictors: localization of the MI (anterior vs inferior), duration of ischemic anamnesis, cycle of clinical VT, non-homogeneity of the scar, area of the fractionized potentials and delayed potentials of electrograms, area of the dense scar.
Резюме. На сегодняшний день имплантация электронных устройств, таких как кардиостимулятор или имплантируемый кардиовертердефибриллятор, нашла широкое применение в клинической практике для лечения различных видов нарушений ритма сердца. Число имплантированных устройств неуклонно растет с каждым годом. Возможные осложнения процедуры установки подобных устройств хорошо известны. В частности, электрод-ассоциированные осложнения нередки для имплантации кардиостимулятора или кардиовертерадефибриллятора. В литературе описан ряд редких осложнений: операция может осложниться кровотечением или выпотом в плевральную полость, полость перикарда. Нами описывается случай развернутого постперикардиотомного синдрома у 71-летнего пациента после операции репозиции предсердного электрода, диагностическая и лечебная тактика. Данный случай является показательным для специалистов, поскольку даже незначительная травма сердца может явиться причиной упомянутой симптоматики.
