ВступГнійний медіастиніт (ГМ) -одна з найбільш тяж-ких форм гнійної інфекції, що зустрічається у хірургіч-ній практиці. ГМ має відношення до практичної роботи загальних, торакальних хірургів, стоматологів, отола-рингологів, лікарів інших спеціальностей і виникає як ускладнення багатьох захворювань [1]. За останні роки кількість хворих із ГМ не зменшується, а високі по-казники летальності спонукають науковців приділяти особливу увагу діагностиці та лікуванню даної патоло-гії. Гнійний медіастиніт характеризується агресивним клінічним перебігом, швидким розвитком синдрому системної запальної відповіді, тяжкого сепсису з полі-органною недостатністю [2, 4]. Тяжкість перебігу ГМ зумовлена наявністю в середостінні важливих органів і анатомічних утворень -центральних симпатичних та парасимпатичних нервів, кровоносних та лімфа-тичних судин. Крім цього, клітковина середостіння інтенсивно всмоктує продукти розпаду тканин, що сприяє швидкому розвитку тяжкої інтоксикації [5, 8]. За повідомленнями ряду клініцистів, сепсис у хворих на низхідний гнійний медіастиніт (НГМ) розвивається у 45-100 % випадків, а летальність становить від 17 до 80 % [3, 5].Разом із цим можливості клінічної діагностики ГМ доволі обмежені, особливо на ранніх стадіях захво-© «Український журнал хірургії», 2017
An important component of the educational program is to ensure professional erudition, development of clinical thinking (CT), the formation of students' basic practical skills. In the year of the pandemic, it is especially difficult to really promote the development of these components of professional development in students, and modern pedagogical teaching methods, despite their effectiveness in the offline learning process, need development, improvement during distance learning. The need for the formation of CT students was determined by the founders of medicine many centuries ago, important in the creation of professional thinking was the psychological and pedagogical relationship "teacher-student-patient". Unfortunately, today this chain is lost, but the need to form analytical and exploratory thinking in medical students is extremely important, so it is necessary to revise and analyze the pedagogical methodology for modernization and use in distance learning. CT is an element of future professional activity with the help of which an algorithm of actions in specific clinical situations is created, it is the ability to identify and analyze, as well as to search and evaluate relevant information to achieve the appropriate conclusion. This article tries to conduct a conceptual analysis of the pedagogical methodology of CT formation and the possibility of their application in the conditions of distance learning of the discipline "obstetrics and gynecology". New technological programs are necessary for modeling simulation training. The main tasks of higher medical school are to achieve maximum development of intellectual abilities of the student, in the online format this can be achieved through the use of "virtual patients", situational tasks and cases, game techniques and clarity. However, methods of CT assessment and finding a strategy for its development are also important, as it is a key element of doctors' daily practice. In the 21st century, students need to think through their development, be able to work in teams, distinguish between relevant and irrelevant information from the patient, and be multilingual and highly developed so that they can be effective in all disciplines. These are the characteristics that are expected of modern medical students. However, when faced with unspecified emergencies, they are unable to make the right clinical decisions, leading to accidents. The reason for this is the lack of CT skills among young doctors, so the formation begins in school, adapting to the conditions created by the pandemic. Medical schools should teach the principles of scientific methodology and evidence-based medicine, including analytical and critical thinking, throughout the curriculum. It is important to develop a professional algorithm for the development of CT in each teacher, in order to rely on the development stimulus not only on the theoretical basis of students, but also on the ability to analyze clinical situations, and what is important to act properly algorithmically. Attention should be paid to the psychological and pedagogical aspects and approaches to the development of each student in particular. Moreover, there is no consensus on the most effective learning model for teaching CT skills and clinical considerations, and this skill is not effectively tested in medical schools, so the analysis of existing methods and their application in distance learning is extremely important.
Вступ. Гострий гнійний медіастиніт (ГГМ) залишається однією із найбільш тяжких форм загальної хірургічної інфекції. Розвиток ускладнень є одним із вирішальних факторів у перебігу захворювання. Матеріали і методи. Впродовж 2000 – 2017 рр. у торакальному відділенні Івано–Франківської обласної клінічної лікарні проходили лікування 65 хворих із ГГМ. Результати. Ексудативний плеврит та емпієму плеври виявлено у 76,9%, гнійний бронхіт – у 80%, пневмонію – у 70,7%, ендо– і міокардит – у 59,2%, гостре ураження нирок – у 24,6%, сепсис – у 100% хворих із тотальним і заднім медіастинітом. Висновки. Попередження розвитку ускладнень та їх лікування є важливою умовою успішного лікування хворих із ГГМ.
Хірургічне лікування хворих на низхідні некротичні медіастиніти С.С. Хірургічне лікування низхідних некротичних медіастинітів (ННМ) представляє найбільші труднощі серед усіх видів медіастиніту внаслідок гнійного ураження всіх відділів середостіння, вираженою ендогенною інтоксикацією, наявним сепсисом із розвитком поліорганної недостатності.Мета дослідження. Оцінити стан ендогенної інтоксикації і тяжкості стану пацієнтів та покращити результати лікування хворих на низхідний некротизуючий медіастиніт шляхом розпрацювання хірургічної тактики.Матеріали і методи. Нами проведено обстеження та лікування 73 хворих із низхідним гнійним медіастинітом, які знаходилися на лікуванні у відділенні торакальної хірургії Івано-Франківської обласної клінічної лікарні в період з 2004 по 2018 роки. Серед цих хворих у 30 (41,1%) осіб розвинувся низхідний некротичний медіастиніт (ННМ). Чоловіків було 21 (70%), жінок − 9 (30%), віком від 21 до 67 років, у середньому вік хворих становив 49,7 ± 4,1 року.Результати досліджень та їх обговорення. Всі показники ендогенної інтоксикації (ЕІ) та тяжкості стану хворих із ННМ становили критичний рівень. Тяжкий ступінь ЕІ було у 17 (56,6%), термінальний ступінь тяжкості інтоксикації -у 13 (43,4%) хворих. Рівень С реактивного білку (СРБ) був критичним і становив у середньому 368,9±32,7 мг/л. Тяжкість стану за шкалою APACHE II в модифікації А.П.Радзиховського становила 22,9 ± 1,9 бала. Згідно зі шкалою тяжкості стану хворих SOFA− рівень органних дисфункцій у хворих із ННМ був втричі більшим від показників у хворих при верхньому медіастиніті. Сепсис діагностовано у 30 (100%), септичний шок − у 23 (76,6%) хворих.Діагностичними критеріями розвитку ННМ ми вважаємо швидке прогресування гнійно-запального процесу на шиї із поширенням у середостіння, виражений індуративний набряк, швидке наростання явищ ЕІ із декомпенсацією органів і систем організму, характерний вид раневого процесу, наявність міхурців повітря, швидка поява гнійного випоту у плевральній порожнині та у перикарді.Хірургічна тактика передбачає проведення операцій на випередження поширення гнійного процесу.У 23 (76,7%) хворих ми провели бокову торакотомію та медіастинотомію, відео-асистовану торакоскопію (ВАТС) − у 7 (23,3%).Висновки. 1. При низхідному некротичному медіастиніті виявлено тяжкий та критичний ступінь ендогенної інтоксикації у 100% хворих.2. У хворих із низхідним некротичним медіастинітом спостерігали швидке прогресування гнійно-запального процесу на шиї із поширенням у середостіння та наростання ендогенної інтоксикації із декомпенсацією органів і систем організму.3. Treatment of patients with acute purulent mediastinitis (APM) is one of the most complex problems of modern surgery. Mortality in patients with APM can reach 17–80%. The aim of the work was to improve the results of treatment of patients with acute purulent mediastinitis using video-assisted thoracoscopy (VATS). During 2000–2018, 87 patients with APM were treated at the thoracic department of the Ivano-Frankivsk Regional Clinical Hospital. The main methods of surgical treatment in primary APM were lateral thoracotomy, mediastinotomy, suturing of the perforation opening of the esophagus with active drainage of the mediastinum and pleural cavity. The VATS with perforations of the esophagus (EP) with APM was applied in 9 (29.1%) patients and with secondary APM in 8 (13.8%) patients. The advantages of the VATS are minimal operational trauma, reducing the time of operation. According to the diagnosis of the EP, complicated APM, the minimally invasive surgical intervention in some cases can provide sufficient radicalism of intervention with minimal traumatism. The purpose of the VATS should be considered sanation and drainage of paraoesophageal fiber and fiber mediastinum. Thus, the choice of the method of surgical treatment of patients with acute purulent mediastinitis should be strictly individual in each case. Video-assisted thoracoscopy is a method of choice in surgical treatment of patients with acute purulent mediastinitis, which is a little traumatic method, provides a good visualization of all mediastinal units, allows for adequate detection and drainage of affected areas. Prospects for further research — improvement of methods of surgical treatment of patients with APM using the method of the VATS.
