Introduction: Since 2011, the French government estimates that about 500 French children have been born in or taken by their parents to areas where terrorist operations prevail. Since May 2017, 75 children who returned to France have benefited from a dedicated health care system. Method: This article is the result of clinical interviews conducted with 53 patients evaluated and taken care of at Avicenne Hospital in Bobigny. To our knowledge, no studies have been published on this subject. Results: A total of 32 evaluations have been completed, all of which indicated the need for care for these children. Of these children, 64% are under 5 years old, and 59% were born in France. Their clinical profiles are heterogeneous and fluctuate with time. Discussion: The multiple adverse events experienced by these children and the uniqueness of children born to families suspected by authorities of having participated in activities related to terrorism make this situation unprecedented. How can we make a diagnosis of PTSD without the help of a precise anamnesis? How can we help these children form a structuring narrative that avoids the pitfalls inherent to generalized fascination?
Aujourd’hui la référence à la psychanalyse est fortement remise en question dans le champ de la pédopsychiatrie française. On peut imputer cette évolution à différents mouvements, tant internationaux que locaux, qui ont impacté le champ de la santé mentale non seulement comme objet de soin, mais aussi comme enjeu sociétal et économique. Cet article analyse la diversité, voire l’hétérogénéité, de ces différents mouvements autant qu’il montre leur convergence. Ainsi sont successivement analysées la « mondialisation des données », la « force d’un certain modèle de rigueur scientifique », la « montée en puissance des enjeux économiques », « l’évolution des droits des patients ». Tous convergent vers une mise à l’écart d’une approche psychodynamique des troubles psychiques de l’enfant telle que les pédopsychiatres la pratiquaient depuis trente ans. Comment alors, dans ce nouveau contexte, repenser cette approche psychodynamique et comment déterminer les marges de manœuvre concrètes qui s’offrent encore à ceux qui s’en réclament, c’est ce que cet article s’efforce d’établir.
Les mineurs non accompagnés souhaitent avant tout coller à ce qu’ils perçoivent comme la demande de l’autre, qu’il s’agisse de ceux qui mandatent au pays ou des éducateurs du pays d’accueil. Cela rend difficile tout travail d’élaboration psychique qui imposerait de baisser la garde. Cette contribution expose le dispositif proposé par une consultation hospitalière transculturelle dite de deuxième ligne aux mineurs non accompagnés. La première rencontre est structurée de façon à permettre au jeune qui est reçu de se déprendre des assignations dont les jeunes migrants sont l’objet et de reprendre la parole en son nom. Dans un deuxième temps la psychothérapie permettra au jeune de travailler sur les effets psychiques du parcours migratoire traumatique et sur son histoire dans un mouvement d’aller-retour entre deux temps et deux cultures, avant de s’engager dans la construction d’une identité d’avenir.
