Evidence-based uitspraken over oefentherapie bij osteoartritis van heup of knie -de MOVE-consensus Evidence-based recommendations for the role of exercise in the management of osteoarthritis of the hip or knee -the MOVE consensus [Rheumatology 2005;44:67-7 Vervolgens is nagegaan of deze aanbevelingen ook ondersteund worden door een hoger niveau van bewijssterkte. Voor een aantal stellingen blijkt dit het geval te zijn en voor een aantal niet. Er worden in de literatuur zeer veel verschillende indelingen gehanteerd om niveaus van bewijssterkte te categoriseren. De indeling zoals door Roddy e.a. gebruikt, is ontleend aan Shekelle e.a. en wijkt enigszins af van de niveaus zoals die bijvoorbeeld door de redactie van Stimulus gehanteerd worden. Het hoogste niveau van bewijs in dit tijdschrift is een meta-analyse, vervolgens een systematic review (zonder meta-analyse), RCT, ander onderzoek, en als laagste niveau de mening van deskundigen. In de indeling van Shekelle e.a. bestaat het hoogste niveau van bewijs (niveau 1) uit meta-analyses of minimaal e´e´n RCT. Om een meta-analyse, waarbij de gegevens uit verschillende RCT's opnieuw geanalyseerd worden op hetzelfde niveau te stellen als e´e´n afzonderlijk RCT is wat ongenuanceerd. Immers, verschillende RCT's spreken elkaar soms tegen en juist in een meta-analyse wordt een overall uitkomst berekend. Een meta-analyse, waarbij er een redelijk aantal RCT's worden samengenomen, moet dan ook als een hogere vorm van bewijs worden gezien dan e´e´n afzonderlijke RCT. Systematic reviews waarbij er, vaak door heterogeniteit tussen studies, geen meta-analyse kon worden gedaan, komen ten onrechte in de indeling van Shekelle e.a. niet voor. Ook kan eraan getwijfeld worden of een clinical trial zonder randomisatie een hogere vorm van bewijs is dan bijvoorbeeld een case-control studie. Kortom, in grote lijnen geeft de indeling die door Roddy e.a. gebruikt is een hie¨rarchie aan van hoge naar lage bewijssterkte, maar over de indeling kan de mening verschillen. Dit doet overigens weinig af aan het artikel van Roddy, waarbij het gaat om de algemene vraag of de mening van deskundigen ondersteund wordt door hogere vormen van bewijs. SamenvattingDoelen. Oefentherapie is een effectieve en veelvuldig voorgeschreven interventie voor osteoartritis (OA) van de onderste extremiteiten. Er zijn echter nog veel vragen onbeantwoord over de praktische aanbieding van oefentherapie. Wij hebben evidence-based aanbevelingen opgesteld om gezondheidszorgbeoefenaren een richtlijn te geven.Methoden. Om de richtlijn te ontwikkelen werd er een multidisciplinaire groep gevormd uit representanten van professionele instanties waarvoor OA relevant is en andere geı¨nteresseerde partijen. Elke participant droeg maximaal tien stellingen aan die essentie¨le klinische zaken beschreven met betrekking tot oefentherapie voor OA van de heup of de knie.
