ZusammenfassungEine Hämaturie wird in der kinderärztlichen Praxis regelmäßig diagnostiziert. Ziel dieses Artikels ist es, dem niedergelassenen Kollegen einen diagnostischen Leitfaden zu geben. Man unterscheidet zwischen Makrohämaturie und Mikrohämaturie. Zahlreiche erworbene oder hereditäre Erkrankungen führen zu einer Hämaturie. Entscheidend für die differenzialdiagnostische Abklärung ist die Differenzierung zwischen glomerulärer und nicht glomerulärer Hämaturie.Die Diagnostik beruht auf Anamnese, körperlicher Untersuchung und Urinuntersuchung. Der Urinteststreifen stellt einen einfachen und sensitiven Test dar, ist jedoch wenig spezifisch. Jeder Verdacht auf Hämaturie sollte mikroskopisch gesichert werden. Die Beurteilung der Erythrozytenmorphologie ermöglicht, zwischen glomerulärer und nicht glomerulärer Hämaturie zu unterscheiden. Dysmorphe Erythrozyten sind ein Hinweis auf das Vorliegen von Glomerulopathien. Zur genauen Differenzierung sind weitere laborchemische Untersuchungen sowie eine Nierenbiopsie erforderlich. Diese sollte bei anhaltender Proteinurie, arterieller Hypertonie oder eingeschränkter Nierenfunktion durchgeführt werden.Der mikroskopische Nachweis von eumorphen Erythrozyten erlaubt, eine glomeruläre Erkrankung auszuschließen. Sonografie, Urinkultur und Untersuchung des Urins auf lithogene Substanzen liefern entscheidende Hinweise auf die Ursache der nicht glomerulären Hämaturie.
