Die heutigen Vorstellungen über den Prozeß der spontanen rhythmischen Reizbildung im Herzen sind in Abbildung 1 zusammengefaßt. Die Elektrophysiologie hat sie insbesondere mit der intrazellulären Ableitungstechnik gewinnen können. Das Potential zwischen Zellinnerem und Zelläußerem, das Membranpotential oder Transmembranpotential, beträgt 90 Millivolt (mV). Durch die Erregung wird die Membran auf etwa + 20 mV umgeladen. Diese Depolarisation ist Folge des Natriumeinstroms in die Zelle. Nach etwa plateauförmigem Verlauf kommt es zur Repolarisation. Kalium strömt aus der Zelle aus. Das ursprüngliche Membranruhepotential wird wieder erreicht. Während der Diastole bleibt in den Fasern des Arbeitsmyokards die Transmembranspannung konstant. In den Fasern des spezifischen Muskelsystems fällt sie dagegen während der Pause kontinuierlich. Bei Erreichen eines kritischen Schwellenwertes, der Reizschwelle, klinkt dieses diastolische Generatorpotential oder Schrittmacherpotential die Erregung aus. Das Herz ist durch diesen Mechanismus zur spontanen Reizbildung und damit zur rhythmischen Kontraktion in der Lage. Alle Manuskripte sind direkt an die Schriftleitung zu richten.
