Ein Problem bei der permanenten Implantation von Demand-Schrittmachern kann der Synchronisationsverlust darstellen,d.h. das Nichterkennen von Spontanaktionen und der damit bedingte Ausfall des Demand-Mechanismus. Das intrakardiale EKG kann durch die Eigenschaften des Schrittmachers in Amplitude und/oder Form so verändert werden, daß die Inhibierung des Ausgangsimpulses erfolgt. Abbildung 1 zeigt den Signalfluß für ein implantiertes System. Störungen in der Synchronisation können sich ergeben durch : l . Belastung des IEKG durch den Eingangswiderstand des Schrittmachers 2. Formveränderungen, infolge der Frequenzabhängigkeit von Über-gangsimpedanz und Eingangswiderstand des Schrittmachers. 3. Die Filtercharakteristik im Schrittmacher. 4. eine zu geringe Amplitude des Signals selbst. Belastung des IEKG durch den Eingangswiderstand des Schrittmachers Je geringer die Eingangsimpedanz des Schrittmachers ist, umso grös-ser wird der Stromfluß in diesem Kreis, umso größer aber auch der Spannungsabfall Über der Übergangsimpedanz, so daß die für den Schrittmacher verbleibende Amplitude geringer wird. In der Abb. 2 sind gemessene Eingangsimpedanzen dargestellt; im physiologisch interessierenden Frequenzbereich wirken sie als reine Widerstände. Beispiele für die Veränderung eines intrakardialen EKG durch Widerstandsbelastung, aufgenommen während der Implantation, zeigt Abb. 3. Gegenüber dem unbelasteten IEKG (l MO) mit einer Amplitude (Spitze-Spitze-Wert) von 13mV trat eine Reduktion auf 10,3mV (80%) bei 10KQ, 9,5mV (73%) bei 7KQ und 8,1mV (62%) bei 5 KQ Last ein. Die Widerstandsabhängigkeit von lEKG-Amplituden wurde für eine Reihe von Patienten aufgenommen. Für 5-8mm2 Elektroden bei Neuimplantationen sind die gemessenen Werte graphisch dargestellt (Abb. 4). Dem Diagramm ist zu entnehmen, daß der Eingangswiderstand von Schrittmachern mindestens 50KQ groß sein sollte, damit ein Amplitudenverlust von mehr als 10% auch im ungünstigsten Fall vermieden werden kann.
