RESUMO -OBJETIVO. Avaliar a etapa de ingestão da fagocitose e do metabolismo oxidativo de neutrófilos em crianças portadoras de desnutrição moderada.MÉTODOS. Foi analisado o sangue periférico de vinte e duas crianças portadoras de desnutrição moderada, sem infecção, comparando-se os resultados aos de vinte crianças eutróficas da mesma faixa etária (2 a 5 anos de idade). A ingestão fagocitária por neutrófilos foi avaliada através da ingestão de partículas de zimosan e o metabolismo oxidativo avaliado pela redução do nitro blue tetrazolium entre um número fixo de 200 neutrófilos. INTRODUÇÃODesde a Idade Média era conhecida a associação entre desnutrição e infecção: os portadores de peste bubônica eram reconhecidos pela desnutrição. O mesmo ocorreu no início deste século em relação à tuberculose. Só em 1968 é que Scrimshaw 1 comprovou que a desnutrição propiciava à infecção, observando uma diminuição da mortalidade infantil por sarampo e diarréia após uma única modificação feita durante cinco anos numa aldeia da Guatemala: a suplementação alimentar com farinha de peixe. Iniciaram-se então os estudos sobre a imunocompetência na desnutrição.Pesquisas mostraram que os órgãos linfóides centrais e periféricos apresentam hipotrofia ou até mesmo atrofia na desnutrição. Foi observada hipotrofia em timo, nódulos linfáticos, baço e tonsilas palatinas 2,3 . A resposta humoral sérica por IgM, IgG e IgA encontra-se conservada nestes pacientes ou até mesmo elevada quando os pacientes apresentaram infecções de repetição. A IgA secretora acha-se diminuída 4 . Resposta celular está diminuída em pacientes com desnutrição moderada: diminuição de linfócitos timo dependentes (T) 5,6 , de células CD4 positivas, levando a uma tendência à inversão da relação CD4/CD8 na desnutrição 7 . A fagocitose e a quimiotaxia de fagócitos mononucleares está reduzida na desnutrição moderada 8,9 . Todas as pesquisas relatadas foram realizadas em desnu- tridos sem infecção, uma vez que a infecção por si só pode levar a alterações na resposta imune.Em nosso meio praticamente não existem estudos sobre avaliação dos neutrófilos em desnutridos. Por estas razões, pela grande incidência da desnutrição no Brasil e pela importância dos polimorfonucleares neutrofílicos na defesa individual, é que nos propusemos ao presente estudo.
