When cancer patients have advanced disease and a primary cure is no longer possible, the focus is on maintaining the patient's quality of life. Recent therapeutic advances in breast cancer treatment mean that even patients with metastatic disease can remain stable for long periods of time. The aim of this study was to look at the health-related quality of life (HRQL) of these patients and compare it with data for the general population and to show the differences in outcomes for different survey instruments used to measure quality of life. A total of 96 breast cancer patients with metastatic disesae or receiving adjuvant therapy were questioned about their quality of life. Patients were investigated using the established survey instruments EORTC QLQ-C30, EORTC QLQ-BR23, EQ-5D-5L and EQ VAS. All patients filled out questionnaires. Statistical analysis was done using MS Excel and SPSS. Although the questionnaires were completed at the same time, the different questionnaires showed significant differences with regard to the level of stress experienced by the patient. When the EQ VAS questionnaire was used, the patient's current state of health was assessed as significantly better than with the EORTC QLQ-C30. Overall, all aspects of patients' quality of life were found to be in need of optimization and HRQL of patients was significantly poorer in all areas compared to the reference population. To improve the quality of life of patients with metastatic disease, it is necessary to continuously monitor the success of therapy. The choice of survey tools is highly relevant as assessments differ considerably depending on the choice of questionnaire.
Genitale Fehlbildungen haben in der weiblichen Gesamtbevölkerung eine Inzidenz von 0,1 -5 %. Bei der Abklärung von Infertilitätskollektiven wird eine deutlich höhere Inzidenz von 3,5 -6,5 % gefunden [1 -4]. Die höchste Rate mit bis zu 38 % an Abnormitäten der Müller-Gänge wurde in einem Kollektiv von Patientinnen mit habituellen Aborten entdeckt [5]. Aufgrund der räumlichen Nähe von Organstrukturen während der Embryonalperiode können zusätzlich zu den uterinen Fehlbildungen auch vaginale, zervikale Abnormitäten und Fehlbildungen der Adnexe sowie anderer Organe wie u. a. Nieren oder Skelett auftreten. ÄtiologieEine Hemmungsfehlbildung der Müller-Gänge wird nach derzeitigem Wissensstand als Ursache genitaler Malformationen (Aplasia oder inkomplette Fusion der Gänge) gesehen, die sich u. a. auch in Syndromen mit assoziierten Fehlbildungen widerspiegelt. Als Beispiele seien das Mayer-Rokitansky-Küster-Hauser (MRKH)-Syndrom mit kompletter Vaginal-und Zervixaplasie bei rudimentären Uterushörnern, das Wolf-Hirschhorn-Syndrom mit assoziierter Uterusaplasie oder das Bardet-Biedl-Syndrom assoziiert mit Uterus bicornis und Vaginalseptum genannt. Bei diesen Syndromen sind keine definitiven genetischen Ursachen bekannt. All diese Syndrome können mit einer Reihe anderer, zum Teil gravierender Fehlbildungen verbunden sein [6]. Die Hypothese, dass eine Aktivierung des Anti-Müller-Hormons (AMH) zur Regression der Müller-Gänge und somit zur Vaginalatresie bzw. zur Uterusagenesie führt, konnte für das MRKH-Syndrom nicht belegt werden. In den genetischen Untersuchungen konnten keine Deletion oder Polymorphismen in der Promotorregion der AMH ausgemacht werden. In der biochemischen Bestimmung des Anti-Müller-Hormons von betroffenen Patientinnen zeigte sich keine erhöhte Serumkonzentration, so dass eine AMH-Überexpression ebenfalls nicht vorliegen kann [7]. Genetische Mausstudien, im Besonderen Knock-out-Mausmodelle, zeigten, dass vor allem die Hox-Gene, hierbei speziell Hox-A9, -A10, -A11 und -A13, eine essenzielle Rolle bei der Entwicklung des Urogenitales spielen. Durch mehrere Mausmodelle konnten dabei Hox-A10 für die Uterusentwicklung, Hox-A11 für den kaudalen Uterus-und Zervixanteil, Hox-A13 für die obere Vagina und Hox-A9 für die Eileiterentwicklung als essenziell identifiziert werden [8, 9]. GebFra-Refresher Bei uterinen Malformationen sind auch Abnormitäten und Fehlbildungen von Organen der unmittelbaren Nachbarschaft zu beachten. Genitale Malformationen sind die Folge einer Hemmungsfehlbildung der Müller-Gänge. Für Syndrome mit assoziierten Fehlbildungen sind bisher keine eindeutigen genetischen Ursachen gefunden worden. Die Entwicklung des Urogenitales wird vor allem durch Produkte der Hox-Gene bestimmt.
Vaginale Fehlbildungen werden normalerweise oft erst in der Pubertät bzw. zu Beginn der Geschlechtsreife festgestellt, uterine Fehlbildungen oft erst in der reproduktiven Lebensphase. Bei den üblichen Kinder-Vorsorgeuntersuchungen U1-U9/J1 wird zunächst die Vitalität des Neugeborenen und das Vorhandensein äußerer Fehlbildungen bzw. Magen-Darm-Passagestörungen untersucht. Die U2 beinhaltet vor allem ein Stoffwechselscreening. Die weiteren Untersuchungen beurteilen im Wesentlichen die motorische und geistige Entwicklung, das Verhalten sowie das Hörund Sehvermögen. Die neue J1 beinhaltet zwar eine körperliche Untersuchung, ist aber im Wesentlichen ausgelegt auf ein Beratungsgespräch, bei dem Themen wie z. B. Alkohol, Drogen, Rauchen und Sexualität besprochen werden. Dies bedeutet, dass bis auf Fehlbildungen des äußeren Genitales, wie z. B. eine Labiensynechie, eine Klitorishypertrophie (z. B. im Rahmen eines AGS-Syn
