Резюме.Цель работы -определить прогностическое значение индекса трансмуральной дисперсии реполяризации у детей с аномально расположенными хордами левого желудочка и оценить вклад этого показателя для диагностики на-рушений сердечного ритма. Материалы и методы. Было обследовано 159 детей в возрасте 0-17 лет. Результаты и обсуждение. Установлено, что у детей с сочетанными аномально расположенными хордами левого желудочка значение индекса трансмуральной дисперсии реполяризации выше по сравнению с пациентами с изо-лированным характером микростигмы (p<0,03). Риск наличия гемодинамически значимых атриовентрикулярных блокад на фоне данной сердечной аномалии выше (р<0,0001) у детей младшего возраста, при низких значениях интервала Tpeak-Tend (Tp-e) и высоких показателях √RR; Tp-e/QT; PQ; QT; QTp. Вероятность регистрации гемо-динамически значимой синоаурикулярной блокады (р<0,001) выше у мальчиков, преждевременно родившихся, при повышении показателей √RR и Tp-e/QTc, увеличении длительности интервалов QTc, QTp. Наибольший риск наличия частой наджелудочковой экстрасистолии (р<0,05) имеют девочки при укорочении интервалов Tp-e и QTp и повышении показателя Tp-e/QT на электрокардиограмме. To determine the prognostic value of the transmural dispersion repolarization index in children with left ventricular false tendons and to evaluate the contribution of this indicator to the diagnosis of cardiac arrhythmias. Material and methods. 159 children aged 0-17 years have been examined. Results. It has been found that the transmural dispersion repolarization index is higher in children with the combination of minor cardiac abnormalities and left ventricular false tendons compared to patients with the isolated character of microstigma (p<0,03). The risk to develop hemodynamically significant atrioventricular blocks is higher (р<0,0001) in toddlers with left ventricular false tendons, at low values of Tpeak-Tend interval (Tp-e) and high indices of √RR; Tp-e /QT; PQ; QT; QTP. The probability of detecting hemodynamically significant sinoatrial blocks (p <0,001) is higher in boys born prematurely, in children with the increasing of √RR and Tp-e/QTc indices, increased duration of QTc, QTp intervals. Цель работы: установить характер нарушений сердечного ритма и проводимости у детей с малыми аномалиями сердца (МАС) при электрокардиографическом исследовании и холтеровском мониторировании (ХМ); разработать модель для прогнозирования синдрома внезапной смерти (СВС). Материал и методы. Обследовано 168 детей в возрасте 2-17 лет. Изучена летальность детей от СВС за 2008-2017гг. в г. Гродно. Результаты. Установлено, что при ХМ у детей с МАС нарушения сердечного ритма и проводимости выявляются достоверно чаще, чем на ЭКГ, при отсутствии МАС различия отсутствуют. Построена модель, позволяющая прогнозировать СВС у детей, имеющих аномально расположенные хорды в левом желудочке (АРХЛЖ) (чувствительность 100%, специфичность 100%). Выводы. Детям с МАС с целью выявления наличия аритмий необходимо проводить ХМ. Применение дискриминантного уравнения позволит выполнять целенаправленный отбор детей с АРХЛЖ в группу высокого риска СВС.
