O objetivo desta pesquisa, de caráter exploratório, com abordagem qualitativa, foi aprofundar o conhecimento sobre a inserção do enfermeiro na equipe interdisciplinar do Centro de Atenção Psicossocial, considerando que essa inserção se reflete em suas possibilidades de cuidar do cliente. Os dados foram coletados através de entrevistas, realizadas de dezembro/2004 a janeiro/2005, com três enfermeiros e dois coordenadores de Centro. Os campos da pesquisa foram: um Centro Psicossocial de Saúde Mental e dois Centros de Atenção Psicossocial para Usuários de Álcool e Drogas, situados: dois no Estado do Rio de Janeiro e um no Estado de São Paulo. Foram atendidas as exigências éticas e científicas da Resolução 196/96. A análise dos dados, de acordo com a categorização empírica, apontou como importantes a questão da formação adequada do enfermeiro para o trabalho nesses Centros e as dificuldades referentes às mudanças que ocorrem nas relações entre os membros de uma equipe interdisciplinar.
ResumoEste é um relato de experiência apresentado por professores da subárea Promovendo e Recuperando a Saúde Mental, da Faculdade de Enfermagem da Universidade do Estado do Rio de Janeiro. A subárea é oferecida em oito do total de nove períodos do curso de Graduação, integrando-se aos temas das demais subáreas em cada período. Partindo dos saberes do aluno sobre os vários temas e conectando-os posteriormente à teoria, procura-se desenvolver sua sensibilidade e empatia, com o objetivo de fortalecê-lo em seu papel de cuidador da clientela nos diferentes campos. Descritores:currículo; ensino de enfermagem; graduação em enfermagem; saúde mental Considerações iniciaisO currículo integrado da Faculdade de Enfermagem da Universidade do Estado do Rio de Janeiro, construído coletivamente e implantado em 1996, fundamenta-se na Teoria Crítica da Educação e utiliza uma pedagogia problematizadora.Composto de áreas e subáreas que se articulam ao longo dos nove períodos do curso, propõe um processo ensino / aprendizagem que integre os vários saberes sobre o homem e sua saúde e que valorize o aluno como sujeito, portador de saberes que devem ser respeitados e superados a partir do desenvolvimento da curiosidade epistemilógica (1) . Este trabalho relata nossa trajetória na subárea Promovendo e Recuperando a Saúde Mental (a partir de agora designada, neste texto, como Saúde Mental), que no currículo integrado foi inicialmente oferecida em quatro períodos passando depois a ser trabalhada em oito do total de nove períodos da Graduação, não só conectando internamente seus vários temas, mas, em cada período, integrando-se com os temas trabalhados nas demais subáreas.Vale ressaltar que, anteriormente à implementação do currículo integrado, já vínhamos caminhando nesse sentido: até a década de 80, oferecíamos as disciplinas Psicologia aplicada à Enfermagem, em um período, ministrada por psicólogo, e Enfermagem Psiquiátrica, em dois períodos (um de ensino teórico-prático e outro de estágio supervisionado). Na década de 80, além das duas disciplinas, iniciamos o ensino da Saúde Mental, voltado para o indivíduo sadio com suas dificuldades e crises (também ensino teórico-prático em um período e estágio em outro); o estágio dessa disciplina integrava os conteúdos da saúde mental às atividades desenvolvidas no hospital geral. Em 1993, optamos por extinguir a disciplina Psicologia aplicada à Enfermagem, cujos conteúdos foram assimilados pela Saúde Mental, e começamos a usar uma nova metodologia, utilizando dinâmicas de grupo e valorizando os saberes da experiência de vida dos estudantes.
A enfermagem vem evoluindo e adquirindo identidade própria, através da elaboração e implementação de metodologias assistenciais, a partir de teorias que lhe dão base científica. Observa-se, entretanto, que a Enfermagem Psiquiátrica não participa desta transformação, mantendo-se à margem. Paralelamente, observa-se que a participação do enfermeiro nas transformações da assistência psiquiátrica é pequena. A partir destas afirmativas colocam-se algumas questões: o enfermeiro psiquiátrico, valoriza seu papel como agente terapêutico? O enfermeiro psiquiátrico sente necessidade de enfermagem planejada? Possui conhecimentos científicos e vê como aplicar a enfermagem planejada em Psiquiatria? Ou a enfermagem planejada não se aplica à Psiquiatria? O presente trabalho se propõe a: 1) analisar estas questões, bem como suas causas, a partir dos dados obtidos em pesquisa realizada em instituições psiquiátricas no Rio de Janeiro; 2) oferecer subsídios para a aplicação da Enfermagem Planejada em Psiquiatria.
Background Heart failure represents a common cause of hospitalization associated with poor short-term clinical outcomes. Little is known about the long-term prognosis of these patients in Latin America. Methods The rationale and design of the study were previously published (1). Briefly, BREATHE was the first nation-wide prospective observational study that included patients hospitalized due to acute heart failure in Brazil. In-hospital management as well as 12-month clinical outcomes were assessed. Patients were included during two time periods: from February 2011 to December 2012 (BREATHE I) and from June 2016 to July 2018 (BREATHE Extension). Adherence to evidence-based therapies was also evaluated. Results A total of 3,013 patients were included in 71 centers in Brazil. The median follow-up was 346 days. The BREATHE population included 39.3% of women, had a mean age of 65.2 (± 15.6) with a mean ejection fraction of 39.7% (± 17.5). Among the comorbidities, systemic arterial hypertension was the most common, present in almost 75% of the sample. At hospital admission, 83.8% of patients had clear signs of pulmonary congestion and the main cause of decompensation was poor adherence to heart failure medications, representing 27.8% of cases. Among patients with reduced ejection fraction, the concomitant use of renin-angiotensin-aldosterone inhibitors, beta-blocker and spironolactone at hospital discharge was 44.5% and decreased to 35.2% after 3 months (p<0.01). Mortality rate at 12 months was 28.9 for every 100 patient years with 26.2% readmission at 90 days and 46.4% at 365 days. The most common etiology of heart failure was ischemic disease (Figure 1) but the worst prognosis was associated with Chagas disease (Figure 2) including an analysis of a composite outcome encompassing death, myocardial infarction, stroke or cardiac arrest after discharge. Conclusions In this large national prospective registry of patients hospitalized with acute heart failure, mortality and readmission were higher than what have been reported globally. Poor adherence to evidence-based therapies was a common both at hospital discharge and 1-year of follow-up. Funding Acknowledgement Type of funding sources: Other.
